[In situ - demonstration of helper-T-lymphocytes in cutaneous malignant lymphomas by diaminopeptidase IV (DAP IV)].

Demonstration of helper-T-lymphocytes in tissue sections of cutaneous lymphomas by diaminopeptidase IV (DAP IV) gives new informations concerning the composition of these malignant infiltrates. It can be demonstrated that in plaque stage of mycosis fungoides the majority of T-lymphocytes are helper cells, thus giving evidence that mycosis fungoides represents a helper-T-cell-lymphoma. In tumor stage DAP IV activity changes, and the number of DAP IV-positive cells decreases remarkably. In malignant B-cell-lymphomas and high grade malignant lymphomas a small percentage of the infiltrate is composed of helper-T-lymphocytes; their existence was not known before. This method permits easy and reproducable identification of helper-T-lymphocytes in dermal infiltrates.

In vitro differentiation of rat peritoneal macrophages into mast cells: an enzymecytochemical study.

Under special culture conditions, rat peritoneal mononuclear phagocytes have previously been shown to transform into typical mast cells, based on their histamine contents, the increase of their membrane IgE-receptors, and the development of metachromatic, electron-dense cytoplasmic granules. In the present investigation, the enzyme-cytochemical characteristics of these cells were examined prior to and during culture. The peritoneal macrophages were found to have strong paranuclear activity of nonspecific esterases and acid hydrolases, while they were negative for peroxidase and chloroacetate esterase. During culture, two main changes in enzyme activity were observed within all the cells: (1) Paranuclear activity was lost in favour of granular enzyme activity throughout the cytoplasm, and (2) chloroacetate esterase appeared in cultured cells and developed a strong granular activity. These findings are consistent with the macrophage nature of the seeded cells and their development into mast cells during culture. They give support to the theory that mast cells originate from the monocyte-macrophage lineage.

Evidence that tissue mast cells derive from mononuclear phagocytes.

A new method has been developed that allows in vitro differentiation of rat mast cells from precursor cells in the absence of feeder layers. In the present investigation, the precursor cells are further characterized as to their nature, state of activation and distribution in different organs of the rat. The data support our previously published evidence, based on morphology and enzyme-cytochemical reaction patterns that the precursor cells are mononuclear phagocytes. Peritoneal macrophages elicited by mineral oil, thioglycollate or proteose-peptone have lost the ability to differentiate into mast cells. Single cell suspensions of spleen and bone marrow contain mast cell precursors, while cells from mesenteric lymph nodes and thymus or purified lymphocytes never yield mast cells in this culture system.

[Multiple cutaneous malignancies and hairy cell leukemia (author's transl)].

In a 78 years old male a hairy cell leukemia was found in addition to a SSM (superficial spreading melanoma). 9 and 13 years before a squamous cell epithelioma and a basal cell epithelioma had developed. Cutaneous malignancies and leukemia develop together more often than it is to be expected statistically.

[Selective ultraviolet phototherapy (SUP) for prophylaxis of polymorphous light eruption (author's transl)].

16 patients with polymorphous light eruption were prophylactically treated with selective ultraviolet phototherapy (SUP). Ten patients showed no or only minimal signs of their disease in a following holiday with high sun exposition; 4 patients had the same skin manifestations as in the years before. Indirect evaluation of the therapeutic success had to be performed in two patients, who did not have holidays in this year. Thus SUP may represent a new prophylactic treatment of polymorphous light eruption.

[Alternations of dermal lymphocytes in mycosis fungoides, a study with the scanning electron microscope (author's transl)].

Isolated dermal lymphocytes from the infiltrate of mycosis fungoides show on scanning electron microscopic examination following characteristics, which differ from normal lymphocyte surface: loss of the round cell shape, increase of absolute length of microvilli, increase of lymphocytes with smooth surface and mitoses. These alterations concern all immunologic subpopulations and may indicate a stimulation of the whole dermal infiltrate in mycosis fungoides.

Proliferation kinetics of the dermal infiltrate in cutaneous malignant lymphomas.

To obtain information about the role of local proliferation in the pathogenesis of dermal infiltrate in malignant cutaneous lymphomas, we determined the percentage of 3H-thymidine-labeled infiltrating cells (3H-index). A linear correlation was found between proliferative activity and clinical stage in mycosis fungoides, i.e., the 3H-index is moderately elevated in stage I and high in stage III. The 3H-index is within normal range in dermal infiltrate of Sézary syndrome, diffuse lymphocytic lymphoma, as well as in lymphocytoma benign cutis. In parapsoriasis en plaques two groups can be distinguished: in the small-plaque variant (chronic superficial dermatitis) the 3H-index is low, whereas the large-plaque variant (prereticulotic poikiloderma) shows strong proliferative activity. Thus, determination of proliferative activity seems to give new insights into the pathogenesis of dermal infiltrate in cutaneous lymphomas.

[Eruptive keratoacanthoma].

A 74 year old woman showed typical symptoms of eruptive keratoacanthomas since three years: exanthematic sudden appearance of numberless papules, few millimeters in diameter with central keratinization, masked facies, ectropion of the eyelids and pruritus. Histology revealed the characteristic architecture of keratoacanthoma. Proliferation kinetics were characterized by increase of cell division within the basal cell layer, prolongation of time for DNA-synthesis (15,7 h) and moderate decrease of generation time. The reactive inflammatory dermal infiltrate mainly consists of lymphocytes. Ultrastructural features comprise occurrence of intracellular desmosomes and elastic and collagen fibers between the tumor cells. Therapeutically a good response was observed to aromatic retinoid Ro 10-9359.

[Colchicine sensitivity index: a means of distinguishing benign and malignant lymphocytic cutaneous infiltrates (author's transl)].

Lymphocytes in patients with chronic lymphatic leukaemia and other malignant lymphomas have an abnormally high sensitivity to colchicine in vitro. This can be expressed in a colchicine-sensitivity index and used diagnostically. Lymphocytes isolated from cutaneous infiltrates in mycosis fungoides, a cutaneous malignant T-cell lymphoma, had an increased colchicine-sensitivity index of 44 (P < 0.01). On the other hand, lymphocytes from cutaneous infiltrates of reactive inflammatory dermatitis cases had an index of 23. It would seem that the colchicine-sensitivity index is helpful in the diagnosis of selected cases of cutaneous malignant lymphomas.

[Kaposi's sarcoma (author's transl)].

Kaposi's sarcoma of the skin and an aplastic syndrome occurred together in a 51-year-old patient. Macroscopically livid papules and nodules were observed. Histomorphologically endotheliomatous cell proliferation with signs of infiltrative growth was found. Because of the aplastic pancytopenia cytostatic treatment of the Kaposi sarcoma was contraindicated. The patient finally died of vascular failure with haemorrhagic diathesis being manifest. Syntropy of Kaposi's sarcoma with malignant haematological diseases is known. However, association with aplastic anaemia has not been observed so far. The pathogenesis of Kaposi's sarcoma is unknown.

[Cystic acne and clinocomptodactyly in Klinefelter's syndrome (46XY/47 XXY mosaic)].

Klinefelter's syndrome was found in a 18 year old patient, suffering for four years from nodulocystic acne. Serum gonadotropins were within normal range, while histology of the testes revealed beginning tubular hyalinization. The case report shows that young patients with Klinefelter's syndrome may present symptoms that seem to exclude this syndrome. Furthermore, our patient exhibited klinokamptodaktylia of hands and feet, a congenital anomaly occurring very rarely in Klinefelter's syndrome.
