[The physical capacity of patients with pacemakers].

The pacemaker patient stimulated by unchanged frequency may realize the necessary regulation of his minute output of the heart only through a change of the stroke volume, not through the otherwise usual change of the heart rate. Thus this circle of persons shows a restricted breadth of regulation in orthostatic stress or physical work. Pharmacological or physiotherapeutic measures with vasodilatory effect increase the inclination to orthostasis with security for the acute application, potentially also in a long-term therapy. Greater loads may, probably by a release of catecholamines, reduce the degree of severity of a blockage and evoke a parasystoly in patients with devices with unchanged frequency. Patients with ventricle-regulated pacemaker do not show such disturbances as long as the heart may follow the own regulation and the own rhythm. By the transition to the stimulation with unchanged frequency possible at any time, however, may appear incisive restrictions also in this group of patients. The data of literature are to be confirmed that atrium-regulated pacemakers give the best prerequisite concerning the maximum stress of the patients. In all regulated devices the maximally possible heart rate is demarcated by the pacemaker itself. It is in most cases approximately 120/min. Independent of the type of the pacemaker moreover must be taken into consideration additional restrictions of the functional capacity which are very different from case to case and which are to be referred to the cardiologic basic disease.

[Lipid chemical parameters and incidence of genetically determined hyperlipoproteinemias in patients between 20 and 45 years of age with coronary heart disease].

In 35 patients at the age between 20 and 45 years with a manifest coronary heart disease various lipid-chemical parameters were established and compared with a control group of the same age of laboratory-chemically and clinically inconspicuous test persons as well as with a group of patients with infarction older than 45 years. For HDL-cholesterol, LDL-cholesterol as well as LDL significant differences of the mean value were the result in the group of juvenile patients with a coronary heart disease and in the healthy control group as well as the group of older test persons with infarction, which confirm the accumulation of lipid-chemical risk constellations in "juvenile" patient with a coronary heart disease. Moreover, the proportion of primary hyperlipoproteinaemias is essentially greater in these patients than in older test persons with infarction. Apart from the recognition of primary hypercholesterolaemias the diagnostics of autosomally combined hyperlipoproteinaemias is of particular importance.

The pathologic implication of duodenal diverticula.

In a prospective study of 80 patients with duodenal diverticula and calculous biliary tract disease, bacteriocholia with typical intestinal bacteria was found in 51 of 71 patients, whereas, in cholelithiasis without duodenal diverticula, this complication was present in 278 of 809 patients. With an increasing distance between duodenal diverticula and the papilla of Vater, the rate of bacteriocholia decreases. Advancing age of the patients results in the augmentation of bacteriocholia. Bacteriocholia on the basis of duodenal diverticula seems to present an additional pathogenic factor for inflammatory biliary tract disease and pancreatitis. Consequently, the pathologic value of juxtapapillary duodenal diverticula is much more prominent than has so far been reported.

[Staged lavage therapy in diffuse peritonitis].

Sequential lavage is introduced as a new treatment for severe diffuse purulent peritonitis. Following surgical removal of the focus and intraoperative intraabdominal lavage, the sequential lavage technique was applied with length of treatment dependent on the severity of each case. Data from 21 patients were recorded for the study. Subject lethality was 19%. Three patients survived the so-called "incurable triad".

[Pancreatogastrostomy in papillary cancer].

In the therapeutic treatment of ampullary cancer in 11 patients pancreatogastrostomy was substituted for pancreato-jejunal anastomosis. Pancreatogastrostomy is a simpler technique and is more reliable than sutures with the small intestine. In the postoperative phase the anastomosis can be monitored at any time by radiological and endoscopic examination. However, examination of lipase in the gastric juice shows that even these anastomoses are closed in two patients as early as the second year after the operation. The frequent closure of anastomoses speaks in favour of the discontinuation of the exocrine function of the pancreas during the operation by means of occlusion jelly together with pancreatogastrostomy for the transitory protection of the resection surface of the pancreas.

[ECG changes in the late postoperative course of patients following alloplastic heart valve replacement].

Based upon a total of 50 patients with alloplastic heart valve substitute an attempt was made to verify postoperative retrogression of the signs of hypertrophy in the ECG and to correlate them with the clinical findings. In accordance with the pertaining literature, the essential criterion for assessing left hypertrophy is the Sokolow index. In corrected aortic lesions there is a close relation between retrogression of electrocardiographic signs of hypertrophy and the clinical degree of severity. All patients with a retrogressive tendency of the signs of hypertrophy showed definite improvement of clinical findings. Patients in whom the ECG deteriorated did not have good clinical findings either. This clear relationship is not demonstrable in mitral lesions, since mitral stenosis and mitral insufficiency influence the left ventricle, the left atrium and the pulmonary circulation in various ways. A discussion of the assessment of the individual ECG symptoms and their causes is added.

[Diagnosis of insufficient perforating veins using thermographic plates].

With the help of thermographic plates developed by the department of physical chemistry of the Martin Luther University Halle it was possible to detect in 50 patients 158 insufficient venae perforantes, which were found by x-ray examination, from total of 175. The causes for 17 insufficient venae perforantes not diagnosed thermographically were inadequate application of the plates in ulcus cruris, the existence of a pair of perforating veins and functionally unimportant bypasses. Plate thermography proved to be suitable in the lower leg for the detection of smaller differences in temperature. Evaluating these results in comparison to the literature references there was no difference between our method and the infrared thermovision. From an economic point of view plate thermography seems to be superior to infrared thermovision. In contrast to this, plate thermography has the disadvantage of a longer time for examination and problems in documentation and registration of the results.

[Alcohol and the pancreas].

In the last decenniums the importance of alcohol for the pathology of the pancreas has increased in Middle Europe. The inhibition of the pancreas secretion which was to be observed at an occasional alcohol intake is when a regular ethanol supply is present changed by a hyperfunction with secretion of a pancreatic juice rich in protein with high enzyme and calcium content. This predisposes to the protein precipitation in the small and medium ducts with resulting intrapancreatic obstruction, which on its part gives the impact to the changes of chronic pancreatitis. In 80-90% of the cases the ethanol-induced lesion of the pancreas is manifest under the picture of an acute pancreatitis. Nevertheless, in most cases the first shift of a chronic relapsing pancreatitis is concerned, the peculiarity of which is the frequent occurrence of calcifications. Clinical picture, complications and sequelae are described.

[Starch granulomas caused by glove powder].

Simultaneously with the introduction of new surgical gloves with heavily powdered outside surfaces, complications repeatedly occurred after laparotomies. Four patients were compelled to undergo a second operation, during which peritoneal fibrous adhesions and foreign body granulomas of omentum were found. After resection of the omentum, healing proceeded without disturbance. Five different types of surgical rubber gloves were examined by scanning electron microscopy. In addition, the effect was evaluated of different commonly applied preoperative rinsing procedures for gloved hands. The amount of powder found on the outside of regularly used gloves varied between 0.059 and 0.287 g. The effect on the proper healing process by starch powder deposits found on the outside of surgical gloves as well as upon optimal cleansing of the surgical teams' hands will be discussed.

[Problems in hypertension therapy in patients with a constant frequency pacemaker].

Patients wtih pacemakers with constant frequency must cope with their problems of the regulation of the circulation without having the possibility of changing the heart rate. These are not inconsiderable under everyday conditions, e.g. in the orthostatic experiment. A large number of medicaments with coronary and antihypertensive effects disturbs the periphery of the circulation as the possibility of regulation which persist in the patients with pacemakers. On the basis of orthostatic experiments on patients with pacemakers with constant frequency the acute effect of drugs, such as nitroglycerin, trapidil, dihydroergotoxin and aminophyllin as well as the long-term effect of antihypertensive drugs concerning the disturbances of the orthostatic regulation is dealt with. The acute effect especially of nitroglycerin is clinically relevant. Concerning the permanent therapy with antihypertensive drugs, there exists probably a possibility of adaptation. In oral application Rauwolfia alkaloids and clonidin do not show a disadvantageous influence on the orthostatic behaviour.

[Screening for hypertension by means of automatic measurement of the systolic blood pressure according to the Penáz-principle].

The plethysmomanometer after Penáz allows the automatic measurement of the systolic blood pressure in a finger artery. On the basis of the comparative measurements on 250 patients the result was a probably linear correlation with a correlation coefficient r = 0.821 between the systolic blood pressure after Riva-Rocci and the systolic pressure of the finger artery. Only in few cardiologic diseases (e.g. disturbances of the cardiac rhythm, severe cyanotic failures, arteriovenous fistules on the forearm) the devices cannot be used. Provided with adequate technical improvements the device might be used for the screening of hypertension.

[Complaints following cholecystectomy].

A subdivision of the most frequent complaints after cholecystectomy is given. For the clarification of the manifold causes of these inhomogeneous pictures of complaints a considerable diagnostic expenditure is necessary, in which case the importance of ERCP is particularly emphasized. In concrements of the choledochus as well as in short-distance papillary stenoses nowadays above all the endoscopic papillotomy is therapeutically taken into consideration. Recommendations are given for the medicamentous treatment of functional complaints.

[Serotonin and dumping syndrome (author's transl)].

5-hydroxyindole acetic acid, xanthurenic acid and kynurenin, --that is to say the major metabolites of serotonin, were measured in the urines of 31 patients with dumping syndrome. Serotonin concentrations were measured as well in peripheral venous blood before and after a glucose load in 47 patient with dumping syndrome. All values measured were in the normal range. Comparison of serotonin levels in peripheral and portal venous blood yielded normal results as well. It is therefore concluded, that serotonin does not play an essential role in the etiology of the dumping syndrome.

[Prevention of surgical wound infections by means of ring drapes].

In order to test the efficiency of wound protecting ring drapes and incisional drapes we carried out a prospective study of 283 patients, who underwent larger operations of the gastrointestinal tract from 1975 to 1979. Whilst complications of wound healing following operations of the upper gastrointestinal tract were not reduced, a reduction of the rate of secondary wound healing by 20% was achieved after operations on the large bowel. Overweight did not cause a higher rate of wound sepsis as it is usually the case when wound protecting drapes are not used. A further significant reduction of complicated wound healing could be observed after preoperative systemic application of antibiotics. This effect could not be stated when antibiotic prophylaxis was started after the operation.

[Bacteriological findings in patients with cholelithiasis and duodenal diverticuli].

In a prospective study of 70 patients with duodenal diverticula and calculous biliary tract disease, bacteriocholia with typical intestinal bacteria was found in 69%, whereas in cholelithiasis without duodenal diverticula this complication was present in 30% of the cases. With growing distance between duodenal diverticula and the papilla of Vater the rate of bacteriocholia decreases. Rising age of patients results in augmentation of bacteriocholia. Bacteriocholia on the basis of duodenal diverticula seems to present an additional pathogenetic factor for inflammatory biliary tract disease in 69% and for pancreatitis in 20% of the cases. Consequently the pathologic value of diverticulas close to the papilla of Vater is more prominent than reported so far.
