Congenital multiple plaquelike glomus tumors.

We encountered two patients with congenital multiple plaquelike glomus tumors. These lesions were present at birth and enlarged with body growth, attaining a diameter of up to 13 cm. The diagnosis was confirmed by histologic and electron-microscopic examination, which revealed the typical changes of glomus tumors. Immunocytochemistry findings demonstrated the tumor cells to be vimentin- and alpha-smooth muscle-actin-positive and desmin-negative. On ultrastructural examination, typical dense bodies and attachment plaques were easily found within the tumor cells. Glomus cells were coated by a thick basal lamina.

[Unusual primary cutaneous localized amyloidosis].

The case of a 41-year-old female patient with an unusual type of primary localized cutaneous amyloidosis is reported. Clinical examination revealed lesions typical for macular cutaneous amyloidosis, while histology and histochemistry, in contrast, revealed the presence of nodular amyloidosis of the skin with deposition of lambda light chain amyloid.

Dermatitis herpetiformis bodies. Ultrastructural study on the skin of patients using direct preembedding immunogold labeling.

Skin samples from three adult patients with dermatitis herpetiformis (DH) and granular IgA deposits in the papillary tips were studied using ultrastructural immunogold technique. IgA positive, so-called DH bodies were identified as amorphous clumps--most probably immunocomplex aggregates--scattered throughout the upper papillary dermis. Dermatitis herpetiformis bodies were seen underneath the basement membrane, sometimes along microfibrillar bundles, as well as adjacent to the papillary collagen fibers and within the surface (microfibrillar) region of elastic tissue. Some DH bodies, however, were not related to any fibrillar components. The collagen and elastic fibers, microfibrillar bundles, anchoring fibrils, and elastic microfibrils themselves were unlabeled. Dermatitis herpetiformis bodies were not found in normal human skin. The results of our ultrastructural study indicate that DH bodies either are bound to a nonfibrillar component of dermal connective tissue or represent deposits of immune complexes trapped in DH skin.

[Colloid milium].

The differential diagnosis and classification of colloid milium, a relatively rare disorder, may be difficult. In this paper, a case report of a 52-year-old male patient with characteristic clinical and histological signs of colloid milium is therefore presented. Recently gained insights into the aetiopathogenesis of colloid milium are discussed.

[The dermatoscope. A simplification of epiluminescent microscopy of pigmented skin changes].

The poor prognosis of thick malignant melanomas emphasizes the importance of their early detection. Skin microscopy using a stereomicroscope has been shown to increase the accuracy of clinical diagnosis significantly in pigmented skin lesions. Owing to the development of the Heine Delta 10 dermatoscope, skin microscopy has become less difficult and more rapid. Therefore, this technique can now be applied routinely in daily office practice.

Association of early malignant melanoma with nevocytic nevi.

The percentage of histologically associated malignant nevocytic nevi is essential for establishing concepts of histogenesis. The literature abounds with studies of this association. However, the results are conflicting and vary between 4% and 72% of malignant melanomas with associated nevocytic nevi with a maximum frequency between 20% and 30%. These different values could be partially explained by the fact that tumors in more advanced stages might have "overgrown" preexisting nevocytic cells. For this reason, only cases of early stage malignant melanoma (tumor thickness less than or equal to 1.5 mm; level of invasion less than or equal to III) were included in this study (n = 150). Thirty-three (22%) of 150 cases of malignant melanoma were associated with nevocytic nevi. In an additional 6.1% of the cases there was a possible but doubtful association. In 45.4% of the cases the nevus cells could be detected below the tumor, in 18.2% they could be seen at the laterally adjacent sides, and in 36.4% they were found at both sites. Fifty percent showed junctional activity from the nevocytic nevi located to the side of the melanoma. We conclude that the finding of approximately 27% of nevocytic nevi with junctional activity among the nevocytic nevi associated with malignant melanoma could be an indicator that at least some malignant melanomas develop within or next to a preexisting and still proliferating nevocytic nevus.

An automatic analysis method for in situ hybridization using high-resolution image analysis.

Specific mRNA for alpha 1 (I) collagen has been detected on a cellular level by in situ hybridization using radioactively labelled alpha 1 (I) antisense RNA probes. We here present an automatic, quantitative, evaluation technique for the determination of grain densities using the high-resolution image analysis system IPS KONTRON. The reliability and objectivity of this method were evaluated by comparing the values of grains per cell obtained by conventional and automatic techniques following in situ hybridization with alpha 1 (I) collagen DNA probes in various specimens. The correlation coefficients between conventional and automatic analysis of grain densities were 0.97 for fibroblasts embedded into a three-dimensional collagen gel, 0.94 for dermal fibroblasts in skin obtained from a patient with progressive systemic scleroderma, and 0.90 for fibroblasts in a 2-week-old scar. All correlation coefficients were significant on the P less than 0.0001 level. This new analysis technique therefore allows a more rapid and reliable quantitative evaluation of in situ hybridization and may also be helpful in differentiating between various cell populations characterized by different biosynthetic capacities.

In situ hybridization--a useful tool for studies on collagen gene expression in cell culture as well as in normal and altered tissue.

We report the application of antisense RNA probes for in situ hybridization to identify collagen type I and type III mRNA synthesizing fibroblasts under in vitro and in vivo conditions in normal and wounded human skin. Non-specific hybridization was excluded by specific distribution patterns of alpha 1(I)- and alpha 1(III) probes in mouse fetuses. In addition, the specificity of hybridization was checked by sense probes, radioactively labelled transcripts of Gemini vectors and a keratin probe. In normal skin weakly activated fibroblasts were sparsely scattered within the dermis, while in wound healing processes mRNA both for alpha 1(I) and for alpha 1(III) was dramatically increased, thus suggesting that collagen synthesis is at least partly regulated at a pretranslational level. In addition, the intensity of the labelling, as defined by image analysis and the distribution pattern of collagen mRNA synthesizing cells, provide strong evidence that wound healing by primary intention starts within the deep dermis.

Semiquantitative analysis of histologic criteria in thin malignant melanomas.

To evaluate the sensitivity of histologic criteria 396 hematoxylin-eosin-stained step sections of 150 randomly sampled thin malignant melanomas (tumor thickness, less than or equal to 1.5 mm; level of invasion, I-III) were analyzed. The highest sensitivity was determined for nuclear atypia (84%), followed by the asymmetry of the lesion (81.3%) and the presence of single cells in the upper layers of the epidermis (80.3%). The sensitivity was limited for the inflammatory reaction of the dermis (56%), poor demarcation of the lesion (49.3%), and infiltration of the adnexa (47.7%). For the mitotic activity a low sensitivity was found (33.3%). In only 7.6% of cases all criteria investigated (with the exception of the infiltration of adnexa) were simultaneously present. The three most sensitive criteria could be found in 46% of cases and two of these criteria in 88% of cases. In conclusion, this semiquantitative analysis of histologic criteria revealed that (1) thin malignant melanomas can be diagnosed with a few, but very sensitive, criteria and (2) the absence of criteria with minor sensitivity is not helpful for correct diagnosis.

[Marshall-Marchetti-Krantz operation with fibrin gluing. Results after 2 years].

Since broad experience concerning the stability of fibrin sealing is needed, it has become part of the routine program in orthopaedics and in traumatology. Because of these experiences, we decided to modify the standard Marshall-Marchetti-Krantz-Operation replacing all sutures on both sides of the urethra and the bladder neck by fibrin sealing. So far, 76 patients have been operated using to this new method. All patients had a urinary stress incontinence grade II-III, urodynamically verified and a pronounced urethro-cystocele. To judge the results of the operation the following check-ups were instituted: 1. Urodynamic control, 2. lateral cyst-urethrogram, 3. gynecological examination (anatomical control), 4. subjective, individual evaluation of the patient concerning the involuntary loss of urine. Our study shows that we achieved good surgical and functional results as can be demonstrated both clinically and by urodynamic check-up. The advantage of this method of operation using fibrin sealing is to be seen in the broad lifting up of the anterior vaginal wall without unphysiological fixation and at the same time avoiding all risks of sutures.

[Evaluating the cervix in pregnancy using vaginal sonography. Studies of cervix insufficiency].

Transvaginal ultrasound provides important detailed information, which cannot be obtained by routine manual vaginal examination. By measuring the outside-inside distance (from os externum to os internum), we can determine the actual length of the cervix. By depicting the cervical canal, we can identify early dilatation. Visualizing the os internum with a possible opening, forming a conus, helps reveal an incompetent cervix. A total of 485 measurements of the cervix was carried out. Fifty patients with normal clinical findings between 28 and 32 weeks of pregnancy were compared to the same number of patients in the 28th to 32nd week of pregnancy, with the clinical diagnosis of an incompetent cervix. A cervical length of 46.3-39.3 mm was found in normal pregnancies, and a length of 34.0-21.4, when a clinically incompetent cervix had been diagnosed, a difference of 12.3-17.9 mm.
