Boron neutron capture therapy of brain tumors: biodistribution, pharmacokinetics, and radiation dosimetry sodium borocaptate in patients with gliomas.

OBJECTIVE: The purpose of this study was to obtain tumor and normal brain tissue biodistribution data and pharmacokinetic profiles for sodium borocaptate (Na2B12H11SH) (BSH), a drug that has been used clinically in Europe and Japan for boron neutron capture therapy of brain tumors. The study was performed with a group of 25 patients who had preoperative diagnoses of either glioblastoma multiforme (GBM) or anaplastic astrocytoma (AA) and were candidates for debulking surgery. Nineteen of these patients were subsequently shown to have histopathologically confirmed diagnoses of GBM or AA, and they constituted the study population. METHODS: BSH (non-10B-enriched) was infused intravenously, in a 1-hour period, at doses of 15, 25, and 50 mg boron/kg body weight (corresponding to 26.5, 44.1, and 88.2 mg BSH/kg body weight, respectively) to groups of 3, 3, and 13 patients, respectively. Multiple samples of tumor tissue, brain tissue around the tumors, and normal brain tissue were obtained at either 3 to 7 or 13 to 15 hours after infusion. Blood samples for pharmacokinetic studies were obtained at times up to 120 hours after termination of the infusion. Sixteen of the patients underwent surgery at the Beijing Neurosurgical Institute and three at The Ohio State University, where all tissue samples were subsequently analyzed for boron content by direct current plasma-atomic emission spectroscopy. RESULTS: Blood boron values peaked at the end of the infusion and then decreased triexponentially during the 120-hour sampling period. At 6 hours after termination of the infusion, these values had decreased to 20.8, 29.1, and 62.6 microg/ml for boron doses of 15, 25, and 50 mg/kg body weight, respectively. For a boron dose of 50 mg/kg body weight, the maximum (mean +/- standard deviation) solid tumor boron values at 3 to 7 hours after infusion were 17.1+/-5.8 and 17.3+/-10.1 microg/g for GBMs and AAs, respectively, and the mean tumor value averaged across all samples was 11.9 microg/g for both GBMs and AAs. In contrast, the mean normal brain tissue values, averaged across all samples, were 4.6+/-5.1 and 5.5+/-3.9 microg/g and the tumor/normal brain tissue ratios were3.8 and 3.2 for patients with GBMs and AAs, respectively. The large standard deviations indicated significant heterogeneity in uptake in both tumor and normal brain tissue. Regions histopathologically classified either as a mixture of tumor and normal brain tissue or as infiltrating tumor exhibited slightly lower boron concentrations than those designated as solid tumor. After a dose of 50 mg/kg body weight, boron concentrations in blood decreased from 104 microg/ml at 2 hours to 63 microg/ml at 6 hours and concentrations in skin and muscle were 43.1 and 39.2 microg/g, respectively, during the 3- to 7-hour sampling period. CONCLUSION: When tumor, blood, and normal tissue boron concentrations were taken into account, the most favorable tumor uptake data were obtained with a boron dose of 25 mg/kg body weight, 3 to 7 hours after termination of the infusion. Although blood boron levels were high, normal brain tissue boron levels were almost always lower than tumor levels. However, tumor boron concentrations were less than those necessary for boron neutron capture therapy, and there was significant intratumoral and interpatient variability in the uptake of BSH, which would make estimation of the radiation dose delivered to the tumor very difficult. It is unlikely that intravenous administration of a single dose of BSH would result in therapeutically useful levels of boron. However, combining BSH with boronophenylalanine, the other compound that has been used clinically, and optimizing their delivery could increase tumor boron uptake and potentially improve the efficacy of boron neutron capture therapy.

Using an MRI distortion transfer function to characterize the ghosts in motion-corrupted images.

Motion artefact suppression remains an active topic in MRI. In this paper, we suggest that certain nonrigid, or spatially variant, characteristics of motion of an object can be represented by extending the work of Mitsa et al. This empirical extension uses a ghost distortion transfer function (GTDF) applied to the k-space (frequency domain) data. We demonstrate the variety of ghost characteristics that can be generated from various two-dimensional (2-D) GTDF's. The distortion transfer function for periodic motion along the Z-axis can be determined from the nonoverlapped portions of the ghost and central image. It required a GDTF with the shape of a belt bandpass filter to produce an image corresponding to the ghosts of a volunteer's abdomen image corrupted by unknown respiratory motion artefacts. The preliminary results of a composite method of motion artefact suppression are presented. The artefact suppression was successful for ghost images described by a GDTF have a low-pass nature, but less successful with ghosts have a GDTF of a bandpass nature.

Discrete roles for peroxisome proliferator-activated receptor gamma and retinoid X receptor in recruiting nuclear receptor coactivators.

Peroxisome proliferator-activated receptor gamma (PPARgamma) plays a major role in adipogenesis. PPARgamma binds to DNA as a heterodimer with retinoid X receptor (RXR), and PPARgamma-RXR can be activated by ligands specific for either receptor; the presence of both ligands can result in a cooperative effect on the transactivation of target genes. How these ligands mediate transactivation, however, remains unclear. PPARgamma is known to interact with both the p160/SRC-1 family of coactivators and the distinct, multisubunit coactivator complex called DRIP. A single DRIP subunit, DRIP205 (TRAP220, PBP), binds directly to PPARgamma. Here we report that PPARgamma and RXR selectively interacted with DRIP205 and p160 proteins in a ligand-dependent manner. At physiological concentrations, RXR-specific ligands only induced p160 binding to RXR, and PPARgamma-specific ligands exclusively recruited DRIP205 but not p160 coactivators to PPARgamma. This selectivity was not observed in interaction assays off DNA, implying that the specificity of coactivator binding in response to ligand is strongly influenced by the allosteric effects of DNA-bound heterodimers. These coactivator-selective effects were also observed in transient-transfection assays in the presence of overexpressed p160 or DRIP coactivators. The results suggest that the cooperative effects of PPARgamma- and RXR-specific ligands may occur at the level of selective coactivator recruitment.

Preservation of cerebrovascular tone and reactivity by sodium channel inhibition in experimental prolonged asphyxia in piglets.

Sodium channels using cAMP as a second messenger play a role in the regulation of cerebral circulation and metabolism. Cerebrospinal fluid (CSF) cAMP levels have been shown to correlate with the degree and duration of hypoxic injury and outcome and to be an indicator of cerebral vascular reactivity. We hypothesize that sodium channel inhibition either before or at termination of experimental asphyxia will attenuate cerebrovascular alterations and maintain CSF cAMP levels. Three groups of piglets with closed cranial windows were studied: asphyxia or group 1 (n = 5) and two treatment groups. Pigs were treated with 50 mg/kg of sodium channel blocker before asphyxia (group 2, n = 6) and after the termination of asphyxia and start of reventilation (group 3, n = 6). Asphyxia was sustained over 60 min by ventilating piglets with 10% O2 gas mixture and decreasing minute ventilation followed by 60 min of reventilation with room air. Every 10 min, pial arterial diameters were measured, and CSF samples were collected for cAMP determination. Vascular reactivity to topically applied isoproterenol (10(-4) M) was evaluated 60 min after recovery. During asphyxia, cAMP levels in group 2 peaked and declined at a later time with mean values remaining significantly higher than those of groups 1 and 3. During reventilation, CSF cAMP concentrations were highest in group 3 and lowest in group 1. Pial arteriolar dilation occurred during asphyxia in all three groups but to a lesser degree in the pretreated group compared with groups 1 and 3. Pial arteriolar reactivity to isoproterenol postasphyxia was preserved in both groups 2 and 3. In summary, in newborn pigs, pretreatment with sodium channel blocker resulted in higher CSF cAMP levels and a lesser degree of pial arteriolar dilation during prolonged asphyxia. Pretreatment or treatment at reventilation restored vascular tone and reactivity.

Defective organellar membrane protein trafficking in Ap3b1-deficient cells.

AP-3 is a heterotetrameric protein complex involved in intracellular vesicle transport. Molecular analyses show that Ap3b1, which encodes the AP-3 (&bgr;)3A subunit, is altered in pearl mice. To provide genetic evidence that mutation of Ap3b1 is responsible for the pearl phenotype and to determine the null phenotype, the Ap3b1 gene was disrupted by homologous recombination. Mice homozygous for the resulting allele, Ap3b1(LN), or compound heterozygotes with pearl, displayed phenotypes similar to those of pearl mice, confirming that Ap3b1 is the causal gene for pearl. Moreover, pearl is likely to be a hypomorph as the Ap3b1(LN) homozygotes had a lighter coat color and accumulated fewer of the micro3 and (&dgr;)3 subunits of AP-3 than did pearl mice. Finally, immunofluorescence analysis of fibroblasts and melanocytes cultured from Ap3b1(LN) homozygotes revealed that the lysosomal membrane proteins Lamp I and Lamp II and the melanosomal membrane protein tyrosinase were mislocalized. In particular, the Lamp proteins were clustered on the cell surface. These findings strengthen the evidence for an alternate pathway via the plasma membrane for cargo normally transported to organelles by AP-3.

Chronic toxicity of rare-earth elements on human beings: implications of blood biochemical indices in REE-high regions, South Jiangxi.

Blood analyses for rare-earth element (REE)-high background regions in South Jiangxi show that the population averages of many of the biochemical indices deviate markedly from normal values in the normal region. These deviations are thought to be caused by prolonged intake of REE through food chains in view of that the toxicity of other harmful metals such as Pb and Cd can be neglected because of their insignificant amounts in the environment. In comparison with the normal region, blood biochemical indices abnormal in the REE-high regions are manifested as low total serum protein (TSP), albumin, beta-globulin, glutamic pyruvic transitanase, serium triglycerides, and immunoglobulin, but high cholesterol. These deviations may be related to the REE concentration and composition of food chains, and are sex dependent. Certain blood indices (such as TSP) of different age groups in the LREE-high region indicate that the influence of REE on males is a one-way irreversible process, whereas females show a strong ability of restoration.

Rare-earth element distribution characteristics of biological chains in rare-earth element-high background regions and their implications.

The rare-earth element (REE) contents of water and vegetables from two typical REE-high background regions and a normal region in Gannan, Jiangxi Province, indicated that the REE contents were significantly different from those of water and vegetables, respectively. The average values are 0.03 mg/L and 0.11 mg/L REE for water from regions A and B. As the REE contents of vegetables from region A are different from region B, it is suggested that there are a number of factors controlling the REE distribution from those among plants. By comparing with the normal region, the soluble REE contents of water from the REE-high background regions are higher than those of the normal region by factors of 18 and 68, respectively. The REE contents of most plants and crops from regions A and B are higher than those of the normal region. It is clear that the REEs are the indispensable elements of plants during their growing period. Why are the REE contents of some plants from regions A and B usually higher than those from the normal region? The answer is that the plants and crops have passively absorbed REE during their growth.

Nonnative intermediate state of acid-stable beta-sheet protein.

Cell adhesion molecule, CD2, from the immunoglobulin superfamily, is comprised of antibodies and Ig-like domains and plays a fundamental role, not only in the immune system, but also in the interactions between cells, specifically in cell-cell adhesion. This study examines the N-terminal domain 1 of CD2 (CD2-1) at different pHs, and in 2,2,2-trifluoroethanol (TFE), using nears- and far-UV circular dichroism (CD), fluorescence, and 1H nuclear magnetic resonance to elucidate factors contributing to the Ig beta-structure. Contrary to the complete unfolding induced by guanidinehydrochloride, CD2-1 retains its native tertiary structure at pHs from 1.0 to 10.0. Like the effects of high temperatures that have previously been observed, TFE reduces the integrity of the tertiary structure, while reorganizing the secondary structure from a native all-beta-sheet to a significantly alpha-helical conformation. The induced helicity of CD2-1 correlates with the helicity inherent in its primary sequence. Our results suggest that electrostatic interactions are less important for the formation of the native secondary and tertiary structure of CD2-1, although they are crucial for CD2's adhesion function. Interference with the protein's hydrophobic interactions and hydrogen-bonding networks, however, causes significant changes in its conformation. Residues of CD2-1, with high conformational flexibility, may contribute for the formation of a metastable dimer by domain-swapping.

Coronary microangiopathy in type 2 diabetic patients: relation to glycemic control, sex, and microvascular angina rather than to coronary artery disease.

UNLABELLED: Coronary microangiopathy is a major complication in diabetics. However, the presence of independent factors in association with coronary microangiopathy in patients with non-insulin-dependent diabetes mellitus (NIDDM) or the difference in coronary microangiopathy between diabetics with coronary artery disease (CAD) and those with microvascular angina is unclear. METHODS: Nineteen patients with NIDDM and microvascular angina, 18 patients with NIDDM and CAD, and 17 age-matched control subjects were studied. Myocardial segments that were perfused by angiographically normal coronary arteries were studied. The baseline myocardial blood flow (MBF) and the MBF during dipyridamole administration were measured using PET and 13N-ammonia, after which the myocardial flow reserve (MFR) was calculated to assess coronary microangiopathy. RESULTS: The baseline MBF was comparable among NIDDM patients with microvascular angina, NIDDM patients with CAD, and control subjects. However, the MBF during dipyridamole administration was significantly lower in NIDDM patients with microvascular angina (126 +/- 42.7 mL/min/100 g) than that in either NIDDM patients with CAD (210 +/- 70.1 mL/min/100 g; P < 0.01) or control subjects (293 +/- 159 mL/min/100 g; P < 0.01), as was the MFR (NIDDM with microvascular angina, 1.90 +/- 0.73; NIDDM with CAD, 2.59 +/- 0.81 [P < 0.01]; control subjects, 3.69 +/- 1.09 [P < 0.01]). Multivariate stepwise regression analysis showed that, among the factors considered, glycemic control was independently related to the MFR (r = 0.838; P < 0.05). CONCLUSION: Glycemic control appears to be essential for coronary microangiopathy in NIDDM.

The effect of zhu yu hua tan tang on intracranial pressure in case of acute cerebral hemorrhage.

The therapeutic effect of Zhu Yu Hua Tan Tang ([symbol: see text] Decoction for removing blood stasis and resolving phlegm) plus general and modern treatment on acute cerebral hemorrhage in 44 cases, and the pressure-lowering effect of Zhu Yu Hua Tan Tang and mannitol in 20 cases of acute cerebral hemorrhage in the basilar region, and in rabbits with experimental encephaledema induced by injection of olive oil into the common carotid artery were observed in this study. The results showed that Zhu Yu Hua Tan Tang plus general treatment can better improved the conscious state of the patients than western medicine, and it can make the intracranial pressure drop clinically by 40.44%. The intracranial pressure-lowering effect of Zhu Yu Hua Tan Tang, though slow, is smooth and long-lasting without any rebound phenomenon, as compared to those of mannitol. Similar results were found in animal experiments, but the intracranial pressure-lowering amplitude of Zhu Yu Hua Tan Tang was stronger than that of mannitol. The difference between the clinical and experimental results needs to be further studied.

[Clinical significance of beta-hCG and human placental lactogen in serum of normal pregnancies and patients with pregnancy induced hypertension].

OBJECTIVE: To examine the level of beta-hCG and Human Placental Lactogen (HPL) in maternal serum of normal pregnancies and patients with pregnancy induced hypertension (PIH) and to study the clinical significance and effects of them on the pathogenesis of the disease. METHODS: Serum levels of beta-hCG and HPL of 142 normal pregnant women and 43 patients with PIH (16 mild, 12 moderate and 15 severe) were determined by radioimmunossay. RESULTS: (1) Serum levels of beta-hCG in all PIH subgroups [(25.33 +/- 17.80) micrograms/L in mild, (33.12 +/- 4.91) micrograms/L in moderate, (42.19 +/- 17.47) micrograms/L in severe] were significantly higher than the normal pregnant group [(12.33 +/- 7.92) micrograms/L; P < 0.001]. There is positive correlation between levels of beta-hCG and degrees of PIH (r = 0.677, P < 0.05). (2) Serum levels of HPL in all PIH subgroups [(14.73 +/- 3.26) mg/L in mild, (11.44 +/- 4.02) mg/L in moderate, (12.73 +/- 4.18) mg/L in severe] did not show significant difference when compared with that of normal pregnant group [(12.78 +/- 4.67) mg/L; P > 0.05]. Furthermore, there was no correlation between levels of HPL and degrees of PIH (r = -0.300, P > 0.05). CONCLUSIONS: Serum beta-hCG could reflect the degree of functional imbalance of the placental trophoblasts on PIH and it may be used as a clinical detecting index of PIH. On the other hand, HPL is not such a useful factor.

[Genetic analysis of hereditary factor VII deficiency from a Chinese pedigree].

OBJECTIVE: To identify the mutation in coagulation factor VII gene from a Chinese patient with hereditary coagulation factor VII deficiency. METHODS: The genomic DNA fragments of FVII gene from a propositus and normal subjects were amplified using polymerase chain reaction (PCR), and analyzed with direct sequencing of PCR products. The PCR amplified genomic DNA fragments of FVII gene from the propositus and her family members were analyzed using restriction enzyme Hgic I. RESULTS: The FVII gene sequences of normal subjects were identical to the data published, while a missense mutation (TGT-->GGT) was found at codon 329 in FVII gene of the propositus. The heterozygous condition for the mutation was revealed in her three family members. CONCLUSION: We have found a novel mutation (TGT-->GGT) at codon 329 in FVII gene of a patient with hereditary FVII deficiency, which leads to a cystein residue replaced by a glysine. PCR combined with restriction enzyme Hgic I digestion would be a rapid diagnostic method for this mutation.

[Analysis of complications of augmentation mammaplasty in 12 patients].

OBJECTIVE: To reduce complications and to improve the long-term results of augmentation mammaplasty. METHOD: Twelve patients who had complications during a period of 1 to 13 years after augmentation mammaplasty were studied. The fibrous capsule of the breast implant was examined pathologically. RESULTS: 12 patients (15 breasts) were included in the study. Of the nine patients with unilateral augmentation mammaplasty, four had 1 complication; four had 2 complications and one had 3 complications. Of the three patients with bilateral augmentation mammaplasty, two had the same complication, one had 3 complications. Every complication has its pathologic and clinical characteristics. CONCLUSION: Many measures are important for reducing complications including strict surgical manipulation, proper selection of good implants, preferred submuscular insertion, whole capsulectomy for prosthesis rupture or leak and through hemostasis, proper bandage, massage and follow-up et al.

[Morphological variation of spermatogenic cell nucleus in Macrobrachium nipponense(de Haan) and its status during reproductive evolution of Caridea].

The morphological variation of nucleus during spermatogenesis in Macrobrachium nipponense and its status during the reproductive evolution of Caridean shrimp were investigated using transmission electron microscopy(TEM) technique. During the whole process of spermatogenesis, the nuclear morphology of spermatogenic cell varied from spherical or ellipsoidal to shallow-dish-like shape, and the nuclear membrane existed only in the rear part of mature sperm varied from intact to unintegrated one. The chromatin gradually varied from loosen to coacervative one, and then, developed differentially into two types of nucleus in the mature sperm, i.e., vesicular nucleus and filamental nucleus, which became an important characteristics of Macrobrachium nipponense. These nuclear morphological variations could be regarded as an important classifying property among Decapoda, on which, studies could contribute much to the analysis of the status of this species in the reproductive evolution of Caridean shrimp.

[Rational range and optimum proportion of energy input in farmland].

There exists a rational range of artificial supplemental energy inputs in fields. In the range, the inputs can improve the yield and the output/input ratio. The rational range of energy input in total energy output is bigger than that in the output/input ratio. It is bigger in low yield fields than high yield fields, which suggests that increasing artificial supplemental energy input can increase the production of low yield fields as same as high yield fields. In the rational range of energy input, there exist high efficient lines in water and fertilizer inputs. The highest efficiency occurs when the rational inputs of water and fertilizer occur in these two lines. There also exist optimum lines of energy and economy input proportions. In the two lines, the proportions of energy and economy output/input of water and fertilizer inputs are the highest, which are the optimum of water and fertilizer inputs. Water and fertilizer may replace each other for equal yields, but would result in a decrease in the output/input ratio.

Acute fatty liver of pregnancy: diagnosis and management of 8 cases.

OBJECTIVE: To investigate the early recognition and management of acute fatty liver of pregnancy (AFLP) to improve maternal and fetal survival. METHODS: Eight cases of AFLP seen in our hospital during the past three years were studied retrospectively. Symptoms, laboratory findings, timing of liver biopsy, and maternal and fetus outcome were assessed. RESULTS: The mean gestational age at onset was 34 +/- 2 weeks. All cases were primigravida. In the early stages, all patients presented malaise, nausea, vomiting and epigastric distress followed by jaundice in the third trimester of pregnancy. LABORATORY FINDINGS: all had raised transaminases and serum bilirubin (32.5-510.8 mumol/L), hypoalbuminemia (22.4-30.0 g/L), hypofibrinogenemia (< 180 mg/dl), prolonged prothrombin time and prolonged partial thromboplastin time. Maternal complication included hepatic encephalopathy (6 cases), ascites (6), hypoglycemia (5), hematemesis (2), and postpartum hemorrhage (5) and preeclampsia (4). Emergency cesarean section was performed in 3 cases. One mother died of fulminant hepatic failure and the others survived. There was no fetus death. Liver biopsy was done on the 5th to 15th postpartum day in 8 cases. CONCLUSION: With increasing awareness, especially in the early recognition of milder cases, and prompt progressive management including early termination of pregnancy by cesarean section and large dose infusion of fresh frozen plasma and albumin alternately, the prognosis of AFLP can be improved.
