Arthritis, Rheumatoid↗

[Postoperative radiology of the colon (author's transl)].

The most important surgical procedures are described. Early postoperative control may reveal insufficiency of the anastomoses or stenosis caused by scar formation. Later examinations may reveal anatomic and functional alterations and have to exclude tumor recurrency. In our patients 13% of abdominal surgery concerned the colon. Of 3000 barium enemas, two were complicated by perforations following colonic or pancreatic surgery. Both patients survived.

[Enchondroma - radiology and pathology (author's transl)].

Analysis of 374 cases of enchondroma, 42% of which were localized in the short bones of the hand. The findings in radiology and pathology are presented and discussed, with special attention to the difficulty of classifying these tumors histologically and distinguishing them from chondrosarcoma. The recurrence rate in the short bones of the hand was 9%, and 19% in the long tubular bones. It was especially high after mere currettage of the tumor.

[Roentgen signs of rare mediastinal masses (author's transl)].

Mediastinal masses are commonly classified into lesions of the anterior, middle, and posterior compartment as a first step in differential diagnosis. Teratoma, neurogenic tumors, goiters, and thymoma with their characteristic localization constitute together already two thirds of all mediastinal neoplasms. However, the differential diagnosis of mediastinal masses should include also rare diseases: lymphangio-hemangioma, aneurysm of the pulmonal artery, intrathoracic meningocele, esophageal cyst, and pleurobronchial cyst are demonstrated. It is emphasized, that differential diagnosis has to consider history and physical data of the patient as well.

[Situation and trends of hyposensitisation treatment (author's transl)].

Since the introduction of the hyposensitization treatment by Noon and Freeman in 1911, an essential change of the hyposensibilisation technique, despite numerous recently published results, has not been able to be demonstrated up to now. In most cases, treatment is still done with and allergen mixture consisting of a variety of unknown compounds with changing composition according to own experiences concerning initial and final dosis of the allergen or the application intervals. The linkage of the allergens to chemical substances, the modification into so-called allergoids as well as the investigation of the hyposensitisation success, such as determination of blocking antibodies, the course of the titre of specific antibodies by the RAST technique, the histamin liberation from leucocytes of allergic persons after antigen contact and the lymphocyte stimulation by allergens are starting points for developing a scientific base to the hyposensitisation treatment and for introducing new results mainly from experiments on animals into the clinical practice. Standardisation of allergens, optimisation of the dosis and the application intervals as well as the further decrease of the allergenicity and the optimal choice of the adjuvants have to be regarded as the problems to be solved in the next future. On the other hand, the use of allergen determinants linked to non-immune carriers or the induction of a tolerance by another way seem to be not yet a subject with clinical relevance during the next years.

[The value of galactography in the secreting breast].

At the Institutes for Diagnostic Radiology of the Universities of Athens (Greece) and Freiburg/Br. (West Germany) the mammographic examination had to be supplemented in 4 res. 2 per cent by injection of contrast medium into the milk ducts (galactography). This procedure is indicated in cases with secreting of the mammary glands. Technic and results are presented. Recognition of small, non-palpable tumors which might not be detected by mammography alone is emphasized.

[Unusual angiographic technic in a ruptured aortic aneurysm (author's transl)].

In a 65-year-old patient with amputation of both legs, a life-threatening acute abdominal hemorrhage necessitates aortography. Following surgical dissection of the right femoral artery a catheter could be inserted through the thrombosed vessel into the aortic lumen. This approach made the diagnosis of a perforated aortic aneurysm by angiography possible.

Aged↗

[Angiography in extracranial vascular processes (author's transl)].

Anatomy and technique in angiography of extracranial vessels is discussed based upon 1321 own cases. Most frequently catheter technique is used. Main indication is cerebral vascular insufficiency, less often aneurysm, trauma and tumor. Results of angiography are discussed.

Angiography↗

[Upside-down spleen (author's transl)].

Upside-down spleen is a rare positional anomaly which is an embryonic malformation and not a 'wandering spleen'. It is not a disease, but is important to recognize, lest it be misinterpreted on angiographic, scanning or ultrasound examination.

Diagnostic Errors↗

[Prevention and treatment of thromboembolism: diagnosis by roentgenology (author's transl)].

In 30% of our patients, thrombotic disease was an indication for phlebography. Visualization of veins was performed preferably by taking X-ray films of the ascending contrast medium. The radiologic findings of the acute stages include filling defects, irregular margins, and convex delineation. Post-thrombotic signs include lesions of the valves, recanalization, and collateralization. In cases of occluded inferior vena cava and axillary vein (Paget-von Schroetter Syndrome) collateral circulation occurs. In 80% of cases, phlebography provides important information following medical or surgical therapy.

Acute Disease↗

[Therapeutic vascular occlusion with angiographic catheter technique (author's transl)].

Therapeutic embolization can achieve occlusion of bleeding vessels, arteriovenous fistulas and tumor feeding arteries. Many different diseases can now be treated by angiographic catheter techniques which required surgical intervention in the past. Various embolic materials were applied to produce transient or permanent vascular occlusions. New materials for embolization will be developed. Indications for embolization treatment of tumors are not definitively established because of insufficient knowledge of tumor biology after embolization.

Angiography↗

[Roentgenologic diagnosis of agenesis of the gallbladder (author's transl)].

A 58-year-old female with a surgically confirmed agenesis of the gallbladder is resported. Infusion-cholegraphy revealed a bud-like bulging of the common hepatic duct with a diameter of 6 mm. Celiac arteriography did not demonstrate cystic arteries supplying the gallbladder. As combined malformations celiac arteriography revealed an accessory spleen and a small hamartoma of the liver.
