[Clinical significance of bacterial beta-lactamase production in the treatment of urinary tract infections with oral beta-lactam-antibiotics].

In 649 urological patients with urinary tract infections (UTI) a total of 1114 bacterial strains were isolated and their beta-lactamase production was investigated by a qualitative, micro-iodometric method. Approximately 40% of all strains, 33% of E. coli species and 8% of Proteus mirabilis species were enzyme producers. All of the isolates of E. coli and Proteus mirabilis defined as non-producers were sensitive to ampicillin and cephalothin in the agar diffusion test. The beta-lactamase producers of these two species did not show the same pattern: almost all of them were ampicillin-resistant, but most were cephalothin-sensitive. The other species were all found to be either producers or non-producers. Therapeutic comparisons in altogether 138 UTI patients treated with ampicillin or cephalexin, two antibiotics with different beta-lactamase stability, confirmed the positive correlation between enzyme production and therapeutic failure in both groups. Cephalexin, however, tended to be more effective than ampicillin against enzyme-producing species.

[Pronounced anterior displacement--a newly defined intraventricular heart conduction disorder].

A pronounced anterior displacement in the VCG which was not evoked by a posterior infarction or a right-ventricular hypertrophy has its cause in a circumscribed disturbance of the conduction of the septal or medial fascicle of the left Tawara-branch of the conduction system of the heart. On the basis of personal observations of this new defined disturbance of the conduction system the parameters for the recognition in the 12 conventional leads of the ECG are demonstrated. Such a pronounced anterior displacement may be supposed in Rv2 greater than 1 m V and Rv2/Sv2 greater than or equal to 1.2. The analysis of an additional 57 VCG with left anterior hemiblock yields in 6 the additional proof of an amphysized anterior orientation. This shows that the group of the left-anterior hemiblocks is not unique and pleads for the enlargement of Rosenbaum's conception of the conduction system in the sense of a functional division into three parts of the left Tawara-branch.

[Diagnosis of space-occupying renal processes: sonography and aimed fine-needle biopsy].

It is reported on 3 cases in which by sonographic examination a tumorous process could be established. In all cases the additional investigation by fine needle biopsy and cytological examination proved the presence of malignant cells. This local renal process proved by sonography could be verified neither by i.v. pyelographic nor arteriographic methods. For two patients the diagnosis based on sonography, fine needle puncture and cytological test could be verified by operation. The third patient was not operated in spite of malignant cells in the cytogram because of negative computer tomography provided by an external hospital.

[The normal pancreas. Demonstration in the sonogram in relation to age].

For 191 patients without any clinically or anamnestically traceable disease of pancreas sonographic inspection was performed, a real time-equipment type Aloka, SSD 240 was used. The age of the patients, divided into 5 groups ranged from 15 to 90 years. The measurements of the pancreas sonographic inspection was performed, a real time-equipment type Aloka, SSD 240 was used. The age of the patients, divided into 5 groups ranged from 15 to 90 years. The measurements of the pancreas were established for head, neck, corpus and tail respectively. The following mean values for the thickness were found: Head: 2,41 +/- 0,41 cm; neck: 1,53 +/- 0,34 cm; corpus: 1,90 +/- 0,37 cm; tail: 1,67 +/- 0,37 cm. The width of the organ was in the area of the head: 1,3 to 3,5 cm, neck 1,0 to 2,7 cm, corpus 1,1 to 3,0 cm, tail 0,8 to 2,6 cm. A comparison between normal weight- and overweight-patients showed no significant differences at all. The echostructure of the pancreas showed a clear increase of intensity with progress in age. The reason for this more detailed echostructure is seen in an infiltration of fat as well as collagenous and fibrous elements. No change in organ size was found in this connection.

[Duodenal ulcer in the aged. Diagnosis, therapy and disease course].

In 68 of 1152 patients older than 65 years an acute ulcus duodeni could be stated by endoscopy. This is 5,9% of the examined patients. Among this group there was a remarkably great number of patients who belonged to the age group between 70 and 80 years. The anamnesis of 42 patients indicated complaints for several years, whereas 26 patients showed symptoms only for a few weeks. The size of the acute ulceration was mostly below 1,0 cm. A determined relation between the size of the ulceration, the patients age and the duration of their anamnesis could not be found. In 12 patients an acute ulcus ventriculi could be proved simultaneously. In 15 cases acute bleeding was the initial symptom of the ulceration. Healing of the ulcera took between 4 weeks and 3 months. The conservative therapeutical steps were not standardized. Stationary and ambulant patients did not show any difference in the time of healing. It was remarkable that the consumption of alcohol and especially of cigarettes was relatively high.

Participation of deoxyribonucleic acid polymerase alpha in amplification of ribosomal deoxyribonucleic acid in Xenopus laevis.

Aphidicolin, a known inhibitor of eucaryotic deoxyribonucleic acid (DNA) polymerase alpha, efficiently inhibited amplification of ribosomal DNA during oogenesis in Xenopus laevis. DNA polymerase alpha, but not DNA polymerase gamma, as isolated from ovaries, was sensitive to aphidicolin. DNA polymerase beta was not detectable in Xenopus ovary extracts. Therefore, DNA polymerase alpha plays a major role in ribosomal ribonucleic acid gene amplification.

Isolation of a SV40-like virus from a patient with progressive multifocal leukoencephalopathy.

A SV40-like virus was isolated from the brain of a patient with progressive multifocal leukoencephalopathy. The virus was antigenically and serologically indistinguishable from SV40 wild type. A unique difference from SV40 was its ability to grow on human glial and CV-I monkey cells. The molecular weight of the viral DNA was very similar to that of SV40 DNA and the DNA cleavage patterns obtained after digestion with Hind II and Hind III restriction endonucleases were indistinguishable from those of SV40.

Demonstration of SV40-related tumour antigen in human meningiomas by different hamster SV40-T-antisera.

Three of 5 human meningiomas tested in cell culture expressed SV40-related tumour (T) antigen as measured by indirect immunofluorescence with four different hamster anti-SV40 tumour sera. The per cent of T-antigen-positive nuclei varied depending on the antisera used. In one case (meningioma G. S.), only 25% of the T-antigen-positive nuclei detected with one antiserum could be demonstrated with another serum. The possible explanations are discussed. None of the meningioma cell cultures tested expressed SV40-related viral capsid (V) antigen.

Mitochondrial DNA replication does not involve DNA polymerase alpha.

Aphidicolin, a known inhibitor of DNA polymerase alpha, is a potent inhibitor of nuclear DNA synthesis in HeLa cells but has no effect on the replication of mitochondrial DNA. Parallel experiments with mitochondria incubated in vitro also show no inhibition of DNA synthesis by aphidicolin; however, DNA synthesis in these isolated mitochondria is completely blocked by dideoxycytidine triphosphate, which inhibits DNA polymerase gamma but not the alpha polymerase. The replication of mitochondrial DNA therefore requires only one DNA polymerase of the gamma type.
