The treatment of condylar fractures is a matter of controversy. After re-examination of 219 patients, who had previous fractures of the mandibular condyle with dislocation, treated by open reduction, conservative or functional methods, the following observations are made: The conservative treatment will lead to the most favourable results. After functional treatment the condylar remodelling is rare and in majority of cases the function of medial pterygoid muscle is not restored.
Tridemorph (N-tridecyl-2,6-dimethylmorpholine) inhibits both the NADH-oxidase and the succinate-cytochrome c oxydoreductase system of non-phosphorylating electron transfer particles from beef heart. The concentration required for half-inhibition amounted to 3,4 muM and 24 muM respectively. Two different sites of action in the respiratory chain could be localized by means of difference spectroscopy and measurements of enzymic activities in various partial systems. The inhibition of the NADH-ubiquinone oxydoreductase activity as well as the suppression of the NADH-induced reduction of all cytochromes on the one hand and the insensitivity of the NADH-ferricyanide oxydoreductase system on the other argue in favour of a site of action similar to rotenone. The partial suppression of the succinate-induced reduction of cytochrome b with simultaneous complete inhibition of the reduction of the other cytochromes indicate an additional site of action analogous to antimycin A. Both inhibitory actions appeared instantaneously after the addition of tridemorph and were counteracted by serum albumin. Furthermore, tridemorph inhibited the oxydation of external ferrocytochrome c but not that of ascorbate/tetra-methyl-p-phenylene-diamine-HCI (TMPID) showing that it is not a true inhibitor of the cytochrome oxidase. The TMPD-induced bypass of the succinate oxidation was inhibited as well. The possible role of the inhibition of the main pathway of the respiratory chain for the fungicidal action of tridemorph is discussed.
The three alcaloids inhibited the NADH oxidase system of electron transfer particles from beef heart up to 90--100 percent. The concentrations of half-inhibition amounted to 50 muM for berberine sulphate and tetrahydropalmatine and 0.55 mM for alpinigenine. All three compounds showed comparable inhibitions on the succinate-cytochrome c oxidoreductase system only at concentrations 20--25 times as high. The site of action may be the iron sulphur region of the complex I of the electron transfer system. The biological importance of this respiratory inhibitions should be taken into account.

Rheumatoid arthritis (RA) presents a variety of immunological phenomena which can be of considerable diagnostic value. Of great significance is the determination of rheumatoid factors. Other autoantibodies are common but not specific for RA. Nevertheless the detection of such antibodies is beneficial for differential diagnostic considerations. Commonly demonstrated in rheumatoid arthritis are immune complexes which can be either soluble or within conclusions of circulating blood leucocytes or of synovial cells. Complement diminution can also be observed in RA, seldom in the serum, more often in synovial effusions. Like other humoral immune findings it lacks also disease specificity. A functional impairment of cellular immunity has also been postulated for rheumatoid arthritis, however the techniques for its demonstration are still cumbersome and cannot be easily used for routine diagnostic procedures. In conclusion, none of the immunological findings in RA is specific, however together with the anamnestic and clinical data it is possible to establish an exact diagnosis of this disease.

Since the introduction of new cytogenetic methods, as the "Pancreatin-Giemsa"-stain and similar methods, it is possible for the first time to determine the chromosomes of group G accurately. There are few cases reported in the literature with an additional small chromosome which could be associated with group G. None of those children presented the classical signs of a Down-Syndrome, therefore a trisomy 22 was discussed. This is the report of a female patient, on whom trisomy 22 was confirmed cytogenetically. So far it was only in a small number of cases possible to prove this thesis by cytogenetic studies. We compare our findings with previous reports of suspected or proven cases of trisomy 22.
Eight pathologic fractures of benign and 52 of malignant origin are reported. Open reduction and rigid internal fixation should be performed to give the patient the use of his affected limb as soon as this can be accomplished. Treatment consists of internal fixation in lesions of the shaft and prosthetic replacement in the case of lesions near joints. Only very rarely a primary amputation must be performed. Malignant fractures are most frequently caused by metastases from breast cancers. The treatment will at least make nursing easier, and three-quarters of these patients can be mobilized. The combination of surgical treatment and radiotherapy is discussed. In spite of this approach the average time of survival of 14 months is short because a pathological fracture due to a malignant tumor is a late symptom of the disease.
An account is given of 12 high ruptures of the triceps surae, a traumatic lesion of the medial belly of the gastrocnemius, caused by sudden overstreching of the muscle by concomitant ankle dorsiflexion and full knee extension. In all cases the authors found characteristic clinical features. Surgical exploration in 5 of 12 cases verified the ruptures of the musculotendinous junction of the medial gastrocnemius belly. A follow-up study of 2 years showed that the surgical treatment seems to be better, especially in younger and athletic patients.
The radiological workup of 15 patients with a carcinoid has been evaluated. As 7 of the 15 patients presented for emergency operation, only the remaining 8 had barium studies (7 barium meals and 1 barium enema). Two of the barium meals were even retrospectively, entirely negative. In two others, initially the diagnosis was missed despite radiological signs, which had been overlooked on follow-up films. Fluoroscopic examination of the small bowel would probably have ensured the detection of the lesion. At the time of diagnosis nine tenth of the tumours were already infiltrating and more than half of them metastasizing. The presence of the carcinoid syndrome implies the production of the causative hormones in the liver or the pulmonary circulation. In order to find the primary intestinal carcinoid in a cureable stage- that is, before it has metastasized-, all patients with diarrhoea and repeated bouts of abdominal pain should have a careful follow-up examination of the small bowel. This examination must include fluoroscopies.
Using a standardized method of obtaining bacterial counts of surfaces by superficial application and subsequent incubation of agar lamina, density and kind of bacteria present were examined in treatment areas of both individual dentists praxis and of dental poli-clinics. The results obtained showed, that 1. typical bacteria of hospital infections (Micrococcus aureus, Klebsiella) were present in only small numbers in both individual dental praxis or in clinical dental treatment areas. 2. the medium level of contamination was higher in the average individual dental praxis than in comparable clinical dental areas. 3. the increase in bacterial contamination in the course of the daily treatmentperiod was considerably more pronounced in the individual praxis as compared to clinics. 4. the following must be considered as especially liable to contaminated by bacteria: towels used by several persons, the head of the dental turbine, the support of the suction device, the patient head rest, the height control bar of the dental chair, the instrument tray, the operating lamp and working garments.
