[Personal experience of urate lithiasis (author's transl)].

In Alsace, urate lithiasis accounts for 32.7% of cases of lithiasis requiring hospitalisation. Fifty nine per cent of patients are hyperuricaemic, and only 10% have typical gout. 14.6% are hyperuricosuric but all have isoaciduria. A male disorder in 3/4 cases, it occurs above all after the age of 50. The relatively late onset, combined with marked obesity (mean weight 81.5 kg) is considered by the authors to be an argument proving the role of dietary excesses in the aetiology of this lithiasis. Excessive protein would seem to be the essential feature.

[Efficacy and tolerance of trimethoprim/sulfamethoxazole administered intramuscularly in patients with urinary tract infections (author's transl)].

The antimicrobial efficacy, general and local tolerance of a new intramuscular application form of trimethoprim/sulfamethoxazole was evaluated in a French-German clinical study. Fifty-five urological patients with urinary tract infections were treated with two i. m. injections (3 ml) daily over three to five days followed by oral medication. The study revealed excellent antimicrobial efficacy of the drug. In general TMP/SMZ was tolerated well after i. m. administration. Elevation of creatinine phosphokinase was observed in only three patients treated i. m. for five days. The high viscosity of the solution renders i. m. injection technically difficult, leading to a high proportion of local complications. The indications for use of i. m. administered TMP/SMZ in the hospital are limited, however it should be suitable for private practitioners in that an effective antimicrobial treatment can be started which can be continued by oral or intravenous medication.

[Renal pneumatisation simulating suppuration : possible consequence of renal artery embolisation. Two cases (author's transl)].

The authors report two cases of renal pneumatisation immediately following renal artery embolisation performed for carcinoma of the kidney. The particularly painful and inflammatory (high fever, leucocytosis, increased sedimentation rate) led to abdominal X-rays which revealed signs of gas in the kidney. In the first case this consisted of rounded clear areas distributed in a linear fashion and indicative of intravascular pneumatisation. In case n degrees 2, intravascular pneumatisation was accompanied by gas in the calyces. In the absence of suppuration and of proven infectious cause, the authors failed to find any explanation for the presence of gas bubbles in the vessels and calyces of the embolised kidney.

Choline excites cortical neurons.

In cats under halothane or methoxyflurane, iontophoretic applications of choline are only eight times weaker than applications of acetylcholine in evoking firing of neurons in the sensorimotor region of the cerebral cortex. The action of choline is suppressed by atropine but not by two agents that block choline uptake (hemicholinium-3 and triethylcholine), and is not potentiated by an anticholinesterase (physostigmine). Choline therefore appears to excite cortical neurons by a direct action, which may be a significant component of its beneficial therapeutic effects.

[Leydig cell tumours of the testis. Two cases presenting opposing sexual features : gynaecomastia, precocious virilisation, and revealed by study in vitro of aromatisation activity].

Two Leydig cell tumours presented with changes in sexual characteristics. The first patient was a child with precocious puberty whilst the second was that of an adult with recurrent gynaecomastia. Histochemical examination in vitro of tumour tissue from the second patient made it possible to explain the synthesis of oestrogens by a primary tissue destined to an essentially androgenic activity.

Ultrastructural changes of the intestinal wall after transileal cutaneous ureterostomy in men. A preliminary study. (With 1 color plate).

Pre- and postoperative specimens of ileal loops used in transileal ureteroplasty have been taken and ultrastructural modifications concerning the intestinal mucosa with special attention to the epithelial cells are described. The enterocytes undergo significant modifications because of the operation (i.e. collapse of striated border and entrancement of lysosomal inclusions). Contact with the urine brings about osmotic changes of the intestinal epithelium as well as irritation of the capillar endothelium. But most structural modifications observed histologically or by electron microscopy seem to be reversible. 6 months after the operation the histological structure of the ileum in contact with urine again becomes morphologically normal.

[A case of carcinoma of the stomach revealed by a testicular metastasis (author's transl)].

A testicular metastasis from a gastric carcinoma is exceptional among the rare secondary localisations of a visceral neoplasm in the testis. It should however be remembered for the testicular tumour sometimes precedes the clinical manifestations of the primary neoplasm. The diagnosis of the testicular metastasis from a gastrict carcinoma can only be made by histological examination and required the use of slides stained with mucicarmine. The method of spread to the testis seems mainly due to inversion of the lymph flow by neoplastic obstruction of the latero-aortic lymph nodes. In the cases reported in the literature, the presence of a testicular metastasis from a carcinoma of the stomach is a factor suggesting a poor prognosis.
