Plasma volume and proteins in voluntary hyperventilation.

In a controlled study the changes of the plasma volume and plasma proteins induced by voluntary hyperventilation (HV) were investigated in nine splenectomized volunteers. The plasma volume changes were calculated from the changes of the hemoglobin and hematocrit. After 20 min of HV in supine position, which lead to a decrease of the venous CO2 partial pressure by 19 Torr and to an increase of plasma epinephrine and norepinephrine levels, the plasma volume was reduced by 12.9%. The intravascular masses of total protein, albumin, and several other proteins decreased during HV but a similar decrease of these proteins was also observed during the control study (C), i.e., rest in supine position without HV. The differences between changes during HV and C were not significant, indicating that the loss of protein was not due to HV. It is concluded that acute HV leads to a rapidly reversible loss of a virtually protein-free solution from the vascular space. The red cell compartment participated in fluid shifts in that the mean red cell volume decreased by 2.2% (P less than 0.02 compared with C). Comparison with earlier work shows that addition of erythrocytes from the normal spleen does not play a part in the HV-induced increase of hemoglobin and hematocrit.

[Effect of combined treatment with cisplatin and irradiation on the survival of human melanoma cells].

During a combined treatment of human melanoma cells with Cisplatin and irradiation under aerobic conditions, interaction between the two treatment modalities is observed. The presence of Cisplatin during irradiation leads to an increased bending, pretreatment with Cisplatin to an increased initial steepness of the survival curve irrespective of the time interval and the Cisplatin dose. Post-treatment with Cisplatin, however, does not influence the shape of the survival curve. Simultaneous treatment of human melanoma cells with Cisplatin and irradiation in hypoxia does not result in the radiosensitization seen under aerobic conditions.

[Effect of cisplatin on the recovery from sublethal and potentially lethal radiation damage in cell culture].

The effect of Cisplatin upon the recovery from sublethal damage during fractionated irradiation and upon the recovery from potentially lethal damage after a single dose has been investigated. In three mammalian cell lines, Cisplatin did not influence the recovery from sublethal and potentially lethal damage. There were differences among the three cell lines in their ability to recover from radiation damage. However, a significant quantitative difference between the recovery from sublethal and potentially lethal damage in any one cell line could not be found.

The effect of etophylline clofibrate on HDL subfractions.

The effect of etophylline clofibrate on lipids and apolipoproteins of the high density lipoprotein (HDL) subfractions HDL2 and HDL3 as well as on very low density (VLDL) and low density lipoproteins (LDL) and the post heparin lipolytic activities (PHLA) of lipoprotein lipase (LPL) and hepatic triglyceride lipase (HTGL) has been studied in 14 patients with type II hyperlipoproteinemia (HLP). The study was preceded by a 4-week washout phase, followed by a 6-week placebo period. During the next 12 weeks, the patients received 750 mg etophylline clofibrate per day. Then the drug was again replaced by placebo for another 6 weeks. During the study the patients were on a low fat diet poor in cholesterol with a P/S ratio over 1.0. HDL cholesterol and apoproteins increased significantly during treatment. In the first verum phase this effect was related to the rise in HDL2 components with minor changes in HDL3 concentrations, whereas in the second verum period a distinct increase of the HDL3 components could be detected. This development was accompanied by a significant increase of the LPL activities during the first 6 weeks of treatment, followed by a decrease to initially measured values after 12 weeks. The drug lowered plasma- and LDL-cholesterol levels by 19% and 22%, and plasma and VLDL triglycerides by 22% and 25%, respectively. VLDL-C apoproteins (C-I, C-II, C-III) declined by 31% with a percentage increase of apo C-II compared with apo C-I and apo C-III.

Anticipatory coarticulation in a patient with apraxia of speech.

Articulatory anticipation of vowel gestures was assessed in an apraxic patient, a dysarthric patient, and three normal speakers. The technique of assessment included perceptual identification of gated speech stimuli. The speech material consisted of /getVte/ utterances with the target vowels /i/, /y/, and /u/. In the case of the apraxic patient the gated vowels were identified at a later instant relative to the preceding plosion burst than in the normal speakers. This result was interpreted as reflecting a delayed onset of coarticulatory gestures, in particular lip rounding. The identification rates for the dysarthric's vowels rather reflected the general reduction of this patient's vowel space.

[Positive modification of hemodynamics in post cardiac surgery patients by hemofiltration. Improved method for the demonstration of myocardial depressant factor (MDF) in hemofiltrate].

The method of haemofiltration was used in 29 postoperative cardio-surgical patients with low blood pressure, high left ventricular filling pressure and low total peripheral resistance, which did not respond to the use of the intraaortic balloon pump or pharmaco-therapy. In severe low-output syndrome hemodynamic parameters are: reduced mean arterial pressure, increased left ventricular filling pressure, increased mean arterial pulmonary pressure as well as significantly reduced total peripheral resistance [4-6, 28]. Inspite of pharmaco-therapy, cardiac performance regarding peripheral perfusion is insufficient; this leads to a vicious cycle of irreversible O2-debt and severe cellular damage. After haemofiltration there was a significant improvement in the haemodynamic parameters, which in our opinion was due to the elimination of toxic peptides such as Myocardial Depressant Factor (MDF). In 27 of a total of 29 patients, haemodynamic parameters returned to normal after treatment. 19 patients were discharged, eight patients died after a number of days or weeks from causes not related to the original cardiogenic shock (cerebral embolism, reinfarction, myocardiopathy and pneumonia).

[Effect of combined treatment with cisplatin and radiation on the survival of Chinese hamster cells].

During combined treatment of Chinese hamster cells with cisplatinum and irradiation under aerobic conditions, there appear interactions between the two treatment modalities depending on the treatment sequence and the time interval. Treatment with cisplatinum followed by irradiation leads to a reduction of the shoulder of the survival curve with increasing time interval. Simultaneous treatment with cisplatinum and irradiation under aerobic or hypoxic conditions does not change the survival curve. Treatment with cisplatinum under aerobic conditions followed by irradiation in hypoxia does not lead to any interaction of both modalities independent of the time interval in contrast to subsequent irradiation under aerobic conditions. The specific sensitization of hypoxic cells by cisplatinum towards irradiation described in the literature could not be demonstrated with our cell line.

[Effect of combined treatment with cisplatin and irradiation on the survival of kidney cells in the Syrian hamster].

During combined treatment of hamster kidney cells with cisplatin and irradiation under aerobic conditions, there appear interactions between the two treatment modalities. The presence of cisplatin during irradiation leads to an increased bending, pretreatment or posttreatment with cisplatin to a gradual adaption of the shape of the survival curve to a straight line. Simultaneous treatment of hamster kidney cells with cisplatin and irradiation in hypoxia does not change the survival curve in contrast to simultaneous treatment under aerobic conditions.

Buflomedil in arterial occlusive disease: results of a controlled multicenter study.

In a double-blind, randomised multicentre study buflomedil, a vasoactive substance, was compared with placebo in the treatment of 93 patients with chronic arterial occlusive disease. After a run-in period of four weeks the patients received either buflomedil (600 mg daily) or placebo over 12 weeks. The pain-free and the total walking distances improved significantly in both groups. However, the differences in the improvement between the two groups were highly significant and in favour of buflomedil: for the pain-free walking distance p less than 0.001 and for the total walking distance p less than 0.01. The results indicate that buflomedil has a beneficial effect on the symptoms and lengthens the walking distance in patients with arterial occlusive disease.

Redistribution of intrarenal blood flow after carotid artery occlusion in the rat.

As indicated by changes in microsphere distribution in the rat, acute bilateral carotid artery occlusion (CAO) was found to cause a small but significant increase in outer cortical flow fraction. When renal perfusion pressure was kept constant, no redistribution occurred upon CAO. In denervated kidneys the redistribution induced by CAO did not differ from that observed in contralateral control kidneys. Animals pretreated with prostaglandin inhibitors showed a significant redistribution in the 'unprotected' right kidney and a minor increase in outer cortical flow fraction in the normotensive kidney. It is concluded that: (1) the increase in outer cortical flow fraction induced by CAO in the rat is due to an increase in perfusion pressure, and (2) that this redistribution is independent of sympathetic vasoconstrictor stimuli, catecholamines and prostaglandins.

Vowel distortion in traumatic dysarthria: a formant study.

The frequencies of the first two formants of three German vowels in word context were determined by a formant tracking routine for 8 male subjects with closed head trauma. A centralized formant pattern was found to characterize vowel articulation in traumatic dysarthria. Different degrees of severity could be assessed and the process of recovery was described by the parameter 'formant triangle area'. An interpretation in articulatory terms is given and the results are discussed in the light of articulatory models.

Vowel distortion in traumatic dysarthria: lip rounding versus tongue advancement.

Formant analysis of tense, high, German vowels was performed to the end of obtaining information about the role of insufficient lip rounding in distorted vowel production of 8 traumatic dysarthrics. A comparison was made between two allophones of /y/ in different consonantal contexts. Noticeable undershoot in lip rounding or protrusion proved to occur in a context of conflicting labial gestures. Where the articulatory realization of a CVC sequence required gross tongue movements, a lingual undershoot resulted as the prevailing deficit. No evidence for dyscoordinative defects was obtained from the results.
