Occurrence of IgE-bearing epidermal Langerhans cells in atopic eczema: a study of the time course of the lesions and with regard to the IgE serum level.

Uninvolved and lesional skin of untreated and treated patients with atopic eczema has been investigated immunohistochemically to determine the conditions in which IgE-bearing CD1a+ Langerhans cells/indeterminate cells (LC/IC) occur in this disease. IgE-bearing epidermal dendritic cells were present in patients with elevated IgE serum level (greater than 300 UI/ml) and the staining pattern was stronger in lesional skin. On double immunostaining, a subpopulation of CD1a+ LC/IC was found not to bear IgE molecules as determined by the ratio IgE+/CD1a+ cells on serial sections as well. The ratio IgE+/CD1a+ cells decreased in patients who underwent a local therapy with glucocorticosteroids. These results suggest that the expression of IgE receptors and/or binding of IgE molecules on epidermal LC/IC in atopic eczema may be controlled by a complex network of mediators from the epidermis or the inflammatory infiltrate, or both, and that this phenomenon could be down regulated by glucocorticosteroids.

Structure of basement membranes in malignant melanoma and nevocytic nevi.

Basement membranes found around tumor cells in nevocytic nevi, Spitz's nevi, and malignant melanomas were analyzed by electron microscopy and antibody staining for several basement membrane proteins. Nevocytic nevi and Spitz's nevi showed a distinct, occasionally discontinuous lamina densa regardless of whether they were located in junctional zones of the epidermis or within the dermis. All basement membranes around nests of aggregated nevus cells, however, lacked anchoring fibrils. This correlated with the absence of type VII collagen. In contrast, type IV collagen, laminin, and nidogen were present at the periphery of the nevus cell clusters in agreement with the presence of an intact lamina densa. Aggregated tumor cells in malignant melanomas were bordered by a lamina densa when located in a junctional position and lacked this structure when they had migrated into the dermis. This process was accompanied by a drastically reduced staining for collagen type IV and nidogen, whereas laminin was still detectable. Anchoring fibrils and their molecular correlate, type VII collagen, were consistently absent. These observations demonstrate major alterations in the composition of basement membranes around malignant melanomas, which can be an important factor for the invasive growth and formation of metastases of these tumors.

Hormonal and non-hormonal medical therapy of benign breast disease.

Mastopathy or nodular painful breast can be managed by several non-hormonal and hormonal treatments. Little data exist for effectiveness of the former. Whilst the hormonal treatments are considered to be very effective, their use can be limited by unpleasant side effects.

Localization of collagen alpha 1(I) gene expression during wound healing by in situ hybridization.

The cellular localization of collagen alpha 1(I) chain gene expression during wound healing in rats was investigated using in situ hybridization. Activation of collagen gene expression was first found within fibroblastic cells around the deep layers of the granulation tissue as early as 16 h post wounding. Heavily labeled cells were also detected near the intact wound edge and around hair follicles. At day 6 intense alpha 1(I) collagen gene expression was found within most cells of the granulation tissue and after day 8 most of the activity was localized to cells directly underneath the epidermis. 26 d after the induction of the wounds hardly any alpha 1(I) collagen gene expression could be demonstrated, which indicates a close, time-dependent control of collagen synthesis during repair processes.

[Ectopia cordis. Report of a prenatally diagnosed cases and a short review of the literature].

Ectopia cordis is defined as a congenital malposition of the heart partially or completely outside the thorax. Severe structural malformations of the heart are very frequently associated and are responsible for the extremely poor prognosis. Only 16 out of 219 cases reported in the literature up to now have survived. We report on a case of Ectopia cordis thoraco-abdominalis associated with an univentricular heart and a hypoplastic pulmonary vascular bed detected prenatally in the 32nd week of gestation. The male newborn died 23 h after a spontaneous and uneventful delivery at term. Considering the poor prognosis no surgical attempts had been undertaken to correct the malformation.

[Disseminated herpes zoster in chronic lymphatic leukemia].

A 73-year-old woman suffering from chronic lymphatic leukemia presented a rare variant of zoster with disseminated eruptions. Ultrastructural negative staining revealed viruses of the herpes group. Herpes simplex was excluded by means of fluorescence-labelled monoclonal antibodies (HSV-1/HSV-2 direct specimen identification/typing test). It is proposed that the conventional classification of zoster into a segmental and a generalized variant should be supplemented.

[Application of the neodymium-Yag laser in treating adhesions].

The authors conducted an experimental study on the possibilities of application of the "in touch" technique for microsurgical adhesion resection with a Neodymium-Yag laser. It enables good hemostasis and is an alternative to the CO2 laser, the risk of causing vital organ lesions being reduced.

Detrusor and compliance changes of the bladder after radical hysterectomy.

Disturbances of bladder function after radical hysterectomy are caused by the damage done to the pelvic nerves. Reduced radicality in cases of carcinoma cannot be discussed. In a prospective urodynamic study we checked bladder changes in patients with radical hysterectomy and compared them to those with incontinence operations. Shortly after operation compliance and detrusor function are severely reduced. After 6 months compliance is normalized, detrusor is still reduced, yet residual urine normal. The patient has learned to use abdominal pressure, thus providing a functional normality.

[Malignant atrophic papulosis (Köhlmeier-Degos disease)].

On the basis of 106 patients with malignant atrophic papulosis in the literature, including one case of our own, the clinical, histopathological and nosological features of this disease are reviewed. Histopathologically, the typical papules with central porcelain-white atrophy show broad wedge-shaped necrosis of the connective tissue due to thrombotic occlusions of small vessels in the corium. The disease leads to death in 50% of these cases, mostly due to involvement of the gut and the central nervous system. Malignant atrophic papulosis is the prototype of a disease, in which skin lesions are the primary sign of a systemic disorder demanding the interdisciplinary cooperation of various specialists.

[Keratoacanthoma centrifugum marginatum].

Keratoacanthoma centrifugum marginatum (KCM) is a rare distinct variant of keratoacanthoma. Based on three personal observations and a review of the literature, the authors describe the clinical and histological features of this neoplasm. Clinically KCM is characterized by the lack of a tendency for spontaneous remission and by continuous centrifugal spread. Histologically there is a subclinical, iceberg-like growth pattern. Like keratoacanthoma, KCM is a highly differentiated, biologically benign, non-metastasizing tumour. The treatment of choice is early excision of the tumour.

Tumor cell growth fractions in human malignant melanomas and the correlation to histopathologic tumor grading.

The growth fraction (GF) of 72 human malignant melanomas was determined by immunostaining with monoclonal antibody Ki-67. A positive correlation of GF and histopathologically-assessed prognostic variables, such as tumor thickness, mitotic rate, and prognostic index, was found. Individual Ki-67 values were considerably scattered in all histologically defined groups of malignancy. Thus, GF as determined by Ki-67 was used to calculate a modified prognostic index. In contrast to the histologically defined prognostic index, the Ki-67 based prognostic index allows further subdivision of thin malignant melanomas with no or few mitotic figures. This may be of help in identifying tumors with a high recurrence potential.

[Lentigo of the glans penis].

The report deals with two male patients with penile lentigo. Histologically acanthosis, basal hyperpigmentation, and melanocytes without atypia were found. Ultrastructural analysis revealed the presence of single melanosomes within the keratinocytes as the most prominent feature of penile lentigo. In one patient, topical treatment with acelaic acid resulted in some improvement within 4 months.

[Disseminated superficial actinic porokeratosis with Bowen's disease].

While there have been several reports about the development of malignant neoplasms in porokeratosis of Mibelli and porokeratosis plantaris, palmaris et disseminata, only one case of squamous cell carcinoma in disseminated superficial actinic porokeratosis (DSAP) has been documented. A review is presented of the variants of porokeratosis that have so far been described in the literature as well as the differential diagnosis, genetic aspects and therapy of DSAP. The first case of DSAP with Bowen's disease is presented.
