[Sideeffects of colloids (author's transl)].

A prospective controlled trial during 1975 and 1976 has revealed that none of the colloids in clinical use (plasma protein solution, gelatine, hydroxyethylstarch and dextran) is free from the risk of anaphylactoid reactions. Even though the incidence of anaphylactoid reactions is low (0.03%), lethal outcome might be encountered. Between the colloids differences exist as far as manifestation (skin, circulatory and respiratory system) and degree of severity (I--IV) of anaphylactoid reactions are concerned. Since the underlying pathomechanisms have not been elucidated yet, true prophylactic measures are unknown. Therefore, it is mandatory to control the patient very carefully at the beginning of infusion; early symptoms of anaphylactoid reactions should trigger immediate therapeutic measures. Recent findings showed that the anaphylactoid reactions to dextran might be elicited by immune complexes. A new model was developed to investigate the chances of specific prophylactic procedures.

[The immediate reconstruction of a large lip-chin defect with two muscular pedicled and innervated island flaps (author's transl)].

Different methods of defect closure in the lipchin region through lateral advancement-flaps have been described by Bernard, Fries, Hertig, Meyer and others. Is there not enough material available, can neck-, breast-or forehead flaps cover the defect, although they do not fulfill the demands for a satisfactory restoration of specific function. In the following work is the possibility of covering of a three layered facial defect with a combined skin muscle flap taken into consideration in view of the disadvantages of a simple or doubled skin flap. By this operation method the procedure consists of preparing various sized cervical island flaps which are connected with on the mastoid pedicled M. sternocleidomastoideus. We are describing a case of extensive tumorresection in the lip-chin area with neurofibromatosis Recklinghausen, whereby we have closed a large defect with two separately laid on muscle-pedicled, innervated island flaps from the fossa supraclavicularis. These bilateral compound flaps fulfill the demands of an immediate restoration of contour and prominence in this area. Apart from this they allow satisfactory voluntary activity, unpleasant dribbling of saliva is prevented.

[The implantation of lyophilized and gamma-sterilized allogenetic incus and of lyophilized and gamma-sterilized xenogenetic malleus (calf) in rabbits and humans (author's transl)].

The histological reactions to the implantation of allogenetic and xenogenetic lyophilized and gamma-sterilized material into the middle ear of rabbits and men is described. In contrary to the behaviour of allogenetic material the xenogenetic shows a clear minor tendency of revitalisation. The reason is the antigen-antibody-reaction which takes place at the capillary network of the donor and leads to a hylinosis with following obliteration of the vascular rete. Therefore the angiogenetic osteogenesis cannot develop. The clinical usuability of such material is discussed.

[Experimental research to carry out a contralateral anastomosis of the arteria carotis (author's transl)].

The contribution is presenting a procedure for reconstruction of cerebral blood flow after resection of the A. carotis communis and the A. carotis externa in animal experiments. The preparation in human corps will demonstrate the possibilities on anatomical points. The surgical intervention is based on the fact that we can tie up the A. carotis externa on both sides without disadvantage. On the healthy side, without formation of metastasis, we prepare the A. carotis externa from the bifurcation up to the region of the Mm. pterygoidei. There we will tie up the vessel. The vessel can be transplanted to the contralateral side via a retropharyngeal "tunnel". On this side we dissect the big vessels of the neck. There the suture will be applied to the transplanted A. carotis externa and to the stump of the A. carotis interna by end-to-end or end-to-side anastomosis. The results are documentated by angiographies and local pressure measurements during the operation.

Immunological properties of a high molecular weight component from yeast cell autolysate in dogs and evaluation of its potential role in human dextran reactions.

A high molecular weight component (HMC) of autolysate from Saccharomyces cerevisiae yeast cells was prepared. HMC was found to be immunogenic in dogs, inducing hemagglutinating antibody formation. Upon HMC challenge of immunized dogs, systemic anaphylactoid reactions were observed in 4/5 animals. The most prominent symptom was decreased cardiac output. Decrease in mean arterial pressure and increase in pulmonary arterial pressure were also observed. Consumption of total serum complement activity amounted to 22% of initial values. HMC also exhibited mitogenic activity in lymphocyte cultures from nonimmunized and immunized dogs. Since yeast autolysate is used as nitrogen source for Leuconostoc mesenteroides in the production of clinical B 512 dextran it is a theoretically possible trace contaminant of such solutions. Therefore, dogs hyperimmunized with HMC were also challenged with clinical dextran. No anaphylactoid signs were observed. These data suggest a negligible causal role of macromolecular contaminants derived from yeast cell autolysate in rare human anaphylactoid reactions following infusion of clinical dextran.

[Viscosity of blood and sensorineural hearing loss (author's transl)].

A statistical survey about the results of treatment of Pentoxifyllin (Trental) in acute and long-standing sensorineural hearing losses is given. The pharmacological effect of viscosity lowering in such patients is demonstrated. The role of erythrocytes and their behaviour in relation to the viscosity of blood its showed by means of literature.

[The process of manufacturing splints for the nose after nasal tip surgery (author's transl)].

The process of manufacturing splints for the nose after nasal tip surgery falls into three catagories: impressions, moulding and insertion. The plastic ware is carried out in a dental laboratory, so as to help the surgeon who is less familiar with the preparation of the material. The special procedure of moulding is suitable for this purpose. The splints are favourable in regard of cosmetics and tissue compatibility.

[Representative study of morbidity in the Berlin population].

Representation of the frequency of crowding in a sample of 657 Berlin residents aged between 18 and 30 years. Sex and age distribution of the examined individuals with regard to the severity and location of crowding. Relationship of the crowding with the symptoms of Class III malocclusion, perversion in the position of the canine and cross-bite.

[The histological reaction of cialit preserved hyaline and elastic cartilage implanted to replace partially bony wall of the guinea-pig bulla (author's transl)].

Cialit preserved cartilage was used to fill defects of the bulla of porpoises and the histological healing was studied. Hyaline and elastic cartilage are showing an integration without reactions. The cartilage is resorbed consecutively by the recipient and substituted by bone. Whereas the hyaline cartilage shows a complete bony substitute and a small enlargement we can registrate that the elastic cartilage often shows a resorptive tendency with replacement of connective tissue and loss of the shape. The histological results are corresponding to the clinical results when using preserved cartilage in middle ear surgery for restoring the posterior wall.

[Determination of mercury-content of tissues preserved in cialit (author's transl)].

The mercury-tissue-concentrations of implants (structures of the middle ear, bones, cartilage, dura) after preservation in solutions of cialit are mostly independent on preservation conditions, low and seem to be physiological. A positive correlation exists between the weight of the implant and the mercury-tissue concentrations. An enrichment with mercury-ions cannot be stated. The statements do not concern the mercury which is bound on cialit in the implant.

[Fertility after treatment with HCG for undescended testes (author's tranls)].

151 out of 212 adolescents having had been treated by human chorionic gonadotrophin (HCG) before puberty because of undescended testes, were re-examined 7 to 12 years later at an individual age of at least 17 years. In 121 of them a semen specimen for sperm counts could be obtained. In 10 patients, however, no ejaculate could be collected because of their hypogonadism. 78 of these 121 patients examined by sperm count had a sufficient testicular descensus after HCG treatment. In 43 adolescents HCG therapy failed, therefore subsequent orchidopexy was performed. A normal sperm count was seen in only 39 per cent of the 121 patients. After unilateral testicular maldescensus 18%, after bilateral maldescensus 43% of the patients had to be evaluated as prospectively infertile. Maier and Spann reported sperm counts below 10(6)/ml in 9 out of 17 patients with bilateral maldescensus who had been treated by surgery alone. In the present study, however, only 9 out of 35 individuals treated for bilateral maldescensus by HCG alone showed sperm counts below 10(6)/ml. Plasma LH and FSH concentrations were determined in 93 patients, 6 out of which showed hypergonadotrophic values of FSH and sperm counts below 10(7)/ml. Subsequently, 3 cases of Klinefelter's syndrome could be diagnosed among the total group of 121 patients. The etiology of testicular maldescensus is by no means uniform since anatomical, endocrine and/or chromosomal factors are involved.

[Immature-cell leukoses in the adult age].

In form of a survey the author enters into selected questions of the etiopathogenesis, the pre-leucaemic phase, the epidemiology, the cytochemical subdivision, the clinic and therapy of undifferentiated cell leucaemia in adult age. The number of two-year sojourns in a hospital and other data are reported on the basis of an evaluation of the signature ledges of the clinical histories in the district Dresden. A study concerning the therapy carried out on 331 patients from six clinics of the GDR emphasizes the value of the polychemotherapy, in which cases according to the COAP- and ViDaP-scheme the results of the treatment are somewhat more unfavourable, compared with the results in the newest combinations of other authors.
