The clinician and forensic medicine.

Although it is an intrinsic part of all medical practice forensic medicine often is either unrecognized as such or is consciously or subconsciously evaded. The failure to apply some rather basic and simple forensic principles that only the physician is capable of doing may result in problems to the patient ranging from frustration to near catastrophe. For physicians who are reasonably well equipped to understand the legal system, the successful conclusion of a legal case, including, sometimes, an appearance in court, can be stimulating and interesting.

Acid rain: what we know, what we did, what we will do.

This paper reviews the involvement of the Canadian province of Ontario in the acid rain issue. Ontario is a major producer of acid gas emissions and suffers significant environmental consequences because of acid rain. The province's substantial contribution to the scientific understanding of acid rain is summarized with emphasis on the extent and origins of the deposition it receives, the impact on the aquatic environment, and the impact on the terrestrial environment. This paper discusses the history of the government's success at reducing acid gas emissions through the 1970's when legislators set out to enhance local or "ambient" air quality, the first legislation to require SO2 reductions from companies already in compliance with ambient air quality legislation, and the current Countdown Acid Rain program which reduces SO2 emission limits by 67%. The process used to establish the tough new limits while reasonably anticipating the development of SO2 control technologies is detailed along with the specific requirements of the major emitters controlled by the regulations. Projections confirm that while the Countdown reductions will bring about significant reductions in deposition, adequate environmental protection in Canada cannot be achieved without some comparable U.S. acid gas abatement effort.

Methodology for the determination of dinitroaniline herbicides in tissue and excreta.

A method is presented for the analysis of trace amounts of dinitroaniline herbicides in tissue and excreta. The method employs extraction of the tissue or excreta with organic solvent, clean up by liquid/liquid partitioning or silica gel chromatography, and ultimate analysis by gas chromatography using electron capture detection. Recoveries of greater than 80% were noted over the range of concentration 0.1 to 1.0 microgram/g.

Some actions of chandonium iodide, a new short-acting muscle relaxant, in anaesthetized cats and on isolated muscle preparations.

1. The actions of the new skeletal muscle relaxant chandonium (HS310) on the cardiovascular, respiratory and skeletal muscle systems of the cat under chloralose anaesthesia and on chick and rat isolated skeletal muscle preparations have been described. 2. In the cat chandonium exhibited a potent, competitive, non-depolarizing neuromuscular blocking action that was rapid in onset and of short duration. It possessed a selective atropine-like action at the cardiac vagus neuro-effector junction but little or no ganglion blocking activity. Neither adrenergic neurone noralpha-adrenoceptor blocking properties were evident. 3. Unlike tubocurarine, chandonium was without effect on the airways system of the anaesthetized cat. 4. Chandonium possessed weak anticholinesterase action. It is conceivable that this effect may contribute to its short duration of action. 5. The results from these studies suggest that chandonium may have possible clinical applications as a short-acting muscle relaxant.

Unilateral superior rectus recession for the treatment of dissociated vertical deviation.

The management of dissociated vertical deviation (DVD) is an unsettled issue. Several authors have advocated bilateral superior rectus recession for all patients with DVD unless a dense amblyopia is present in the nonfixating eye. Our experience suggested that DVD was being successfully treated with unilateral bilateral superior rectus recession. We reviewed the charts of 57 patients who underwent graded unilateral superior rectus recession and 10 patients who had bilateral superior rectus recession for the treatment of DVD. DVD, a manifest deviation, was considered as distinct from occlusion hyperphoria, a latent deviation present only under cover. Of the patients who underwent unilateral superior rectus recession, only six developed a significant (14 prism diopters or more) DVD in the unoperated eye. The presence of any DVD in the unoperated eye appeared to be predictive of an unacceptable postoperative deviation (15 times greater chance, P less than .001). Patients with an occlusion hyperphoria in the unoperated eye showed no increased propensity to develop a significant DVD following surgery. The results of bilateral surgery were disappointing. Eight of ten patients had residual DVD in one or both eyes of 10 delta or greater. Our results suggest that unilateral surgery is an effective treatment of DVD in patients with a manifest deviation in only one eye. Although neither unilateral nor bilateral surgery was entirely satisfactory for the treatment of bilateral DVD, we recommend bilateral superior rectus recession for these patients.

Saphenous vein graft aneurysms demonstrated by computed tomography.

A case of aneurysms of saphenous vein coronary artery bypass (CAB) grafts is presented with CT and echocardiography findings. Aneurysms of the bypass graft are rare complications of CAB surgery but many occur due to atherosclerosis, infection, surgical technique, or vasculitis.
