A simple method of distinguishing the bacterial viruses T3 and T7, and a critical reevaluation of their heterologous and homologous exclusion.

A method is presented allowing a clear distinction between bacterial viruses T3 and T7 by plating on selectively permissive host cells. The indicator strains are Escherichia coli cells containing either cloned pif genes (exclusively permissive for T3) or the EcoRV DNA restriction system (permissive only for T7): The efficiencies of plating of the two phages on these hosts differ by more than 8 orders of magnitude. This method was applied to reinvestigate the controversial question of mutual exclusion between T3 and T7. Under single-burst conditions, about 50% of coinfected cells (permissive for both viruses) produced T3 and T7 progeny while about 25% reproduced only T3 and about 25% only T7. The burst size of co-infected cells was slightly reduced, compared to controls infected with only one virus type. Homologous exclusion among T3 phages was also not seen; rather, there was a gene dosage effect: T3-encoded RNA polymerase activity as well as T3-specific RNA synthesis increased proportionally to the multiplicity of infection (2.5-20 plaque-forming units/cell).

Cation distribution in the organ of Corti of the guinea pig.

Cations were precipitated with potassium antimonate in the cochlea of the guinea pig and the distribution of the formed precipitates was studied by electron microscopy. The precipitate density in different cells of the organ of Corti was determined on electron micrographs by counting numbers of precipitates per unit area. The spatial distribution of the precipitates was also determined by electron spectroscopic imaging (ESI). Significant differences were found among the cells of the same tissue being analyzed. These precipitate-rich cells may play a role in a postulated current flow in the organ of Corti.

Ultrastructural localization of Ca2+-binding sites in the spiral limbus, the stria vascularis and Reissner's membrane of the guinea pig.

Cations were precipitated with potassium antimonate in the cochlea of the guinea pig and electron microscopy was used to analyze the distribution of the formed reaction products. Differences in precipitate density between neighboring cells in Reissner's membrane, in the stria vascularis and in the limbus are described. Electron spectroscopic imaging was performed to obtain information about the spatial distribution of the precipitates and their elemental composition.

Diseases of the intrapetrous carotid artery.

In the early years of skull base surgery, total tumor removal was often deemed impossible due to involvement of the intrapetrous carotid artery. In contrast, previously considered unresectable lesions may be removed totally in selected cases, with reasonable expectation of successful treatment at the present time. Current techniques and operative exposures, when modified to accommodate the problem of intracranial extensions of tumor and when approached with neurosurgical collaboration, permit the removal of many of these difficult tumors. This retrospective study of 33 patients treated over the past 5 years offers a critical analysis of our treatment, and a categorization of abnormalities known to affect this anatomic region containing the vessel.

Cranioplasty with Palacos R in reconstruction of frontal sinus defects.

The use of methylmetacrylates has not been widely accepted for the repair of frontal bony defects due to the possibility of secondary infection. The development of a new type of methylmetacrylates into which antibiotics, such as gentamicin, are incorporated has reduced the risk of secondary infection. The surgical technique and the results are illustrated on three patients. Favourable results were obtained in small and in large bony defects and even after previous frontal osteomyelitis.

[Ultrasonic diagnosis of the salivary glands].

Sonography of the salivary glands is a valuable diagnostic tool in acute and chronic sialadenitis, sialadenoses, lymphadenopathies and tumours lesions. A new examination technique is presented: ultrasound-guided fine needle biopsy. We report on our experience in 308 patients with parotid and in 47 patients with submaxillary gland diseases examined between 1982 and 1987. The diagnostic accuracy and limitations of sonography are discussed compared with sialography, computed tomography and magnetic resonance imaging.

Polyclonal IgG synthesis of mononuclear cells induced by the homopolymer of 4,6-dideoxy-4-formamido-alpha-D-mannopyranosyl units isolated from Brucella melitensis.

Brucella melitensis attaches selectively to B lymphocytes. The bacterium induces in vitro only a slight proliferation of mononuclear cells and inhibits the spontaneous IgG synthesis. In order to characterize these effects, the O-chain of the lipopolysaccharide of Brucella melitensis (a polysaccharide consisting of poly-4,6-dideoxy-4-formamido-alpha-D-mannoside) was prepared and its influence on lymphocyte functions tested. This compound had no effect on blast transformation whereas IgG synthesis of the cells was stimulated more than tenfold by the addition of the Brucella polymannoside.

[Tympanoplasty with incus interposition].

We report on a 5-year experience with 44 patients (1980-1985) with incus interposition using a modelled or sculptured incus, either autograft or homograft, to correct ossicular discontinuity when a functional malleus and stapes are present. After a period of 21 months, 75% of our patients achieved an air-bone gap closure between 1 and 10 dB. The postoperative hearing results of patients with trauma and middle ear atelectases were better than the hearing results of patients with cholesteatomas. These long-term results are dependent upon intact malleus attic ligaments and an intact tensor tympani tendon. It is very important that the modelled incus is repositioned under the malleus handle neck.

[A rare nasopharyngeal tumor: Hodgkin's disease].

Primary cases of Hodgkin's disease in the nasopharynx are extremely rare. The authors report on two such cases: A 27 year old male had a large tumour, lined with a smooth mucous membrane, in the nasopharynx. The results of examinations of the neck, thorax, and abdomen were normal. The tumour was removed through the palate. Histological findings confirmed the diagnosis of lymphocyte-prominent Hodgkin's disease (subtype paragranuloma). 11 years ago a nasopharynx tumour was removed by adenotomy from a 57 year old female. A diffuse hyperplasia of the lymphatic tissue was diagnosed at the time. Recently cervical lymph nodes appeared, and a second tumorous lesion was discovered in the nasopharynx. Histological examination of the tumour and cervical lymph node indicated lymphocyte-prominent Hodgkin's disease (subtype paragranuloma). On re-examining histological sections from the tumour removed 11 years ago, the authors again saw the types of change indicative of Hodgkin's disease. The probable course and outcome of lymphocyte-prominent Hodgkin's disease of the nasopharynx, especially of the paragranuloma subtype of the disease, are discussed. Treatment of both patients included adjuvant extended-field radiotherapy.

The significance of calcium antagonists in rat experimental tympanosclerosis.

The effect of calcium antagonists in experimental tympanosclerosis following infection with Streptococcus pyogenes and vitamin D3 intoxication has been analyzed in a rat animal model. Compared with untreated animals, calcium antagonists exert a positive effect on the degenerative process and on secondary calcification in the subepithelial layer. This effect could be substantiated planimetrically.

Influence of autoclaved Brucella melitensis on the proliferation of human lymphocytes.

Brucella melitensis is a facultative intracellular bacterium which binds spontaneously to human B-lymphocytes. In order to find out whether there exists a relation between bacterial adherence and lymphocyte reactivity, the proliferation response of normal adult peripheral blood lymphocytes to autoclaved B. melitensis has been studied. Our results indicate that autoclaved B. melitensis induce only a slight increase in proliferation. This low lymphocyte response is due to proliferating B-cells. In high doses B. melitensis inhibited the response of lymphocytes to Concanavalin A and phytohaemagglutinin. This effect is probably due to the presence of lectin binding saccharides on the bacterial surface competing with the T-cells for the mitogens.

[Calcium antagonists and damage to the organ of Corti in acoustic trauma].

The protective effect of Ca2+ antagonists in acoustic trauma was evaluated by morphometric analysis of the guinea pig cochlea. Damage to the outer and inner hair cells was significantly reduced. Ca2+ antagonists may exert a beneficial influence on the cellular membrane of the sensory cell or on the cochlear vasculature if given in advance and in the early post-traumatic phase.

[Neo-adjuvant chemotherapy of head and neck cancer].

Pre-operative chemotherapy with vincristine, moderately high doses of methotrexate plus "rescue", and bleomycin for six weeks was administered to 26 patients with untreated carcinoma of the head and neck. Treatment was well tolerated and compliance was very good. Objective side effects necessitated ending treatment in ten patients. Tumour regression of over 50% (responders with partial or complete remission) was recorded in 17 patients; five of them had no histological evidence of tumour in the surgical specimen. In three patients the tumour regressed slightly, in two it remained unchanged and in four it had progressed at the end of treatment (nine non-responders). After a minimum observation period of 36 months 12 of the 26 patients were still alive. Median survival time had not yet been reached in the 17 responders. But the median survival time of the non-responders was 15 months. It is concluded that this form of treatment is very effective and well tolerated. If response is achieved it opens the prospect of prolonged survival time.

Experimental tympanosclerosis following infection with Streptococcus pyogenes and vitamin D3 intoxication.

A rat animal model was used to study the ultrastructure of submucosal calcifications induced in the middle ear following inoculation with Streptococcus pyogenes and high doses of parenteral vitamin D3. The morphological changes present in affected animals resembled the classical picture of tympanosclerosis. While calcification occurred about bacterial remnants and myelin structures, the most important calcification centers were lysosomal and non-lysosomal matrix vesicles in the extracellular spaces. These formed band-like calcifications close to the basal membrane without affecting the epithelial layer. This animal model offers the possibility of studying the effect of various therapeutic regimens in the treatment of the dynamic tympanosclerotic process.
