[Clinical diagnosis of bladder cancer].

The minimum requirements of the International Union against Cancer (UICC) to assess the TNM-classification of urothelial bladder cancer include since 1978 the clinical examination, urography, cystoscopy, bimanual palpation under anesthesia and biopsy or transurethral resection of the tumor. The single biopsy of the tumor should be supplemented by multiple biopsies of all bladder walls in combination with the resection of the tumor to diagnose multifocal tumor growth. The exfoliative urinary cytology should be included in the routine diagnostic regimen: cytology is helpful in detection of severe dysplasia and controls the histologically determined grade of the tumor. In selected cases computer tomography improves the accuracy of clinical evaluation of the depth of tumor infiltration. In our hands the lymphangiography can not assess the regional lymph node metastases: therefore, the staging operation is recommended before radical cystectomy is performed.

Spontaneous phasic activity of the detrusor: a cause of uninhibited contractions in unstable bladders?

Uninhibited detrusor contractions of neurogenic origin have been described repeatedly. Since a spontaneous myogenic activity of the detrusor has been demonstrated in isolated muscle strips, it seems reasonable that an increase of this spontaneous contraction activity may induce bladder instability of pure myogenic origin. 30 patients separated into two groups were investigated. All patients suffered from involuntary contractions of the detrusor, accompanied by loss of urine. Nifedipin was applied as a selective Ca-antagonistic drug to block the phasic detrusor activity. In our experiment, involuntary detrusor contractions could not be suppressed, and also bladder capacity could not be increased by this drug. This result leads to the assumption that the spontaneous phasic activity of the detrusor does not induce any involuntary bladder contraction.

[Belt-Fuqua technique: a useful alternative in hypospadias repair (author's transl)].

This Belt-Fuqua method is of particular help when the entire foreskin lies on the ventral surface of the penis. Such a situation results from an exaggerated buttonhole technique at the time the chordee is corrected as the initial procedure either by chance or intentionally. A portion of the transposed foreskin is used to fashion a tube of appropriate length and diameter to reach the end of the penis. This new urethra is then drawn through a tunnel on the ventrum of the penis. This operation has several advantages over the available alternative procedures: 1. The Belt-Fuqua technique uses the entire foreskin on the ventral surface of the penis for the extension of the uretha. 2. The abundance of foreskin allows easily to fashion a complete tube of appropriate length and diameter. 3. The new urethra is completely covered by intact subcutaneous tissue and the remainder of the foreskin and is crossed by no sutureline. 4. The procedure is relatively simple to perform, is less likely to result in fistula formation than the alternative procedure used and has not been accompanied by the complications of stricture or meatal stenosis in our hands.

[The septic kidney: primary nephrectomy or conservative surgery? A clinical study of 110 cases (author's transl)].

The "Septic Kidney" usually originates from infected hydronephrosis. The clinical appearance is characterized by poor general condition, in particular shock, severe flank pain, high fever with chills, leucocytosis and often azotemia. The pathogenesis and definition are discussed on the basis of 110 cases. The various therapeutic modalities such as primary nephrectomy, conservative surgery or endoscopic instrumentation are compared and the indications defined. Conservative procedures are preferred over primary nephrectomy.

The time-distance diagram: a new method to analyze ureteral peristalsis by cineradiography.

We assessed ureteral peristalsis in dogs by cineradiography and analyzed the films in a single frame follow-up by measuring the position of bolus front and rear end in relation to the bladder and the kidney. The data were plotted as function of time in a time-distance diagram. The bolus rear end which is the active contractile rings, as proven by electromyography, travels with constant velocity. The bolus front shows remarkable changes in propagation velocity which can be explained by differences in wall forces that resist the opening of the ureteral lumen. Thus, the time-distance diagram describes precisely the constantly changing dynamics of ureteral peristalsis along the entire ureter.

Grading of transitional cell tumours of the urinary tract by urinary cytology.

1122 cytological diagnoses have been made on 2500 Papanicolaou stained cytological slides from Patients with urothelial carcinomas. The cytological accuracy was 78% for G1 tumours, 94% for G2 and 96% for G3 carcinomas. A histologically diagnosed G1 carcinoma was cytologically G1 in 81% of cases, a G2 carcinoma was recognized in 64% and G3 carcinoma in 48%.

Therapy of urothelial bladder tumors. Results of a retrospective study of 615 cases.

615 urothelial bladder tumors are evaluated retrospectively after reclassification of histological and cytological specimens according to the recommendations of the World Health Organization (1973) and International Union Against Cancer (1974). The reason for a subdivision of the T1 category for exophytic noninfiltrating carcinomas is discussed. The importance of stage and grade is demonstrated as well as the different prognosis of patients with primary or recurrent tumors of the same T category. The results of the study demand a differentiated therapeutic regimen for urothelial bladder tumors.

Paranephric abscess: a changing concept.

The aetiology of the paranephric abscess has been greatly modified since the introductiion of antibiotics. Its frequency has decreased but its prognosis has not been improved. 20 cases of paranephric abscess have been studied retrospectively. Its main cause is no longer the haematogenic staphylococcus infection but primary kidney infection. The disease is difficult to diagnose as the evolution is slow and the symptoms unspecific. The main symptoms are: abdominal pain, feeling of physical prostation, subfebrility, acute pain in the flank and significant increase in blood sedimentation rate. The infection is due in most cases to a silent kidney or a kidney stone with pyonephrosis. Accompaying diabetes mellitus was often observed.

Resorptive and absorptive hypercalciuria. Therapy with sodium cellulose phosphate or thiazides.

Patients with recurrent stone disease and hypercalciuria were cleared up according to Nordin's schedule. In cases of absorptive hypercalciuria, an ion exchanger operating in the intestine, sodium cellulose phosphate (SCP), is applied under strict control of oxalate, calcium and magnesium excretion as well as ionized calcium in serum. Under treatment with SCP (27 patients), we found a reduction in the renal excretion of calcium and magnesium, and, as a side effect, a significant augmentation of the renal oxalate excretion. In cases of resorptive or resorptive/absorptive hypercalciuria, except in patients with primary HPT, 23 patients were mediated by thiazides (Esidrix). This drug effects a marked decrease of urinary calcium based on a higher rate of reabsorption of calcium in the distal tubule. No severe side effects especially primary HPT were observed.

Long-term treatment of hypercalciuria with thiazides, sodium or potassium cellulose phosphate, separately or in combination.

Urinary supersaturation in respect to brushite or calcium oxalate represents the main pathogenic factor in stone formation. Of the patients with calcium oxalate stones 30 to 40% present with hypercalciuria. Herein we determine and compare the effects and side effects in the treatment of hypercalciuria with sodium cellulose phosphate or Campanyl, a potassium versus calcium ion exchanger, and thiazides alone or in combination with an ion exchanger.

Calcium oxalate stone disease: effects and side effects of cellulose phosphate and succinate in long-term treatment of absorptive hypercalciuria or hyperoxaluria.

Absorptive hypercalciuria was treated in 27 patients with cellulose phosphate. In all patients urinary calcium decreased and stone formation virtually ceased. The most striking side effect was an excessive hyperoxaluria, necessitating withdrawal of the drug in 8 patients. Succinate decreased the hyperoxaluria in 14 of 19 patients. All patients had mild hypercalciuria and hypermagnesiuria. This study was done to determine the therapeutic value and the side effects in the treatment of absorptive hypercalciuria with sodium cellulose phosphate and of hyperoxaluria with succinate.

Effects of beta-adrenergic stimulating and blocking agents on the dynamics of the human bladder outlet.

The alpha-adrenergic innervation of the functional urethra is a well-known fact, while beta-adrenergic influence is rather unknown until now. We studied the influence of beta-stimulating and beta-blocking agents on the human urethra by the urethral pressure profile (UPP). A decrease of the UPP under orciprenaline sulfate and an increase under propranolol could be mentioned.

Pacemaker localization in the renal pelvis of the unicalyceal kidney. In vitro study in the rabbit.

To determine the precise localization of the pacemaker in the renal pelvis of the unicalyceal rabbit kidney, the spontaneous activity of muscle strip preparations was investigated in vitro. (1) The highest frequency of spontaneous contractions was found in the most intrarenal part of the renal pelvis. (2) The frequency diminished from the most intrarenal parts to the pyeloureteral junction. (3) No spontaneous activity was observed in ureter preparations; even adrenaline, noradrenaline, acetylcholine and oxytocin did not start any contraction. In the pyeloureteral system the cells with the highest spontaneous frequency will act as a pacemaker. Cells with a less frequent activity can only work as secondary or latent pacemakers.

Ureteral calculus: experience with 521 stone extractions.

Few urologic problems require the consideration of so many factors as does an obstructing calculus in the ureter. The questions that arise in each case are whether an operation should be done for immediate relief of the obstruction or whether manipulation should be done, possibly subjecting the patient to a period of disability, painful attacks of colic and febrile reactions.

[Urological and nephrological complications after general surgery. Urological complications. Kidney and ureter].

Urologic complications can result from abdominal, gynecologic, and retroperitoneal operations. As a result, surgical treatment of colonic tumors, genital tumors, the vertebral column, the lymphatic system, and the aortoiliacal vessels assumes a greater importance. Immediate therapy is possible as a result of intraoperative diagnosis. The postoperative diagnosis may become manifest as early or late symptoms. Certain criteria for avoiding such complications are mentioned. Following diagnosis of a urologic complication, immediate therapy seems to produce the best results and the shortest period of hospitalization.
