[Therapy of eczematous diseases in general practice].

The treatment of the different diseases which are comprised under the determination of "eczema" is mainly an anti-inflammatory treatment. Thus, topical corticosteroids play a large role. Development of more potent topical corticosteroids has lead to increasing numbers of unwanted side effects. In local therapy of eczema it is important to consider the phase of the disease when one chooses the vehicle: wet dressings, lotions, creams or ointments are applied according to the actual state of the skin. Special localisations also have to be considered e.g. in scrotal or perianal exzema.

[Regression of xanthomas in endogenous hypertriglyceridemia under low carbohydrate diet].

Report on two patients with endogenous hypertriglyceridemia. In both patients normal serum lipid values were reached in a comparatively short time under a diet with reduced carbohydrates and calories. In one diabetic patient who needed insulin at the beginning of the treatment the disease could be controlled by dietary measures alone after a few days. Different biological half-life periods of the various serum lipid fractions explain why, under a reducing diet, the rate of decrease of lipids is variable according to the respective component. Thus, the ratio of triglycerides and cholesterol can vary in the same patient within a few days and may change his classification under different types of hyperlipidemia according to Fredrickson. It would appear that in these cases a classification based on etiological considerations is more recommendable.

[Multiple subcutaneous Trichophyton rubrum abscesses. Pathomorphosis of a generalized superficial tinea due to impaired immunological resistance].

A patient with chronic polyarthritis is reported who was treated with corticosteroids over several years and also received azathioprine. The patient developed extensive tinea pedum and corporis which was suddenly complicated by multiple subcutaneous abscesses caused by the same organism, Trichophyton rubrum. This type of dermatophytosis has only rarely been described. It shares some characteristics with the maladie dermatophytique of Hadida and Schousboe. Chronic granulomatous dermatophytoses such as granuloma trichophyticum Majocchi or tinea granulomatosa nodularis cruris or acute suppurative dermatophytoses of the kerion Celsi type present different clinical and histological pictures and develop under different pathogenetic conditions.

Contact radiotherapy of cutaneous hemangiomas: therapeutic effects and radiation sequelae in 818 patients.

The paper presents statistical data on the therapeutic effects and radiation sequelae following Chaoul contact therapy of 818 cutaneous hemangiomas treated between 1938 and 1952. 73.3% of all irradiated hemangiomas showed initial improvement but complete involution with an excellent cosmetic result was observed in only 50% of lesions 5 years after treatment. During the same period of time, more than one-third of all patients developed mild to moderate cutaneous radiation sequelae (hyper- or hypopigmentation and telangiectases, rarely atrophy). The high incidence of late radiation effects is probably relation to the high total doses administered in this series of patients, the very short intervals between treatments and the age of the patients. Other radiation radiation hazards are also discussed. Since large studies have proven conclusively that spontaneous involution occurs in 95% of hemangiomas after several years, indications for radiotherapy of hemangiomas are extremely limited.

[Current problems and new developments in therapy of mycoses (author's transl)].

Mycoses for most of them) represent a group of infectious diseases which seem to increase steadily although numerous fungicidal or fungistatic therapeutics are available. A severe problem is provided by the so-called opportunistic fungi which become parasitic only after the host's immunological protection has been impaired by predisposing factors. Therapy resistance and prevention of relapse are problems of a general nature in the therapy of mycoses. As special topics local treatment of dermatophytoses, of Candida mycoses, and new development in systemic treatment of deep mycoses are discussed.
