[Surgical therapy of glomus jugulare tumors].

A one-stage resection of large glomus jugulare tumours with intra- and extracranial extensions is described. The patient is placed in the sitting position. Monitoring minimizes the danger of air embolism. Modifications of technique are described which reduce the intraoperative blood loss to approximately 1,000 ml.

[The pathogenesis of otitis externa].

In otitis externa the formerly grampositive colonization of the auditory canal changes towards a gramnegative microbial flora. Pseudomonas aeruginosa plays a major role in recurrent infections and in swimmer's ears. Various serotypes of ps. aeruginosa i.e. type 1;6 and 5 could be isolated. In swimmer's ears ps. maltophilia was identified twice. There was no correlation between the severity of the disease and the protease and lipase activity of pseudomonas isolates. Exoenzyme activity seemed to bear influence on the chronicity of the disease. Therapeutic success led to a decline of enzymatic activity.

[Cytomorphologic and cytophotometric studies in malignant midline reticulosis].

Biopsies of two patients with a clinical diagnosis of lethal midline granuloma were examined by electron microscopy, histology and cytology. Morphological changes included necrosis, inflammation, and infiltration of atypical histiocytic cells. Judging by the cellular characteristics, the DNA-content and -distribution pattern, this localised, invasive and destructive lesion is a low-grade malignant histiocytic tumour. Our results show that final tumour diagnosis can only be established by special cytological methods.

[Functional results of stapes surgery].

In 135 patients the functional results of stapedotomy and stapedectomy were controlled. As a rule, stapedotomy results were superior to stapedectomy results. Small hole stapedotomy using a 0.4 mm. teflon-wire piston yielded optimal sound transmission between 500-8000 Hz. In addition, this was the only technique to prevent cochlear damage at 4000 Hz and 8000 Hz.

[The development of the nose following trauma in the growing stage].

In a longitudinal study, esthetic and functional sequelae after nasal fracture in 49 patients during the growth period had been analysed. The results were estimated according to the following criteria: extent of trauma, timing of trauma and mode of therapy. The sequelae of severe fractures were determined primarily by the extent of initial trauma. In case of minor trauma, the long-term results were influenced by age factor and mode of treatment. In adolescents good cosmetic results were obtained by restoration of the integrity of nasal structures. In infantile trauma, the realignment of the structures yielded good functional but poor cosmetic results. In untreated infants good cosmetic results were paid at the expense of insufficient nasal function.

[Systemic disease closely related to Cogan's syndrome (author's transl)].

In a 51-year-old patient with non-syphilitic interstitial keratitis and disturbances of inner ear function (Cogan's syndrome) the course of the disease could be followed for 8 years. Organ manifestations which indicated generalised vascular disease were present in accordance with reports in the literature of this rare syndrome. In addition to multiple digital arterial occlusions, recurrent pulmonary infiltrations were observed which have not previously been described in connection with this syndrome. With the exception of the inner ear hearing loss the symptoms could be improved by systemic administration of corticosteroids and azathioprine. The patient remained free of recurrence for over three years.

[Characterization of a CaCl2-dependent transfection system of Escherichia coli and T3 phage DNA].

Transfection by DNA isolated from bacteriophage T3 was studied using Escherichia coli 921/0 as host. The following conditions were found optimal: Competent E. coli 921/0 were obtained by harvesting the bacteria at the onset of late exponential growth (5 X 10(8) cells/ml) and treating the latter with 0.05 M CaCl2. Hereafter, the microbes were suspended in 50 mM Tris-HCl buffer (pH 7.2) and the concentration adjusted to 7 X 10(9) cells/ml. T3 DNA was added and the suspension kept at 0 degrees C for 15 min. Determination of the number of infectious centers was then carried out in the usual way. The efficiency of transfection under these conditions amounted to 10(4) p. f. u./microgram DNA. Preincubation of competent bacteria with T4 DNA at 0 degrees C before the addition of T3 DNA reduced the number of infectious centers. However, if T3- and T4 DNA were added simultaneously no decrease of the transfection efficiency occurred. Calf thymus DNA was without influence on transfection.

[Sandwich irradiation of patients with squamous cell carcinoma of the tonsil, the base of the tongue and the hypopharynx].

Taking into account the death rate of all other causes for that age group and comparing the survival time and the recurrence rate in patients with preoperative short-time irradiation with telecobalt 60 and a focal dose of 20 Gy with a second group of patients treated with postoperative radiotherapy, we were unable to detect any advantage of the so-called "sandwich therapy" in cancer of the tonsils, the base of the tongue or the hypopharynx. On the contrary: patients with "sandwich therapy" had to be hospitalized longer because of the interval between preoperative radiotherapy and surgery and because of the delay in restoring postoperative speech and deglutition.
