Cell electrophoresis for diagnostic purposes. I. Diagnostic value of the electrophoretic mobility test (EMT) for the detection of gynaecological malignancies.

Lymphocytes from 278 gynaecological patients (100 controls and 178 patients with a malignant condition) have been investigated for their response to encephalitogenic factor, cancer basic protein, and KCl extract of adenocarcinoma of the body of the uterus as "antigens", using tanned sheep erythrocytes ETS as indicator particles in the electrophoretic mobility test (EMT). Electrophoretic mobility was measured with a Zeiss cytopherometer. The study was split into three test series producing in the cancer group 66% correct positive test results (34% false negatives) and in the control group 83% correct negative results (17% false positives). Consequently, with the instrumentation used, EMT is, at least in our hands, not sufficiently reliable for the diagnosis of cancer.

Cell electrophoresis for diagnostic purposes. II. Critical evaluation of conventional cytopherometry.

Determination of the electrophoretic mobility of test cells has been widely used in an attempt to detect so-called lymphokines in a laboratory test for cancer, but operational difficulties are inherent in conventional cytopherometers. This study therefore investigates the technical and operational aspects of cell electrophoresis, using the Zeiss cytopherometer; e.g. influence of electro-osmosis, focus uncertainty, movement due to convection and other sources of error. Implications and possible improvements in the test are discussed.

Structure of Cu2(indomethacin)4 and the reaction with superoxide in aprotic systems.

The copper complex of indomethacin (1-(p-chlorobenzoyl)-5-methoxy-2-methyl-indole acetate), a common anti-inflammatory drug, was prepared and characterized. Crystal structure determination revealed the dimeric form of the 1 : 2 complex, namely Cu2(indomethacin)4 x L2, in the unit cell. Surprisingly, the copper-copper distance (263 pm) was very close to metallic copper (256 pm). The two coordination sites in the copper-copper axis can be readily replaced by superoxide. An intriguing similarity to Cu2(acetate)4 was seen. Due to the lipophilic nature of the indomethacin ligand, this copper complex reacted with superoxide in aprotic solvents. the superoxide dismutating activity was successfully demonstrated in Me2SO/water and acetonitrile/water mixtures using the nitro-blue tetrazolium assay and pulse radiolysis. The second-order rate constant of 6 x 10(9) M-1 x s-1 in strictly aqueous systems dropped only slightly to 1.1 x 10(9) M-1 x s-1 when aprotic solvents were used. This is the fastest rate constant ever observed for a copper-dependent dismutation of superoxide. The KO2-induced lipid peroxidation in both erythrocytes and liver microsomes was suppressed by 70% in the presence of 1 x 10(-10) mol x ml-1 of Cu2(indomethacin)4. The inhibitory action dropped to 25% when Cu2Zn2superoxide dismutase was employed. The formation of copper x indomethacin in rat serum after administration of indomethacin was shown in vitro and vivo.

Prosopo-affective agnosia associated with chronic organic brain syndrome.

Impairment of the ability to recognize facially expressed emotions was studied in 14 chronically disoriented patients with chronic organic brain syndrome (CBS). This impairment was named prosopo-affective agnosia (PAA). A diagnostic requirement was relatively intact neurologic functioning in underlying perceptual-verbal-motor processing. A test was designed for facial-affect recognition in the accurate differentiation of normal persons from chronically disoriented CBS patients. No normal subject made any errors in this test. Despite decades of illness and hospital living, patients with a history of schizophrenia or major affective disorders scored almost at a normal level (95 vs. 100 percent) in this test, and significantly higher (95 vs. 66 percent) than did the disoriented CBS patients. The social and therapeutic implications of the findings are stressed. CBS patients may be impaired with respect to receiving and appreciating elementary aspects of social communications such as recognizing a smile, anger, sadness or disapproval on the faces of people who surround them. This disability requires understanding and a special attitude on the part of the therapeutic team toward such patients.

[Accuracy of lymphography in carcinoma of the cervix (author's transl)].

A review has been made of lymphographic diagnoses recorded from patients with cervical carcinoma over more than five years. The diagnosis by lymphography of nodes metastases had to be altered in some 25 per cent, and correction proved necessary also in cases in which lymphography had failed to yield conclusive information. - In the group with lymphographically secured cancer lymph nodes coincided with histological results in 70 per cent of all primary and 90 per cent of secondary checks. Comparable gaps were recorded also from the group with lymphographically inconclusive information on lymph node cancer. - The following conclusions are drawn from the study described: (a) The accuracy of lymphographic diagnosis depends on the experiences of the examiner and changes within time and this must be considered in the evaluation of therapeutic success or failure. - (b) Conclusive findings in the context of lymph node problems are obtainable from selective use of lymphography.

[Cancerostatically active 6,6a,7,8,13,13a-hexahydro-1-benzopyrano[4,3-b]-1,5-benzodiazepine diastereoisomers (author's transl)].

The action of many psychotropic drugs on the adenylate cyclase system and its role in the regulation of tumour cell division justify the expectance that potentially psychotropic 1-benzopyrano[4,3-b]-1,5-benzodiazepine derivatives (3a,b and 6a,b) will also exert a cancerostatic effect. The synthesis of 6,6a,7,8,13,13a-hexahydro-1-benzopyrano[4,3-b]-1,5-benzodiazepines (4a and b) and their separation into the antileukaemic (L 1210) cis-forms and the inefficacious trans-forms are described. The cis-form 5a, unlike the trans-form 6a, stimulated the adenylate cyclase system.
