Prerenal hypertension caused by a postoperative A-V fistula.

The A-V fistula in the renal pedicle with functioning renal parenchyma leads to a prerenal hypertension by the blood-steal effect from the renal artery through the fistula into the vein, according the pressure gradient over the fistula. The etiology of these A-V fistulas is pointed out. A case corrected by surgical removal of the fistula is reported.

[Roentgenologic diagnosis of splenic trauma (author's transl)].

Description of radiologic criteria in splenic trauma. Abdominal plain film: Enlarged shadow of the spleen with blurred margins; elevation - or rupture - of the left diaphragm; homogenous radiodensity in the left hypochondriac region; fractures of ribs or transverse process. Angiography is indicated in doubtful cases to clarify the following lesions: contusion, sub-or extracapsular hematoma, rupture and fragmentation. Possible diagnostic errors in arteriography (63 examined cases) are discussed.

[Cell-mediated immune-phenomenona in progressive chronic polyarthritis].

With the help of the lymphocyte transformation test 34 patients with ascertained progressive chronic polyarthritis and 20 control persons were examined. A proteoglycan fraction from normal cartilaginous tissue was used as antigen. Hereby was found: 1. The antigen-conditioned stimulation of the lymphocytes in the group with progressive chronic polyarthritis was greater than in the control persons. 2. In the formation of the transformation index the difference between the two groups could be statistically ascertained. With the help of the present examination a further proof was given that in the process of rheumatoid arthritis cellular immune phenomena play a part. However, which value cellular immune mechanisms have in the etiopathogenesis of the progressive chronic polyarthritis, cannot be answered with the results of the examinations.

[Cell-mediated immune-phenomenona in hyperthyroidism and euthyroid goiter (studies using the lymphocyte transformation test)].

For the establishment of cellular immune phenomena in hyperthyroidism and the euthyreotic struma 27 patients with hyperthyroidism, 18 patients with euthyreotic struma and 33 healthy control persons were examined with the help of the lymphocyte transformation test as to their ability to the blast cell transformation under addition of watery extract of the thyroid gland. In the cases of hyperthyroidism significantly increased transformation indices were found compared with the controls. Also in the euthyreotic strumae increased values were found compared with the controls. They lay at the borderline to significance. Three different extracts were used as antigen. The transformation indices calculated in these cases were partly significantly different. From this is concluded that not in every watery extract of the thyroid gland the responsible antigen exists in sufficient quantity and/or quality.

[Angiographic diagnosis of paraduodenal hernia (author's transl)].

The case is reported of a 26-year-old man who had several episodes of subileus which responded to spasmolytics. Mesenteric angiography revealed a paraduodenal hernia, with displacement of several jejunal arteries towards the mesenteric root and compression of intestinal vessels at about the level of Treitz' ligament. The angiographic findings were confirmed at operation, which consisted of repositioning the jejunal loop and closing the hernial orifice. The patient has since been free of complaints.

[Arteriosclerosis: efficiency of angiography (author's transl)].

Stenosis, occlusion, and aneurysmatic dilation are the principle angiographic signs in arteriosclerosis. Aorto-arteriography, either by direct puncture or indirectly via a catheter, permits visualization of all vascular sites of surgical interest, determination of the location and extent of the lesion, and evaluation of the function of the collateral system as well as the inflow and outflow. This stresses the extreme importance of angiography in preoperative planning and postoperative documentation of successful therapy.

[View of the past: Roentgenography of an Egyptian mummy].

Report concerning roentgenographic examinations of an Egyptian mummy dating back to 900 B.C. from the "Museum für Völkerkunde Freiburg/Breisgau". The study was performed to detect funeral gifts which can be found otherwise only if the linnen bandage of the mummy is destroyed. Extraordinary findings were degenerative osteoarthritis of the spine and joints, pseudo-gout, marked caries of several teeth and signs for a healed occipital osteomyelitis. Suggestions are discussed about a particular compression of thorax and pelvis which is apparently due to the technique of bandaging. This was not observed in examinations of other mummies. Fractures of the first to third phalanges suggest forceful removal of precious rings. A circular-shaped opening in the parietal bone and a small radiodense foreign body which projects over the pharynx remain unexplained.

[An unusual cause of an "intramural" gas collection in the right colon (author's transl)].

A case of subserous gas collection in the right colon following surgical repair of an iatrogenic uterus perforation is presented. The patient showed an ileus-like roentgen symptomatology when ischemic necrosis of the bowel was suspected. Laparatomy demonstrated the remainder of the gas collection in the area of the right pelvis extending retroperitoneally towards the right colon, thus presenting intramural pneumatosis. Therefore the differential diagnosis of intramural gas collection as a sign of ischemic bowel necrosis has to include the harmless subserous pneumatosis following abdominal trauma (especially iatrogenic uterus perforation).

[Coronary-angiographic demonstration of the dilating effect of dipyridamole on coronary arteries and its inhibition by caffeine (author's transl)].

Selective coronary cineangiographies are performed repeatedly in 13 mongrel dogs. The contrast medium itself has no demonstrable dilating effect upon the vessels. After the first injection of 0.2 mg/kg of dipyridamole the left anterior coronary artery is significantly (p less than 0.001) dilated, which is in contrast to some results described in literature. Immediately after an injection of high doses of caffeine (10 mg/kg) the previously enlarged coronary diameter decreases significantly (p less than 0.001). During the action of caffeine subsequent infections of dipyridamole result in a very small, or no change in coronary diameter. The coronary-dilating effect of dipyridamole can be inhibited by high doses of caffeine (10 mg/kg).
