Methods of strategic planning for blood services.

Among the operations research methods applied to strategic planning problems mixed integer programming has proven to be a very useful framework for a mathematical formulation of the important strategic relations in blood services. A case study shows how the method works with mathematical details omitted. The input data and the results are illustrated and discussed.

Changes in nucleolar structure, number and size in cellular activation and inactivation. Observations in human phytohaemagglutinin-treated lymphocytes.

Lymphocytes in human peripheral blood are small, relatively inactive cells. The addition of phytohaemagglutinin (PHA) to cultures of these cells causes a marked increase in cellular and nuclear size, DNA-synthesis and metabolic activity, which reaches a maximum three days after the onset of culturing. The cells then undergo an inactivation process over a period of approximately ten days by which time they have reverted to cells resembling small, inactive lymphocytes. Within the first three days, nucleoli increase in size and number, changing from ring-shaped to nucleolonema-exhibiting to compact nucleoli. In the course of the inactivation process the nucleoli decrease in size and change from compact nucleoli directly into ring-shaped nucleoli. Thus activation and inactivation pathways are different. There is an increase in the number of nucleoli during the inactivation phase up to the seventh day in culture, followed by a slight decrease until day 14. This suggests that nucleoli in metabolically active cells have a tendency to fuse, whereas those in inactive cells tend to fragment.

[Novel modification of a 2-row intestinal anastomosis technic].

A novel modification of a two-row intestinal anastomosis technique is introduced. A step-like anastomosis was carried out by a short stepwise resection of the several intestinal wall laminae with intramural adaptation on the specific juncture. In 44 end-to-end intestinal anastomoses in dogs and 5 in children there were not observed any insufficiency and stenosis.

Excretory urography as a screening method for abnormalities of the upper urinary tract in asymptomatic boys with undescended testicles.

Congenital dystopic testicles frequently occur jointly with abnormalities of the kidneys and the system of draining urinary ducts. A series of 100 asymptomatic boys has been studied by intravenous excretory urography after having undergone orchiopexy for retained testes. All children were clinically non-suspect and there was no history of urinary tract malformation. The investigations brought to light 7 major and 9 minor abnormalities. However, the authors do not recommend the use of intravenous urography as a routine procedure in all children with undescended testicles. The indications of the diagnostic procedure must be limited.

[The diagnosis and treatment of mixed anaerobic vaginal dischargs (authors' transl)].

In about 5% of 3355 patients the microscopic examination of the vaginal discharge in normal saline showed a characteristic fast moving rod. More than 80% of these patients complained of a discharge with a foul smell during the menstrual period. Bactriology showed an anaerob gram negative rod which was difficult to grow and is as yet unclassified. Following treatment with one gram of Mimorazol every 12 hours for 3 doses, 71,5% of the patients were cured and showed a normal vaginal flora without further treatment. In around 24,13% of the patients the gram negative rod disappeared but a persistent mixed flora required further treatment. In 4.3% of the patients a recurrence occurred. An attempt was made to prevent recurrent infection prior to the follow-up examination by prohibition of sexual intercourse with an untreated partner, bathing in public swimming pools, and the use of sponges or wash cloths. In the course of the investigation the suspicion of contamination from the water of public pools became apparent. The diagnosis, treatment and transmission and the pathogenecity of the fast moving negative rod is discussed.

[Aseptic necrosis of the femur head in congenital hip dysplasia. Radiological signs for a prediction or an early diagnosis (author's transl)].

Radiographs an patient histories of 36 children were examined in order to determine radiological signs that would allow a prediction or an early diagnosis of the aseptic necrosis of the femur head nucleus (Luxations-Perthes) during the treatment of hip dysplasia. Necrosis of the nucleus appeared in 17 joints (of 15 children). We had the following findings: a) Mikroepiphyses have a high risk (59%). b) In all cases of necrosis the angles of the femur neck near epiphysis were rounded, while this sign was never found in dysplastic joints without necrosis.
