[Impact of synthetic sexual steroids on blood volume (author's transl)].

The isotope dilution technique was used to study the oestrogen component of "Deposiston", a long-acting contraceptive, and its effects on blood volume. Blood volume increased by about ten per cent within 15 days from administration of 3 mg of ethinyloestradiol-sulphonate (1 mg/week). Haematocrit was reduced, at the same time. This haemodynamic initial effect should be always taken into account for any assessment of biochemical parameters in response to hormonal contraception.

[The lateral cervical cyst as a cystic lesion of a lymph node (author's transl)].

By injecting a blue stain into the tonsillar region in patients having a lateral cervical cyst the lymphatic channels of the oropharynx were identified. The distribution of the stain within the capsule of the cyst was observed during the operation. This experiment, carried out in three cases, indicates that lateral cervical cysts may be interpreted as cystic lesions of a lymph node. Inflammation of the nasopharynx and oropharynx is believed to play a role in actuating the process leading to a cyst.

[Causes and mechanisms of orbit and eye injuries].

Clinical findings on 219 ball injuries demonstrate that whereas air-filled balls do not produce fractures they do produce skin and eyeball lesions. In 264 cases, orbital fractures, including blow-out-fractures, were caused by inelastic objects hitting the maxillo-facial region. In this group, no trauma of an air-filled ball, especially no tennisball trauma, was seen. The pathomechanism of elastic and inelastic objects is presented with special regard to the effect of suction at the moment the air-filled ball returns from the orbital region.

[Changes in plasma volume and electrolytes after treatment of therapy-resistant hypertension with diazoxide and furosemide].

10 patients (5 females, 5 males, average age 50.2 +/- 5.8 years) with a therapy-resistant hypertension were for 10 days treated with injections of diazoxide and furosemide (hypertonalum and furesis). A significant decrease of the blood pressure as well as a decrease of sodium and potassium in the serum occurred, where as the electrolyte content of te 24-hour collection urine was influenced in homogeneously. The blood and plasma volume, respectively, was also significantly reduced. Heart rate and blood sugar significantly increased, urea-N, creatinine, hematocrit and hemoglobin did not increase significantly. The new regulation of the blood pressure to an oral antihypertensive permanent therapy continued 4 weeks to 6 months.

[Cessation of treatment in childhood acute lymphatic leukemia. Long-term observations after meningosis prevention with intrathecal gold colloid radioisotopes and methotrexate].

Cytostatic long-term treatment for about 36 months was administered to 18 children with acute lymphatic leukaemia who were in long-term remission (43-98 months). Prophylaxis of meningosis involved intrathecal 198Au colloid and methotrexate. Seven recurrences occurred during the long-term remissions: the bone marrow was involved six times, leukaemic meningosis occurred once. Five out of seven recurrences occurred within one year of cessation of treatment. There were no complications induced by intrathecal radio-gold, such as the apathy syndrome or leukoencephalopathy. Intrathecal methotrexate led to side effects before administration of radio-gold: encephalopathy twice, paraplegia once. Symptoms regressed completely in two children, one child with encephalopathy continues to have symptoms. All three children were given 198Au colloid intrathecally thereafter which was tolerated very well. 198Au colloid represents an alternative for prophylaxis of meningosis with 60Co telecobalt irradiation in leukaemias and non-Hodgkin lymphomas in childhood.

[Vertical "X" sign test (author's transl)].

Fukuda's blindfold vertical writing test was modified in a vertical drawing test which consisted of five vertical series of "X" sings. The lists of signs of 150 healthy persons were compared to the findings of 111 patients. Persons with peripheral or central vestibular lesions had marked deviations in about 70% beyond the standard deviation of normals. The test is recommended for routine explorations. The procedure and the evaluation are easy and can be used for the objective identification of vestibulospinal deviation.

[Early experience with repeated determinations of 131-I-hipuran clearance-equivalent during pregnancy ].

Serial observations of ureteric and renal function during pregnancy were carried out by using isotope clearances; renal scintigraphy was performed for the first examination and isotope nephrography subsequently. The results of both types of examination are tabulated. It is shown that these procedures provide the necessary information on renal and ureteric function, given the indications which we have listed. The information concerns both kidneys or can be confined to a single kidney.

The reliability of Frenzel's glasses and the optimal observation period for routine explorations.

In many clinics and medical offices the electrographical registrations of vestibular reactions are only available for patients with special problems. For routine explorations Frenzel's glasses are still in use. It was one purpose of our studies to show that the observation with Frenzel's glasses is no less effective than ENG registration, in regard to nystagmus frequency. This result is based on investigations with special equipment for simultaneous registration and computerized evaluations. The disadvantage using Frenzel's glasses without any other equipment is the impossibility of determining the culmination phase exactly. Therefore we looked for a defined period in which the frequency was correlated best to the frequency of the culmination phase determined by ENG recordings. A total of more than 100 different intervals were analyzed. The optimal period for routine investigations is a 1 min interval beginning 10 sec after the end of irrigation. Limiting the evaluation of a caloric response to this defined observation period is recommended for various reasons.

[Important nuclear medicine research methods in vesico-uretero-renal reflux].

The at present usual nuclear-medical methods in diagnostics and control of the course of the vesico-uretero-renal reflux are discussed. The semiquantitatively evaluated ING or the side-separated isotope clearance and the function scintigraphy are important parameters for therapeutic decisions in the basic diagnosis and the control of the course of the vesico-uretero-renal reflux.

[Genesis of microcytes in hemodialysis patients with chronic uremia].

The study is concerned with factors that may reduce the life time of erythrocytes of patients under intermittent haemodialysis. The red cell population of these patients consisted of micro-, normo-, macro- and megalocytes. The classification of the cell size was shiftet to greater diameters in comparison with healthy donors. The evaluation of the biological half time of erythrocytes labelled with 51Cr resulted in a decrease of about 16 days. The autoradiographic evaluation of the erythrocyte size-distribution patterns 1 and 24 hours after the injection of labelled red cells into the donors circulation clearly demonstrated a decrease of macro- and megalocytes. Simultaneously, the number of microcytes rose considerably. The measurement of erythrocyte deformability could demonstrate a clearcut relationship of flexibility for a) disturbed erythropoiesis resulting in the production of macro- and megalocytes, and b) fragmentation and premature elimination from the circulation due to unfavourable rheologic properties of erythrocytes of abnormal big and small sizes.

[Lung-scintigraphy in the control of children with malignancies treated by radiochemotherapy (author's transl)].

Cytostatics- and radiation-induced alterations of the parenchyma of the lung were investigated in 30 children with malignomas before, during and after therapy by means of lung perfusion scintigraphy. Before the tumour-therapy (2 children) lung-scintigrams were regular. 16 children (Hodgkin- and non Hodgkin-lymphoma, acute lymphocytic leukemia with mediastinal tumour, intrathoracal neuroblastoma and Ewing-sarcoma) received epidiaphragmatical radiation and cytostatics. All 35 lung-scintigrams of these patients (1-60 months after beginning of therapy) were abnormal. Within 6 months after radiation obstructions to perfusion could be demonstrated in ray-treated parenchyma of the lung only. Subsequent to 6 months after radiation, during cytostatics, disturbances of perfusion were diffusely spreading in the lung parenchyma. 12 children (acute lymphocytic leukemia, Histiocytosis X and osteogenic sarcoma) received cytostatics only. All 18 lung-scintigrams of these patients (1-55 months after beginning of therapy) were pathological. After cessation of therapy (radiochemotherapy or chemotherapy only) scintigraphically improvement of perfusion occurred in the majority of patients. Obviously the diffusely spreading obstructions to perfusion represent alterations during the early phase of their development induced by chemotherapy.

[The value of radio-isotope investigations of the urinary tract for the diagnosis of urinary tract calculi during pregnancy (author's transl)].

According to the literature, urinary tract calculi are a rare complication of pregnancy. Because of the radiation effect of excretory urography on mother and fetus, intravenous pyelography can only rarely be used for the diagnosis of calculi in pregnancy. A sequential radio-isotope nephrogram and the simultaneous determination of the J 131 hippurate clearance equivalents were used in two pregnant patients. The cases are reported. The results are discussed and compared to reports in the literature. The exposure to radiation is about 50 mrem for radio-isotope nephrography compared to from 500 to 4000 mrem for excretory urography. Radio-isotope nephrography is therefore useful for the preliminary diagnosis of urinary tract calculi in pregnancy and the simultaneous clearance determination yields additional data on the function of the urinary tract.

[Pharyngeal abscesses following tonsillectomy (author's transl)].

A few weeks after tonsillectomy two patients presented with recurring pharyngeal abscesses, and a fistula and a cyst were identified as the probable reasons for these. The embryology and differential diagnosis is discussed. The Grisel Syndrome and its presentation following tonsil remnant infection is demonstrated in another patient.
