[Computer-assisted analysis of the tocogram. Initial experience using a new evaluation procedure].

This paper describes first observations with a new computer aided analysis of tocogram. It seems that length of uterine contraction is an important parameter for quality of labour. In 9 from 12 cases promotion to constant length of labour with relatively low absolute duration of contractions was connected with a distinct increase of the velocity of cervical dilatation. The uniformity can be an important factor for the conversion into active phase of labour.

Detection of early pregnancy factor (EPF) in pregnant and nonpregnant subjects with the rosette inhibition test.

We tested for early pregnancy factor (EPF) using a rosette inhibition test with polyclonal anti-lymphocyte serum from the horse and two monoclonal antibodies specific for the E-receptor of T-lymphocytes. When lymphocytes were preincubated with early human pregnancy sera, rosette inhibition titres were four or more dilutions higher (P less than 0.01) than when lymphocytes were preincubated with nonpregnant sera.

Human early pregnancy factor: serum concentrations before and after therapeutic abortion in comparison with beta-hCG and an early pregnancy associated protein.

Serum concentrations of the early pregnancy factor (EPF), beta-hCG and the early pregnancy associated protein (EPAP) were measured in 12 patients before and after therapeutic abortion for social-medical reasons. Detection of EPF was performed by the rosette-inhibition assay, beta-hCG quantification by radioimmunoassay and EPAP measurements by rocket immunoelectrophoresis using a monospecific polyclonal rabbit antiserum. The disappearance rate of EPF after termination of pregnancy was closely correlated with the decrease of beta-hCG concentrations (rpbis = 0.71) and loosely correlated with the decrease of EPAP concentrations (rPbis = 0.43). No correlation has been found between hCG and EPAP values.

Human early pregnancy factor and early pregnancy associated protein before and after therapeutic abortion in comparison with beta-hCG, estradiol, progesterone and 17-hydroxyprogesterone.

Serum activities and concentrations, respectively, of early pregnancy factor (EPF), early pregnancy associated protein (EPAP), beta-hCG, estradiol, progesterone and 17-hydroxyprogesterone were estimated in 20 healthy primigravidae within the tenth to twelfth completed gestational week before and after therapeutic abortion. EPF, beta-hCG and estradiol markedly decreased after termination of pregnancy, whereas progesterone and 17-hydroxyprogesterone concentrations moderately fell. EPAP showed no significant alterations in a 24 hours period. Because of its possible role as immunosuppressive substance and its short half-life EPF may be a promising immunobiomarker for disturbances in early pregnancy.

[Sex steroids and the immune system].

Endocrine and immune system are strongly correlated. Hormonal contraceptive drugs influence humoral, cellular and mucosal immunity in different kind and in dependency of the relationships between estrogens and gestagens. At present most authors support the opinion that estrogens are responsible for immunosuppressive features. This review summarizes experimental and clinical reports.

[Early embryonal signals].

As early pregnancy signals are known up to now: early pregnancy factor (EPF), early pregnancy associated protein (EPAP), early pregnancy associated thrombocytopenia (EPAT), platelet activating factor (PAF) and the so-called amplified pregnancy signals. Clinical relevance in the near future may obtain EPF estimations for diagnosis of early pregnancy after sterility therapy and for monitoring of the first trimester of pregnancy.

[Pregnancy-associated alpha2-glycoprotein (alpha2-PAG)--past, present and future].

The pregnancy-associated alpha 2-glycoprotein (alpha 2-PAG), first described 30 years ago, is a high-molecular neuramino-glycoprotein. Its serum concentration is normally low, but substantially increases during pregnancy and in some malignant tumour diseases. A review is given of occurrence and quantification of alpha 2-PAG under physiological and pathophysiological conditions, site of synthesis and localization, purification and physico-chemical properties, biological role and possible clinical use.

[Multicenter study of obstetrico-perinatologic quality control in the Rostock district. I. Comparative medical statistics].

Report about outcome-quality assessment in obstetrics and perinatology using an uniform check list for data collection in the district of Rostock. Various possibilities of outprint of the results in 1985 are demonstrated. For daily data collection in obstetrics a special book has been developed. The investigation concerns a data pool of more than 55,000 deliveries. Considering a trend of the results the following optimal criteria in obstetrics and perinatology are postulated: (table; see text) The check list of the Society of Perinatal Medicine of the GDR for obstetrical and neonatal data collection should be generally used. The soft ware for the computed data analyses has been developed by the "Rechenzentrum der Ernst-Moritz-Arndt-Universität Greifswald". Analysis and output of obstetrical and perinatological datas by means of a personal computer are prepared.

[Detection of early pregnancy factor by the rosette inhibition test: use of cyclosporin A].

For the detection of the early pregnancy factor (EPF) as early pregnancy signal and as tumour marker only the rosette inhibition test is generally accepted. In the testing of 10 EPF-positive and 10 EPF-negative sera the immunosuppressive agent cyclosporine A was used for the first time. The differences between both groups were highly significant (alpha less than 0.001). Similar results were obtained by using the more expensive anti-human T lymphocyte globulin.

Regional hyperthermia in the treatment of recurrent deep-seated tumors: preliminary analysis of phase I trial.

This is a preliminary report on a phase I pilot study of regional hyperthermia of advanced tumors, using annular phased array system (APAS) in combination with radiation therapy and/or chemotherapy. All patients were treated without anesthesia. In all hyperthermia sessions, physiological parameters such as blood pressure, cardiac rate, respiratory rate, and EKG were monitored throughout the treatment. Tumor temperature was monitored with intratumoral probe with an additional probe either in the esophagus or rectum for monitoring systemic temperature. Present reports are on analysis of the first 30 patients: a total of 139 treatment sessions were analyzed. The results indicate local discomfort, anxiety, systemic temperature elevation, and tachycardia were the predominant factors limiting the number of sessions, the duration of sessions, and the deposition of power, in 18, 6, 6, and 2 patients, respectively. The complications were rare. No injuries to visceral organs clearly attributed to the hyperthermia have been recorded.

Regional hyperthermia--assessment of tolerance to treatment.

To assess the patient's tolerance and the complications of treatment associated with delivery of regional hyperthermia using APAS BSD equipment, the data on 30 patients who received regional hyperthermia at the Radiation Oncology Center, Washington University School of Medicine, St. Louis, Missouri, has been reviewed. Most patients presented with advanced or recurrent tumors not amenable to conventional treatment modalities. Hyperthermia was administered in combination with either radiotherapy or chemotherapy. Cardiovascular evaluation was a standard part of pre-treatment work-up. Temperatures were obtained through interstitial probes, catheters inserted into the body cavities and (to record core temperature) esophagus. Local discomfort, anxiety, systemic temperature elevation, and tachycardia were the predominant factors limiting the number of sessions, the duration of sessions and the deposition of power, in 18, 6, 3, and 2 patients, respectively. The complications of treatment were rare. Significant injuries of the superficial tissues were recorded in two patients in whom this could be attributed to contact of urine and stool with the skin surface. Neuropathy was observed in two patients, both of whom had pelvic masses adjacent to or invading into the affected nerves. Both patients had received pelvic irradiation. Neuropathy developed within several days after the first hyperthermia session and proved reversible within several months. No injuries of the visceral organs that could be clearly attributed to hyperthermia have been recorded.

[Significance of the detection of early pregnancy factor for monitoring normal and disordered early pregnancy].

In twenty patients with the tentative diagnosis of disturbed early pregnancy, the early pregnancy factor (EPF) was estimated by means of the rosette inhibition test and the early pregnancy associated protein (EPAP) by rocket-immunoelectrophoresis. The results were compared with hCG concentrations in the urine, beta-hCG concentrations in the serum as well as with clinical and sonographical findings. In intact pregnancies, positive EPF values are an additional sign of embryonic viability. Negative EPF values can be an early indicator for pregnancy losses. The diagnosis of an ectopic pregnancy can be confirmed by negative EPF values in spite of elevated hCG levels and uncertain sonographic findings. In cases of missed abortion the EPF estimation was always negative in contrast to other parameters of pregnancy. EPF detection may be helpful to differentiate menstruation disorders and early pregnancy losses. Because of its longer half-life, the EPAP is not such a sensitive parameter for early pregnancy disturbances as the EPF.

[Behavior of T-lymphocytes during the normal menstrual cycle].

FSH, LH, estradiol-17 beta, progesterone, and T-lymphocytes were examined daily in 12 biphasic cycles of 6 healthy women (age 19-29 years). T-lymphocytes were estimated by the alpha-naphthyl-acetate-esterase-reaction as T-cell-marker. There was a significant reduction of T-cells in the first and a nonsignificant reduction in the second cycle phase. The suppressions were related to the high estradiol levels in the follicular as well as the luteal phase. If there is a causative connection and whether the whole immune system is depressed or other regulatory mechanisms interfere isn't quite clear.

[Endometrium proteins].

Research in animal and human reproduction attends with proteins of the endometrium increasingly. Especially in the time of the blastocyst implantation and during survival of the fetal allograft the protein pattern of the endometrium could be important. In the development of the blastocyst of rabbits uteroglobin rendered as a sensitive marker for the biological activity of progesterone. In animal models the desynchronisation of the uteroglobin secretion demonstrated the inhibition of the implantation. In human an alpha 2-globulin was found represented an analogon regarding to rabbits uteroglobin possibly. In the female genital host defense mechanisms of the mucosal barriere especially local produced immunoglobins are important. During pregnancy decidual suppressor factors the maternal tolerance of the fetus.

[Pregnancy determination by the detection of early pregnancy associated protein].

First results of the immunoelectrophoretic detection of an early pregnancy associated protein (EPAP) are reported. For this a rabbit antiserum was produced against a serum fraction, which had early pregnancy factor activity, and used for rocket immunoelectrophoresis. By this method 95.3 per cent of 64 sera from patients with intact pregnancies were classified to be pregnant. There was found a positive correlation (r = 0.05; p less than 0.01) between the concentration of EPAP and the period of gestation (4th to 14th gestational week). However, the EPAP antiserum also reacted with serum proteins from probands using hormonal contraception. Further characterization and purification of the EPAP will help to produce an antiserum with higher specificity and therefore it will be possible to make a more reproducible early pregnancy test.

[Effect of maternal constitutional factors and weight increase in pregnancy on the neonatal measurements of weight and length].

The significance of maternal constitutional factors and of maternal body weight increase during pregnancy for the condition of the newborn was investigated in 9,578 pregnancies. Statistical calculations were made by means of two-dimensional contingency table analyses. The body size of the mother, her relative weight at the beginning of pregnancy and the increase of body weight during pregnancy were correlated positively with the measures of the newborn. The weight of newborn has been mostly influenced by the maternal body size. It seems to be necessary to include the maternal constitution in the clinical valuation of the newborn status. The use of regional correction tables enables a more exact identification of hypotrophic newborns.
