Pituitary gonadotropin inhibition by a highly active analog of luteinizing hormone-releasing hormone.

Chronic treatment with highly active analogs of luteinizing hormone (LH)-releasing hormone (LH-RH) induces "paradoxical" antifertility effects. Intact male rats, a reduction in testosterone production is observed after the administration of [D-Ser(But)6]-LH-RH(1-9)ethylamide (burserelin, Hoe 766), 50 ng/day subcutaneously for 4 weeks. In castrated male rats treated with the same dose of analog, plasma LH levels were reduced from days 14 to 28 and plasma follicle-stimulating hormone (FSH) levels were reduced from days 21 to 28 of treatment. Pituitary LH and FSH concentrations were also decreased. The plasma prolactin level was reduced at 14 days of treatment. The hypothalamic LH-RH content remained unchanged and the adrenal corticosterone content was lowered. These findings indicate a direct inhibitory effect of the analog on gonadotropin secretion in the absence of the gonads, and may explain some paradoxical antifertility effects observed with high doses of LH-RH analogs which exceed the physiologic dose range.

[Audiogram-analogue representation of the vestibular reaction to caloric stimulation (author's transl)].

A new diagram with audiogram-like presentation of the vestibular reaction to caloric stimulation is proposed, which facilitates the identification of typical vestibular disorders. Spontaneous nystagmus with the relevant parameters is entered in the chart. The diagram can be used for any parameter of the reaction, such as frequency, slow phase velocity or total amplitude in a defined period of time, e.g. 30 sec of maximal reaction.

[Anomalies of the stapes (author's transl)].

Reported are four cases of anomalies of the stapes and the oval window niche. Their diagnostic and operative problems are discussed. The report includes one case of spontaneous cerebrospinal otorrhea. Surgical exploration of the middle ear demonstrated a fluid leak from a fistula in the stapes footplate. Two cases presented combined congenital anomalies of the stapes and facial nerve as a cause of hearing loss. Additionally one case of "columella" shaped stapes was observed. The embryological aspects of the middle ear structures are discussed.

[Functional hearing loss in speech audiometry (author's transl)].

In a group of 238 patients assessed for medico-legal reasons, 26 subjects were found to have functional hearing problems. In detecting the presence of functional hearing loss, the reliability of speech audiometric patterns was shown. Each patient demonstrated at least one suspicious response. The evaluation revealed an irregular configuration of curves ("step-like", 56%) and a better understanding for monosyllabic testwords than for digits (28.8%) as the most important criteria. In the control group of 40 hard-of-hearing patients without functional hearing losses, testing-irregularities were found in only 2.5-3.7% of the patients. Furthermore, discrepancies between speech reception thresholds and pure tone averages (84.6%) as well as poor test-retest reliability in tone audiometry contributed to an identification of functional hearing loss. Békésy-audiometry Type V was obtained in 73% of those patients studied.

[Isotope nephrographic studies during hormonal contraception using Depositon].

By means of isotope nephrograms of a total of 29 renally healthy women who had been taken for more than 1 year the drug Deposiston, the renal function is investigated. Checking is done by using the semiquantitative evaluation parameters according to Zum Winkel, Pabst and Hör and to own criteria. Our results show that the highly dosed application of estrogens does not induce significant changes of renal function in the isotope nephrogram.

["Hypothyroid" goitre in puberty treated with a combined T3/T4 preparation (author's transl)].

After administration of combined T3/T4 preparations (ratio of 4 : 1) to juveniles with goitre, size II-III (WHO), and hypothyroid T4 or PBI levels, the T3 value reached hyperthyroid levels 7-8 hours afterwards. The increased T3 level with normal T4 blood levels can lead to iatrogenic T3 hyperthyroidism. Serial tests demonstrated that at increasing doses a raised T3/T4 ratio, which is observed with low T4 values, can return to normal but with increasing doses this ratio, caused by T3 rise, can again become elevated. For this reason one should reconsider treatment with T3/T4 preparations and, if necessary, replace it by monotherapy with L-thyroxine.

[TRH refractory TSH values, triiodothyronine levels and T3/T4 quotients in hypothyroid children treated with T3/T4 preparations (author's transl)].

TRH-tests, triiodothyronine (T3), TBI, and thyroxine (T4) determinations were carried out in 23 children with primary hypothyroidism treated with T3/T4-preparations. T3/T4 quotients were calculated. 7 out of 10 patients with TRH refractory TSH values and normal T4 levels had T3 values above normal range. In 6 of them pathologically elevated T3/T4 quotients could be observed. A TSH refractory TSH suppression was only found in patients with superior normal T4 values. After a reduction of the replacement doses the T3 values were normal. The findings show--that not only the T4 values should be controlled under treatment with thyroid preparations but also the T3 and TSH serum levels. The replacement therapy of hypothyroidism with T3/T4 preparations in proportion 1:4 should be revised.

["Prophylactic" intrathecal radiogold (198au) in leukaemia in children (author's transl)].

198Au radiogold-colloid (Behring) was injected as "prophylactic" therapy against central nervous system leukaemia in 26 children with acute leukaemia (22 acute lymphatic and 4 myeloid). During the period of observation (1.3. 1972 to 1.9.1975), 23 children remained free of central nervous system recurrences: 16 children are alive 3 to 39 1/2 months later, seven died after 4 1/2 to 29 1/2 months from the underlying disease. In one child CNS leukaemia developed after about five months, in another one during a second haematological recurrence, while the third one developed retrobulbar infiltration in the optic nerve. In general, 2 mCi 198Au gold-colloid was injected intrathecally via a lumbar puncture. The method was well tolerated.

[Sequential scintigraphy in the investigation of changes in renal function during pregnancy (author's transl)].

Changes in renal function during pregnancy were studied by sequential scintigraphy in 16 women with normal kidneys. The examinations were carried out during the first and third trimesters. Time activity curves were obtained by a computer-assisted storage and analysis procedure, using a "region of interest" technique for renal parenchyma and renal pelvis. The study included topographic changes and the evaluation of 13 semi-quantitative parameters related to the renal parenchyma and of nine parameters related to the renal pelvis. The results were demonstrated graphically. The predominantly rightsided changes in renal function during the third trimester are discussed in relation to the relevant literature.

[Performance of uteroplacental perfusion in contrast to hormone-analytic investigations in intrauterine retardation and under the influence of intravenous long-term application of heparin (author's transl)].

The digital estimation of utero-placental circulation by means of nuclear-medical examinations with help of 113 m In-Cl and regarding to the "region of interest" gives no depositions in cases with an intrauterine retardation. This concerns the activity as well in the placenta as in the uterus too. Exceptions represents intrauterine retardations on the base of heaviness EPH-gestosis and placenta previa. Herewith the rise of activity corresponds with the thickness in the III. trimester of pregnancy. A treatment about several weeks with Hepain under bed conditions leads to no proofed influence of the uteroplacental circulation, on the contrary to a rise of estriol and HPL-values, meaning a improvement of placental performance. The estimation of the utero-placental circulation by means of nuclear-medical examinations is situated to the problem of fetal condition by EPH-gestosis, placenta previa and other.
