[Retardation problems within the scope of prematurity].

Among 804 short-weight infants (birth weight less than 2500 g) born at the Department of Obstetrics and Gynaecology, Medical School, Friedrich-Schiller University, Jena, from 1. 1. 1978 to 31. 12. 1981, one-third were small for gestational age babies. These hypotrophic infants showed, in relation to premature infants (appropriate for gestational age) a higher prenatal and intranatal mortality (statistically not significant). A frequent severe foetal distress mainly on the background of chronic or subacute placental insufficiency implied a higher rate of obstetrical operations by the hypotrophic infants. This was particularly evident by the incidence of caesarean sections (24.7% : 15.2% respectively). The premature hypotrophic infants had the highest caesarean section incidence, amounting to 30.3%. Postnatal adaptation proved more favourable with the small for gestational age babies than with the premature group. There was in fact a statistically significantly lower rate of asphyxiated infants detectable by means of APGAR-score less than or equal to 7 five minutes after labour and a lower neonatal and late mortality in spite of "acidotic morbidity", which was statistically significantly higher in the group of small for date infants (p less than 0.05). We consider as possible cause a higher average duration of gestation with approximately average birth weights. The difference in the clinical behaviour between the premature-hypotrophic and hypotrophic infants were smaller than between the premature-eutrophic and hypotrophic infants. This contradictory behaviour before, during and after labour requires the specialist to be capable of meeting the diagnostic and therapeutical requirements in every respect.

Catabolism of capped (3'-5')- and (2'-5')-adenylates in rat liver nuclei.

We describe studies concerning the ability of a nuclear dinucleoside triphosphatase to act as a decapping enzyme in RNA catabolism. The enzymatic release of GMP from the Gp3A moiety was determined in the capped RNA model compounds Gp3A3'pA, Gp3A3'pA-isoprop and Gp3A2'pA in isolated rat liver nuclei; i.e., in the environment in which the dinucleoside triphosphatase operates in vivo. The Gp3A cap moiety is hydrolyzed in (3'-5') linked nucleotides only, whereas an extension of the Gp3A in the 2'-direction prevents the nuclear triphosphatase to operate.

[Twin pregnancy--a special problem of modern obstetric medicine].

Basing on an exemplary organized health protection incorporating up-to-date technique, progressive management of delivery and optimal neonatal care, we were able to reduce the perinatal mortality of all pairs of twins at the UFK Jena in the periods between 1977/79 (n = 204) and 1980/82 (n = 174) from 8,33% to 3,30%. The difference was statistically significant (p less than 0.05). The late morbidity of these children also showed a statistically significant decline (p less than 0.05).

[The "robotron NATALI" microcomputer-assisted birth monitoring system].

The microcomputer system "robotron-NATALI" from VEB Kombinat robotron Dresden, GDR, is applicable for bed side-monitoring and disposed about a new graphical representation of CTG's for fetal condition and uterine activity. A dialogue between physician or nurse and the computer is possible. After parturition all administered data are printed out in form of a partogram, inclusive the graphical representations during labor.

[A new form of representation of foetal cardiotocograms (author's transl)].

Based on a computerized analysis of CTG's we have developed a new graphical representation of fetal CTG's. The reproduction results from displays over 13 hours labor-progressing. A mathematical model is used. We have applied a discrimination function (DF) which is composed by 10 quantitative electronic parameters. Two fixed periods are compared. For mathematical and statistical evaluation the multivariate analysis of variance in connection with methods of dimension-reduction and discrimination was applied. A nonelementary discrimination function was taken into account. The value of the discrimination function was checked by means of postnatal data (pHNA and APGAR-score 5 minutes postnatal). The predictability "normal" pHNA - related to the computerized CTG- analysis - was exact in 80% (pHNA greater than or equal to 7,20). In 20% we found prepathological pHNA-values (pHNA:7,19-7,11). The predictability "prepathological" pHNA - related to the computerized CTG- analysis - was in 3,5% correlated with pathological pHNA-values (pHNA less than or equal to 7,10). Pathological pHNA - values may be almost ruled out by computerized CTG - analysis. Computerized "normal" CTG's were in 97% correlated with APGAR-scored values (5 minutes postnatal) > 7. Computerized "prepathological" or "pathological" CTG's were in 26% correlated with APGAR-score values (5 minutes postnatal) less than or equal to 7. The proposed computer-program is already used in this form in clinical practice.

[Influence of na-salicylate on uterine motility in labour (author's transl)].

A dosage of 10 mg/kg body weight Na-salicylate has no depressive effect on uterine motility. The examination carried out intra partum in the condition of normal uterine activity support our opinion that relatively high doses of 3--5 g per day or the simultaneous administration of beta-adrenergical substances are necessary to obtain an effective tocolysis.

[Computer evaluation of tocograms following administration of Spasmalgan (denaverin)].

Computer analysis of tocograms after administration of Spasmalgan (Denaverin). The influence of spasmolyant Spasmalgan on 5 parameters of labour has been examined in 50 intraamniotic derived tocograms by on line fetal monitoring. Only a little increase of the basal tone has been shown after administration of Spasmalgan. The method is recommended for testing the uterotropic influence of pharmacons in labour.

[Delta pH F changes sub partu and their clinical relevance].

Investigations of delta pHF-changes in labor led to the opinion, that there exist no agreement between this changes and the fetal or neonatal condition. Nevertheless they represent an important criterion--together with electronic parameters--for estimation of placental function. By achievment of limiting values (delta pHF greater than or equal to 0,10) and persistent pathological fetal heart rate patterns the termination of labor is necessary. A pHF less than 7,25 should no be reached. That ist of great importance for a low acidotic morbidity.--In this connection the determination of pHF to begin of the fetal monitoring is requisite. This is meaningful for the dynamics of pHF-values in labor in accordance with the fetal monitoring.

[Monitoring and delivery of the premature infant in breech presentation].

We explored 183 deliveries in breech presentation. The birthweights from this offsprings were 1941 +/- 418,1 g. 101 were primiparae. The perinatal mortality of the underweigth fetuses was 5 more higher when delivered vaginally in relation to delivery by caesarean section. These differences were statistically significant (p less than 0,05). In relation to multiparae 71,8% of the offsprings survived when delivered vaginally, in comparison to 79,2% offsprings from primiparae. The APGAR-scores (1 and 5 min. p. p.) less than or equal to 7 or greater than or equal to 8 showed no significant differences by the offsprings delivered vaginally or by caesarean section. The pHNA was higher in the group of children, delivered by means of caesarean section. The conclusions are discussed.

[On-line fetal monitoring (meaning for data acquisition) (author's transl)].

We demonstrate an enlarged and modernized equipment for the on-line-fetal monitoring. With the aid of illustrations the received informations by help of continuous on-line-fetal monitoring are demonstrated too. They concerned the deposition of electronical and corresponding gasanalytical dates. The importance of registration from all obstetrical parameters and given up drugs by a code-system is put out. Furthermore preferences of the computations are discussed.

[Concentration of heavy metals (Pb, Cd, Hg) in maternal blood].

BACKGROUND: It was the aim of these investigations to find out, whether or not pregnancy risks result from the heavy metal concentrations of the mother's blood (Pb, Cd, Hg). MATERIALS AND METHODS: The control group is composed of 125 women in child bed. Women with hypertonia during the pregnancy (n = 11), women after premature delivery (n = 25) and after a miscarriage (n = 21) are the risk collective. Blood: The investigations were carried out in EDTA-blood. Questionnaire: Place of residence, profession, nourishment and tooth amalgam surface. From this we received information about risk factors for a heavy metal burden. METHODS: The Cd- and Pb-concentrations were measured by "Graphitrohr-AAS with Zeemancorrection" and the Hg-concentration by "Cold-Vapour-AAS". RESULTS: 1. Women in child bed in the Jena area have a heavy metal concentration of Pb, Cd and Hg in the lower reference range. 2. At the risk pregnancies (hypertonia during the pregnancy, premature delivery and miscarriage) no increased heavy metal burden was found. 3. There was a significant correlation between the blood mercury level of women in child bed and the number of tooth amalgam fillings. DISCUSSION: In the Jena area women in child bed have a heavy metal concentration in the lower reference range. Environmental heavy metal burden producing frequent miscarriage, premature delivery and hypertonias in the pregnancy could not be shown. The tooth amalgam filling significantly increased the Hg-burden. Environmental stress and eating habits may also play a role. CONCLUSION: At the risk pregnancies (hypertonia during the pregnancy, premature delivery and miscarriage) no increased heavy metal burden was found.

[Computer-assisted cardiotocography analysis within the scope of continuous electronic labor monitoring with simultaneous use of noninvasive and invasive methods of registration].

In frame of a simultaneously study with noninvasive and invasive registration technic by application of 2 fetal monitors with following computer aided CTG-analysis at 22 patients we found in relation to the quantitative CTG-parameters DF (fetal condition), D (dip area), BI (bradycardia index) as well as BR (bradycardia residuelle) in part distinct differences for the range of distribution in spite of missing statistical significance (p less than 0.05) which point to insecurity in trigger signals for the noninvasive method. As exceptionally dangerous turned out to be false trigger signals in range of medium-term heart rate changes in attendance to auto-correlation US-DOPPLER-technic. The noninvasive registration of labour pains impressed forcibly for some time about longer CTG-phases to be unsuitable for the exactly judgment of uterine activity (Montevideo-unit). In conclusion the noninvasive method in supervision of labour should be considered more critical than earlier particular under forensic points of view.

[Assisted spontaneous delivery vs. Bracht manual aid within the scope of vaginal delivery in breech presentation. Late morbidity in children 5-7 years of age].

For judgment of late morbidity we investigated 52 children after vaginal delivery in breech presentation age from 5 to 7 years. This concerned 27 children after assisted spontaneous labour and 25 children after manual expression according to Bracht. All the children were mature and eutrophic and without neonatal distress after delivery. They were explored with help of an voluminous screening programme, concerning not only the intellectual development but also the psychomotoric capacity. The result was an very high intellectual conduct in frame of the test according to Raven (PR greater than 75). The difference in confrontation to the collective after manual expression according to Bracht was statistical significant (p less than 0.05). Neuropsychologic investigations according to Simernitzkaja demonstrated evident differences in disfavour to children after manual expression according to Bracht concerning the visual memory performance for symbols and letters. Further investigations referring to the graphomotoric accomplishment of one hand or both hands reinforced the opinion; that means the visual motoric totality performance in a narrowed sense or the psychomotoric performance in a enlarged sense was statistical significant poorer pronounced (p less than 0.05). The results underline the claim to estimate more critical the manual expression according to Bracht till now.

[Prospective nephrosonographic studies of the maternal urinary tract in pregnancy].

In this study 55 women (38 pregnant women without renal complications and 17 patients suffering from renal symptoms) are prospectively examined between the 16th week of pregnancy and 2 weeks after delivery using nephrosonography. Our results underline the value of the nephrosonographic investigation of the maternal kidneys during pregnancy and make clear the "physiologic" changes of renal parameters.

[Fetal movement and acceleration behavior in unimpaired and impaired newborn infants].

The following biophysical examination parameters were found by CTG-synchronous registration of fetal body and respiratory movements and compared in 7 fetuses impaired at birth (pHa less than 7.20, 1- and 5-minute Apgar scores less than or equal to 7 points) and 76 unimpaired born fetuses (pHa greater than or equal to 7.20, 1- and 5-minute Apgar scores greater than or equal to 8 points): number of fetal body movements per 10 minutes examination time; total and mean durations of these movements per 10 minutes; number of fetal respiratory movements per 10 minutes; number and total duration of fetal respiratory movement periods per 10 minutes; frequency of fetal respiratory movements; proportion of 10-minute periods with and without fetal body and/or respiratory movements and the amounts of the quotients from the amplitude of accelerations in fetal heart rate resulting from fetal body movements and the durations of these body movements. The mean interval between biophysical examination and childbirth was 11 days. Principally, the impaired born fetuses were less active in their movements than were unimpaired born fetuses. Apart from the frequency of fetal respiratory movements and the proportion of 10-minute periods without fetal body movements, the above mentioned parameters differ significantly with a probable error of alpha = 0.05. All biophysical parameters were registered on an uterus without labour and the cardiogram as well as the biophysical profile--with the exception of one fetus from the group of impaired borns--being unremarkable. The 10-minute periods without fetal body or respiratory movements were excluded from analysis.

[Movement and acceleration behavior of eutrophic and hypotrophic fetuses sub partu].

Cardiogram synchronous registration of fetal body and respiratory movements (real-time ultrasonic examination) was employed to compare the movement and acceleration behaviour of 130 normotrophic fetuses to that of 13 fetuses with body weights between the 6th and the 10th weight percentiles according to Kyank and of 13 fetuses with body weights less than or equal to the 5th percentile. The mean duration of examination was 80 minutes. The normotrophic fetuses exhibited the highest movement activity and reactivity of the cardiovascular system. In 88.6%, accelerations of fetal heart rate were associated with fetal body movements and were independent of fetal weight. Comparison with a previous communication of the authors revealed that intranatal fetal movement activity and reactivity of the cardiovascular system were only slightly below that with a uterus without labour. The only exception were fetal respiratory movements with significantly fewer intranasal observations.
