[Glucocorticosteroid therapy in bronchial asthma with special reference to the prevention of life-threatening bronchial obstruction in high-risk asthma].

The treatment of bronchial asthma with glucocorticosteroids requires consideration of unequivocal principles which include strong indication, considering the individual risk-benefit-relation. Glucocorticosteroids should be fitted in a medicamentous-therapeutic hierarchy. Inhalable glucocorticosteroids hardly cause systemic side effects, so they are very suitable for the continuous treatment, if the need in glucocorticosteroids is low. The indication for systemic application of glucocorticosteroid, their side effects, relative contraindications and pharmacological interactions are given. Careful analyses of cases of death, caused by bronchial asthma in accordance show that an inadequate glucocorticosteroid-therapy was the most important avoidable factor. The attention of the clinicians must be directed on a small group of patients characterised by high risk of death ("high-risk-asthma"). These patients, having certain clinical signs, need not only special intensive care, but also experienced, early and sufficient treatment with glucocorticosteroids to prevent life threatening situations.

[Determination of macrophage activity in bronchoalveolar lavage fluid in patients with lung diseases].

In order to investigate the metabolic activity of alveolar macrophages in the bronchoalveolar lavage fluid from patients with extrinsic allergic alveolitis, pulmonary sarcoidosis, bronchogenic carcinoma, chronic bronchitis, tuberculosis, bronchial asthma and from sensitized asymptomatic persons, the quantitative and qualitative Nitroblue-tetrazolium test was used. There was observed a significant increase of the alveolar macrophage activity in patients with bronchogenic carcinoma, chronic bronchitis, sarcoidosis and extrinsic allergic alveolitis. The results of the investigation emphasize the central role of the alveolar macrophages in the triggering and regulation of the inflammatory reaction in the lung and consequently play an important role in the elucidation of pathogenic mechanisms in lung diseases.

[Monitoring of medication in optimalization of preoral theophylline in long-term care].

A narrow therapeutic index, considerable interpatients variability of clearance and biotransformation, rapid alterations of pharmacokinetics point out theophylline to a problem-drug. An effective long-term treatment with this drug is only possible with retard-preparations. For a certain therapy a drug monitoring-controlling the theophylline-concentration in the serum--is necessary, in what extend and under which conditions is discussed in detail.

Affinity of human carcinoma cell nuclei for polyinosinic acid demonstrated by monoclonal antibodies.

The anti-nuclear cross reactivity of the monoclonal anti-actin antibodies M 372/809 was studied in some detail. The reactivity against a number of nuclear constituents was examined in the ELISA test and the capacities of these constituents to block the M 372/809 anti-nuclear and anti-actin reactions were evaluated in indirect immunofluorescence tests against tissue sections and monolayer cultures of fibroblasts and Vero cells. The repetitive polynucleotides polyinosinic and polyguanylic acid and their deoxyanalogues, actin and vimentin, were found to have the antigenic epitope. The epitope was covered or otherwise inactivated in the presence of polycytidylic acid. Using the M 372/809 antibodies as a reagent, carcinoma cell nuclei were found commonly to have an affinity for polyinosinic and polyguanylic acid. This was seldom noted with non-neoplastic cells.

Growth-activity related variation of a nuclear antigen in monolayer-culture cells quantitated by immunofluorometry and microspectrophotometry.

Antibodies of the hybridoma clone M 372/809 react with a nuclear antigen in growing cells. Using immunofluorometry and microspectrophotometry, the antigen was found to accumulate to a maximum coincident with the marked early increase in nuclear protein contents. The following DNA increase appeared when the intensity of the reaction with M 372/809 antibodies was decreasing. The antigen increase was inhibited by alpha-amanitin. It may therefore be associated with transcription early in the cell cycle.

[Therapeutic results in lung diseases caused by atypical mycobacteria].

Between 1972 and 1981, 38 patients suffering from lung disease by atypical mycobacteria were admitted to our institute. 36 out of them were males and 2 females with an average age of 55 years. In 10 patients silicosis was present. The patients received chemotherapy at special combinations using Rifampicin, Ethambutol and one or two additional drugs. 2 patients could be healed by lung lobe resection. In 12 patients x-ray regression could be observed. In the remaining patients, stabilization of the lung lesions and sputum conversion--at least temporarily--could be obtained. Because of multiple drug resistance, the therapy of mycobacteriosis is complicated, and permanent conversion of sputum cannot always be obtained, especially in diseases caused by M. intracellulare-avium. Bacteriological relapses are to be expected in 30 to 40 percent.

Dermatitis herpetiformis: relation between circulating antibodies against reticulin and gluten, small-intestinal mucosal status and absorptive capacity.

The status of the jejunal mucosa and of the intestinal absorptive capacity were investigated and related to the occurrence of antibodies against reticulin and gluten in 55 patients with dermatitis herpetiformis (DH), 28 on a normal, 11 on a gluten-reduced and 16 on a gluten-free diet. The mucosal status was characterized on the basis of histopathological findings and the numbers of intra-epithelial lymphocytes. Absorption was evaluated by 5-h urine and 1-h serum D-xylose tests. There was a positive correlation between the degree of pathological mucosal changes, malabsorption and the occurrence of circulating antibodies against reticulin and gluten. The serum xylose test was more sensitive than the urine xylose test for screening of the relatively mild enteropathy of DH and identified 88% of the patients with an abnormal mucosal status. The serological test (antibodies to reticulin and gluten) identified 80% of such patients. Among patients on a gluten-free diet there was some discrepancy between the serum xylose and the serological test, in that 5 of the 16 patients on this diet had an abnormal serum xylose test result, but no antibodies. In DH patients on a normal diet, the presence of antibodies to reticulin and gluten provided the same information about the presence of mucosal lesions as the serum xylose test. In the whole material a combination of the serum xylose test and the serological test identified 24 of 25 patients with an abnormal mucosal status.
