[Evaluation of long-term community based intervention on cardiovascular diseases: 9-year follow up study of the mortality from the cohorts].

OBJECTIVE: To evaluate the effect of long-term intervention on all-causes mortality and the mortality of cardiovascular diseases. METHODS: In May 1987 in Beijing, 2 geographically separated community with a registered population of about 10 000 each were selected as either intervention or control community. In each community, a cohort containing 2 700 subjects (>/= 35 years) free of stroke was sampled. A baseline survey was administered to screen the eligible subjects for intervention. Death information from the cohorts was collected. RESULTS: By the end of Dec. 31, 1995, the 9-year cumulative all-causes mortalities were 3.51% (303/2 861) in the intervention cohort and 4.71% (390/2 760) in the control cohort. Cardiovascular diseases and neoplasm were the first and second leading causes of death in the cohorts. Both the all-causes mortality and the mortality of cardiovascular diseases were significantly lower in the intervention cohort than in the control group. The risk for dying of all-causes death was reduced by 22% (RR = 0.78, 95% CI: 0.67 approximately 0.91) and the risk for dying of cardiovascular diseases was reduced by 24% (RR = 0.76, 95% CI: 0.57 approximately 0.96) in the intervention cohort. CONCLUSION: Long-term community intervention for the risk factors of cardiovascular diseases can effectively reduce the mortality of cardiovascular diseases as well as all-causes mortality.

[MR myelography: analysis of 126 cases].

OBJECTIVE: To investigate the value and limitation of MR myelography (MRM) in depicting the spinal canal obstruction. METHODS: Heavily T2-weighted coronal MR myelography was performed with 3D fast-spin-echo (FSE) and fat-suppression sequence in 126 cases, including 6 cerebellomedullary cistern, 18 cervical, 19 thoracic and 83 lumbar cases. The resulting slice were then projected into a composite image using a standard maxium intensity projection (MIP) algorithm. RESULTS: 90.5% (114/126) of MRM yielded reproducible high-quality image of the spinal thecal sac. 106 lesions (extradural, intradural, intramedullary) were detected in 101 cases (80.2%). The level and degree of spinal canal obstruction were visualized and confirmed by operation in 66 cases with a high sensitivity of 100%, and a diagnostic accuracy of 91.3%. CONCLUSION: Heavily T2 FSE MR myelography is a noninvasive and reliable method. MRM surpasses conventional and CT myelography and could replace them in some degree.

Surgical treatment of portal hypertension: 45 year experience.

OBJECTIVE: To review the experience in surgery for 912 patients with portal hypertension. METHODS: The 912 patients were divided into three groups: before 1978 (group A), 1978 - 1989 (group B), and 1990 - 1998 (group C). The results were reviewed of portosystemic shunt (PSS), disconnection and disconnection plus PSS for emergency and elective operation at different periods. RESULTS: In the 912 patients of groups A, B, C, the number of PSS, disconnection, and PSS plus disconnection was 345, 13, 0; 84, 209, 0; 63, 204, 63, respectively. The number of emergency operation was 45, 13 and 4 in each group. The operative mortality of shunt in the groups A, B, C was 7.82%, 7.14% and 0, respectively; that of disconnection in the groups A, B, C 15.38%, 6.28% and 4.94%; and that of PSS plus disconnection in group C 3.51%. Encephalopathy occurred in 14.37% of the PSS group, in 5.68% of the disconnection group, and in 5.77% of the disconnection plus PSS group. Rebleeding occurred in 10.78% of the PSS group, in 18.95% of the disconnection group, and in 7.69% of the disconnection plus PSS group. CONCLUSIONS: Operative indications for the cirrhotic patients with portal hypertension depend on their liver function. For those patients with poor liver function, surgery is considered when their liver function is improved. We suggest prophylactic operation is valuable. Other indices from barium meal, color ultrasound, and endoscopy are also considered. The number of emergency operation has been decreasing. Since the 1980s, disconnection operation has been adopted increasingly. In our hospital, the number of patients receiving disconnection has been increasing from the early 1980s and the operation has almost replaced shunt operation in the 1990s. Disconnection combined with splenorenal shunt can lower the portal pressure and maintain the portal flow. Disconnection combined with splenorenal shunt seems to be a better procedure of choice.

[Annular constriction of popliteal vein for treating severe venous insufficiency].

OBJECTIVE: To study the mechanism and therapeutic effects of annular constriction for severe venous insufficiency of legs. METHODS: Annular constriction of popliteal vein was performed by Gore-Tex vascular graft in 82 patients. Among them, 78 patients had deep venous valve insufficiency of legs (III, IV degree), 2 postphlebitic syndrome, and 2 valve absence of deep vein. Under normal respiration and breath holding, venous pressure was measured at the proximal and distal part from the constricting site. RESULTS: Great difference was noted in the venous pressure between the proximal and distal part from the constricting site. The pressure was (9.4 +/- 4.5) mm Hg, (41.2 +/- 28.1) mm Hg at calm and (32.4 +/- 16.2) mm Hg, (41.2 +/- 28.1)mm Hg at breath holding (P < 0.01). Symptoms disappeared or released after operation. Ulcer healed in all patients. Venous pressure decreased by over 60%. Pressure recovery time was over 22 seconds. There was no reflux distal to the constricting site. Deep venous angiography and color Doppler showed an effect similar to venous valve. CONCLUSION: Annular constriction is effective in treating severe venous insufficiency of legs.

[Repairing peripheral nerve defects by tissue engineering techniques:an experimental study].

OBJECTIVE: To develop a novel effective substitute material or technique to repair peripheral nerve gap. METHODS: We inoculated expanded Schwann cells (SCs) at re-arranged bio-absorbable polymer polyglycolic acid (PGA) fiber and incubated for two weeks, then we developed a novel tissue-engineered scaffolds. The scaffolds were used as cellular isografts to bridge 15 mm long gap of sciatic nerve in inbred strains of Wistar rats. In an autologous and pure PGA fiber control group, the same surgical procedure was used. Evaluation included general observation, electromyographic examination, muscle measurement, and histological observation of serial sections at 12 weeks after surgery. The total number and density of reinnervation and thickness of myelin sheath was measured by computerized image analysis. RESULTS: SCs put out a long and thin prominence and migrated along the PGA fibers in spirality or parallel when they divided and finally rank into a cell-chain formation similar to Büngner's band. TEM/SEM and immunohistochemical survey demonstrated that the SCs at PGA fiber also secreted a great deal of ECMs included laminin, which play a very important role in peripheral nerve regeneration. Non-tubular scaffold comprised SCs and laminin (LN) channel in three-dimensional longitudinal rank. Animal transplantation study indicated the sensory and motor functional results of hindlimbs of experimental group rats reached to similar level of those found in nerve autograft control group. The number of reinnervation in the experimental group rats was slightly fewer than that in nerve autograft control group; but the axonal density was just reversal between the two groups because of a large area ischemic necrosis in the center of nerve autografts. CONCLUSION: This new paradigm offers a potential solution to repairing a long gap of peripheral nerve.

[An ultrastructural observation on rat retina after photoreceptor cell implantation].

OBJECTIVE: To further study the retinal neuronal signal. METHODS: The Wistar/RCS (RCS rats are rats with hereditary photoreceptor degeneration) rats were respectively as donors/acceptors, and the retinal pathway was reconstructed with the technique of pure photoreceptor transplantation. The photoreceptor layer of the retina was obtained with the technique of retinal whittle by manual method or excimer laser. The specimens were got separately at 2 weeks and 1 month after the transplantation and studied under the light and transmission electron microscopes. RESULTS: Most transplanted photoreceptors with physical poles were lined up regularly between the retinal pigment epithelium (RPE) and inner nuclear layer. It was shown that in the new outer plexiform layer the relatively integral synapse and its interconnection were seen. CONCLUSION: The retinal neuronal pathway can be reconstructed by retinal transplantation.

[Trabeculectomy combined with phacoemulsification for treatment of glaucoma complicated with cataract].

OBJECTIVE: To evaluate the effectiveness of small-incision triple procedure, including phacoemulsification, posterior chamber intraocular lens implantation and trabeculectomy, in patients with coexisting glaucoma and cataract. METHODS: Twenty patients (26 eyes) with coexisting glaucoma and cataract underwent 3.5 mm-incision triple procedure. The mean follow-up was 16.1 months (3 - 41 months). RESULTS: The mean preoperative intraocular pressure (IOP) was (23.01 +/- 2.63) mm Hg (1 mm Hg = 0.133 kPa) which decreased to a mean postoperative IOP of (13.93 +/- 1.85) mm Hg (P < 0. 001). Seventeen (65%) of 26 eyes had a best-corrected visual acuity of >or= 0.6 at the last follow-up (range, 0.05 - 1.0). The mean magnitude of astigmatism was 0.81 D (range, 0 - 3.00 D), 4 eyes had no astigmatism. Although 2 eyes used antiglaucoma medications shortly after the surgery, no eyes used any medications at the last follow-up. The early postoperative complications included corneal edema in 5 eyes (19%) and shallow anterior chamber in 3 eyes (12%). The late complication was mainly the after cataract in 6 eyes (23%). CONCLUSIONS: The small-incision triple procedure appears effective for treating selected patients with coexisting cataract and glaucoma, improving the valid visual acuity rapidly, reducing intraocular pressure with less medications and having less postoperative complications.

[A case control study on the distribution of apolipoprotein AI gene polymorphisms in the survivors of atherosclerosis cerebral infarction].

OBJECTIVE: To investigate the frequency of the restrict fragment length polimorphisms (RFLP) at 2 MspI sites in the 5' - end of apolipoprotein AI (ApoAI) gene and its potential interaction to atherosclerotic cerebral infarction (ACI) in Chinese Han population. METHOD: Polymerase chain reaction (PCR) technique was used in a sample of 199 cases with documented ACI and 204 healthy matched by age and sex individuals selected from Chinese Han nationality in Beijing. The studied loci include promoter region (-75 bp) and the intron - 1 (+83 bp). RESULTS: (1) Both ACI and control groups M1 + + and M1 + - were major frequent genotypes in Beijng area, M2 + + were major frequent genotypes in intron - 1; (2) M1 + - genotype appeared more common among patients than in controls (0.487 vs 0.368, P < 0.05, OR = 1.64); (3) Individuals with M1 + -/M2 + + haplotype were significantly increased in patients compared with controls (0.437 vs 0.314, P < 0.05, OR = 1.70). CONCLUSION: It is suggested that both M1 + - genotype and M1 + -/M2 + + haplotype of ApoAI gene might associate with ACI in our study.

OBJECTIVE: To understand the clinical characteristics and variable tendency on inpatients with colorectal cancer admitted to Shanxi Tumor Hospital from 1989 - 1999. METHODS: Statistical analysis on all cases from 1989 - 1999. RESULTS: Results showed that 3,050 cases with colorectal cancer were admitted during 11 years including 2,246 (73.64%) cases of surgical patients. The 1,168 cases males and 1,078 females were with ratio being 1.08/1.0 (M:F). There was no obvious change on the numbers of inpatient from 1991 to 1994, but the numbers was higher than that in 1989 and 1990. The numbers of inpatient rose from 1995 to 1999 year by year. In all the admitted inpatients during the 11 years, 1,348 cases were from countryside and 898 cases from city with the ratio being 1.5/1.0. When adjusted by the general population in 1992, the frequency of the admitted patients was higher in city (14.66/10(5)) than that in countryside (5.96/10(5)). Regarding the distribution of inpatients, it was found higher in Tiayuan city and Jinzhong area than that in else areas. The age of inpatients rose from 40 years and reached high peak in 51 - 70 years. Dukes stages showed that most of inpatients were middle or advanced stage and chief ill location was in rectal (82.36%). Most histological classification referred to adenocarcinoma (56.23%) and papilla adenocarcinoma (20.88%). CONCLUSION: Data suggested that it is very important to strenghen the knowledge of prevention and self-protection of colorectal cancer. Screening and early diagnosis and early treatment should be paid great attention to improve survival rate among high risk population.

[Study on the differentiation and apoptosis of promyelocytic leukemia cells induced by tributyrin].

OBJECTIVE: To investigate the differentiation of NB4 and MR2 cells induced by tributyrin (TB) in combination with different doses of all-trans retinoic acid (ATRA) and to see if TB can induce apoptosis of NB4 and MR2 cells. METHODS: Cell differentiation was analyzed by NBT reduction and CD(11b), CD(14) and CD(33), apoptosis by morphology, DNA electrophoresis, flow cytometry (FCM) and TUNEL in situ hybridization, bcl-2 expression by reverse transcriptase polymerase chain reaction. RESULTS: 0.2 mmol/L TB in combination with different doses of ATRA could potentiate the differentiation of NB4 cells induced by ATRA alone. No MR2 cell could be markedly induced by ATRA alone or in combination with TB. Treated with 1 mmol/L TB for 24 hours NB4 and MR2 cells exhibited a morphological characteristic of apoptosis and typical DNA ladder on gel electrophoresis. FCM analysis showed that TB could interfere with cell cycle in NB4 and MR2 cells, with a similar ratio of sub-G(1) in both cell lines. TUNEL in situ hybridization confirmed these results. Further study showed that TB could gradually down-regulated bcl-2 expression. CONCLUSION: TB exerts synergetic effect on ATRA-induced differentiation and induces apoptosis in promyelocytic leukemic cells.

[Study on mechanism of Shenle and Fragmin in preventing and treating glomerulosclerosis in rats].

OBJECTIVE: To study the effects of Shenle and Fragmin on the degradation of extracellular matrix metalloproteinase-9 (MMP-9) and tissue inhibitors of metalloproteinase-1 (TIMP-1) in the remnant kidneys of 5/6 nephrectomized rats treated with Shenle and Fragmin. METHODS: The model of focal and segmental glomerulosclerosis was made by 5/6 nephrectomy in Wistar rats. After being treated with Shenle (4 g.kg-1.d-1) or Fragmin (1000 IU.kg-1.d-1) for 20 weeks, the blood pressure, proteinuria, serum creatinine, lipids, renal pathological change of remnant kidney of the model animals were examined, and the mRNA expression of MMP-9, TIMP-1 and type IV collagen in the remnant kidney were also detected. RESULTS: Shenle and Fragmin could significantly reduce the blood pressure, proteinuria, serum creatinine and lipids, alleviate the pathologic change of kidney tissue, decrease the type IV collagen, MMP-9 deposition. Shenle and Fragmin down-regulated the mRNA expression of MMP-9 by 30.0% and 28.5% and TIMP-1 expression by 59.3% and 55.0% in the remnant kidney of the 5/6 nephrectomized rats respectively. CONCLUSION: Shenle and Fragmin might ameliorate glomerulosclerosis through modulating MMP-9/TIMP-1 gene expression, alleviate the abnormal metabolism and accumulation, promote extracellular matrix degradation in glomeruli remodeling, it might be the important mechanism of Shenle and Fragmin in treating glomerulosclerosis.

Pattern adaptation of relay cells in the lateral geniculate nucleus of binocular and monocular vision-deprived cats.

To test whether the pattern adaptation in thalamus is dependent upon postnatal visual experience during early life, the responses of relay cells to prolonged drifting grating stimulation were recorded extracellularly from the dorsal lateral geniculate nucleus (dLGN) of cats reared with binocular and monocular lid suture. In binocular vision-deprived cats, 68% of cells recorded showed significant adaptation to prolonged grating stimuli within 30 s, with a mean response decrease of 33%, and then stabilized gradually. This adaptation was stronger than that of relay cells in normal cats. In monocular vision-deprived cats, 53% of the cells driven by the deprived eye showed similar adaptation as did 44% of the cells driven by the non-deprived eye. These results indicate that pattern adaptation could be maintained or even enhanced after visual deprivation in early life. It is suggested that pattern adaptation is a general and intrinsic property of the dLGN cells, which may be mainly determined by genetic factors.

[Expression of platelet derived growth factor receptor-beta in fibroblasts of keloid].

OBJECTIVE: To investigate the expression and distribution of platelet derived growth factor receptor-beta(PDGFR-beta) in normal skin and keloid and to discuss its biological function in keloid formation. METHODS: 1. To detect the expression and distribution of PDGFR-beta in normal skin and keloid tissue by immunohistochemistry; 2. To detect the receptor expression in vitro by Flow cytometry (FCM); 3. To detect the subcellular distribution of receptor by Laser confocal microscope. RESULTS: 1. Immunohistochemistry showed that normal skin and keloid tissue were almost the same in expression but different in distribution of PDGFR-beta; 2. There was more expression of PDGFR-beta in normal fibroblasts than that in keloid fibroblasts in vitro by FCM; 3. Laser confocal microscope revealed that the PDGFR-beta concentrated on the surface of cell membrane in keloid fibroblasts, but in normal skin fibroblasts, the receptors were coagulated on the nuclear membrane and intranucleus. CONCLUSION: Compared with the fibroblasts in vivo, there was a difference of the PDGFR-beta expression in fibroblasts in vitro, more expression of PDGFR-beta in normal fibroblast than that in keloid fibroblast in vitro; and the subcellular distribution of PDGFR-beta was different in normal skin and keloid fibroblasts. The characteristics of the expression and distribution of PDGFR-beta in keloid may contribute to the formation of keloid.

[Ultrastructure of the lymph capillaries in human dental pulp].

OBJECTIVE: The purpose of this article is to observe the ultrastructures of the lymph capillaries in the human dental pulps. METHODS: Investigations have been performed on dental pulps from 8 human permanent teeth, including 4 mandibular first premolars and 4 maxillary first premolars, which were extracted for orthodontic reason. As soon as possible, we cleaved the teeth longitudinally and immersed in the fixative solution at 4 degrees C for 1 hour, then kindly removed the pulps and immersed in the same fixative solution for 3 hours, then routinely postfixed, dehydrated, embedded, and prepared the ultrathin sections (50-70 nm) without semithin section localization. The lymph vessels were distinguished from blood vessels on ultrathin sections by their morphological features. RESULTS: We have observed three fundamental kinds of intercellular junctions, overlapping(51%), end to end(36.1%), and interdigistating (10.6%). A few complex junctions also could be seen, but we failed to find the open junction. The Weibel-Palade bodies were occasionally present in the lymphatic vessels endothelial cells of young dental pulps. And we even found the lipofuscins in the cytoplasm of the endothelium. Some inclusion bodies, such as paracrystalline inclusions and myeloid bodies, also could be seen. There were many micropinocytotic vesicles in the cytoplasm of the lymphatic endothelium, the diameter of the most vesicles was about 50-80 nm, some was about 100-500 nm. Some vesicles appeared to lie free within the cytoplasm, and some were seen to touch or open onto the luminal or abluminal surfaces of the endothelia, which suggested that many vesicles were not simple discrete units but rather from parts of chains that reached either luminal or abluminal surfaces. It was reported that micropinocytotic vesicles, open junctions and intraparietal channels provided fluid transport. CONCLUSION: Dental pulp is a special organ, and the lymph capillaries in it have their own ultrastructural features.
