[The fascial planes round the rectum. Anatomic and computer tomographic correlations (author's transl)].

About 1000 patients mostly with rectal carcinomas but also with hematomas, abscesses and soft tissue tumors received CT-examinations of the posterior compartment of the pelvis before and after surgical treatment and radiation therapy. The morphology of the perirectal space is demonstrated under normal and pathologic conditions and can be compared with the current anatomical knowledge. Special attention has been focussed on the site of the so called "Hüllfaszien" or "Grenzlamellen" of the rectum and the division of the perirectal space. Nomenclature and description of the perirectal fasciae and spaces are somewhat controversial. Therefore misinterpretations of pathologic CT-findings are not uncommon.

Blood vessels and nerves of the human penis.

The spongy tissue of the penis together with the arteries and veins functionally becomes an erectile system because these structures are surrounded by layer of collagenous and elastic fibers capable to withstand blood pressure and to stretch. The caliber of the terminal branches of the arteries inside the spongy tissue is modified due to the presence of epitheloid cells and smooth muscle cells. The veins drain the blood into three main regions: the femoral region, the deep pelvic region and the upper pelvic region. The penile nerves enter the ilioinguinal nerves, the perineal nerves, the dorsal penile nerve and the pudendal nerves. These afferent fibers terminate in the spinal cord (Th 12-S 4). The parasympathetic and sympathetic fibers run together with the pudendal nerves and arise in S 2-S 4 or Th 12-L 1. Both the pudendal nerves and the hypogastric ganglia must be carefully spared in the course of all surgical procedures.

[Bioconstruction of the human ileocaecal valve (author's transl)].

The lips of the ileocaecal valve are permanent folds parallel to the long axis of the small intestine, which are formed by invagination of all layers of the terminal ileum into the caecum. In the basal two-thirds they contain processes of all muscular layers of ileum and caecum, which form a system of muscle bundles that is connected with the tunica muscularis mucosae. The luminal part of the lips contains a venous plexus embedded in the loose plexus of the tunica mucosa. Arteriovenous anastomoses facilitate the rapid filling of these venous plexus. The ileocaecal valve in humans acts as a combined angiomuscular-angioplical lock-system that under normal conditions prohibits a caeco-ideal reflux.

[The myoarchitecture of the pylorus (author's transl)].

The gastric pylorus is not a constricting sphincter but a valve closing on distension, resembling the situation in the lower esophageal sphincter. Both contiguous segments (stomach and duodenum) are able to regulate its function physiologically allowing also some degree of reflux.

[The arteries of the forehead as the basis of nasal reconstruction with forehead flaps].

For the reconstruction of nasal defects, paramedian forehead flaps with a small pedicel are used. The nutrition of these flaps depends on the presence of an arterial blood vessel in the pedicel. In constructing the flap it is absolutely necessary to know the position and the course of the vessles in the forehead. The branches of the ophthalmic artery - dorsal nasal artery, supratrochlear artery, and supraorbital artery - are responsible for the nutrition of the forehead. These arteries are connected with those lying next to them. Additionally, they all anastomose with the frontal branch of the superficial temporal artery. The branches of the ophthalmic artery, lying in a more lateral position, follow a diagonal course compared with those lying more medially. Considering all forehead vessels, the forehead branch of the dorsal nasal artery is widely unknown, although it is very important for the supply of the central part of the forehead. Together with the supratrochlear artery, which lies on a paramedian line passing through the medial angle of the eye, it represents the nutrition vessels of the forehead flap. In case of injury of the ophthalmic artery, the anastomosis of the angular artery with the dorsal nasal artery can be of great importance for the supply of the flap.

[Further investigations of insufficient stretching-closing mechanism of the terminal esophagus (author's transl)].

The anatomy of the closing mechanism of the terminal esophagus was studied in the dogs since this animal is frequently utilized for investigations of esophageal function. With the aid of a special method we have demonstrated a stretching-closing mechanism similar to that of the human subject. An operatively placed hiatus hernia with a tendency to reflux shows the expected reaction of the muscular screw mechanism. This insufficient stretching-closing mechanism is similar to an insufficient "ring sphincter". The reconstruction of the closing mechanism by means of "stretching and gastropexie" which has proven successful in practice is discussed.

[Radiation effects in brain and spinal cord (author's transl)].

Radiation sensitivity of both the brain and spinal cord in prenatal and postnatal stages, in infancy and adult age is represented also in consideration of a combined treatment with methotrexate. In adults, application of important doses of high-energy radiation increases the risk of injurious effects to the central nervous system. If the spinal cord is involved, more than 60% of the radiolesions have a progredient course ending with death. The pathogenesis and disposing factors are referred to, and the incidence of radiation necrosis with regard to age and sex, the degrees of injury and their frequence within different ranges of dosage are analyzed on the basis of data from universal literature. An examination of "tolerance doses" for the spinal cord is made by means of Strandquist-diagrams and of the Ellis-formula. The slopes of regression lines are reported for various "degrees of response" in skin, brain and spinal cord following radiation therapy. In the Strandquist-diagram, slopes of regression lines are dependent on the "degree of response", flattening if skin and spinal cord are affected by radiation in the same degree, necroses having the same slope for both the organs.

[The development of diverticulosis and diverticulitis (author's transl)].

The cause of colonic diverticula is myostatic spasm. A low-residue diet contributes to this state as transit time through the bowel is slowed. Information gathered in 102 cases was analysed. The smooth muscle architecture of specially prepared operative specimens of diverticulosis were examined. In every case an asymmetric contracture of the smooth musculature led to the formation of gaps. Here the mucosa and muscularis mucosae can protrude through the bowel wall. The muscular spasm is responsible for incarceration of the diverticula. Inflammatory diverticulitis is compared to appendicitis.

[Long-term cardiac arrest by cardioplegic coronary perfusion (author's transl)].

UNLABELLED: The Mg++ aspartate-procaine-cardioplegia has been proven in animal experiments as well as from 1970 til 1975 in more than 1000 open-heart-procedures by a myocardial temperature of 32 degrees C and aortic crossclamping time up to 40 minutes superior to all other known procedures of cardiac preservations. To guarantee a safe myocardial protection of the arrested heart for a remarkable longer period of total ischemia, we further developed the cardioplegic technique in the animal lab, and use it now clinically. PRINCIPLE: The arrest is induced by cardioplegia (Mg++ aspartate-procaine), than the arrested heart is cooled down to 15-20 degrees C by cardioplegic coronary perfusion maintaining the oxidative metabolism. The perfusion is stopped. The begin of ischemia is still under normal ATP-levels and continuous cardioplegia. TECHNIQUE: 1. Crossclamping of the aorta; 2. cardioplegic induced cardiac arrest by Mg++ aspartate-procaine (Kirsch); 3. Surface cooling of the heart; 4. Coronary perfusion by hypothermic cardioplegic solution (8-10 min, flow 80-120 ml/min, perfusion pressure maximal 30 mmHg). Perfusate: O2-saturated, erythrocyte free, 6% hydroxyethyl starch solution added 2 mM Mg++ aspartate, 4 mM procaine, 50 mM Na+, 5 mM K+, 0,5 mM Ca++, 25 mM HCO3-, 10 mM glucose, 200 mM mannitol, 250 mg/l 6-methylprednisolone. RESULTS: 84 patients (29 ACVB; 55 valve replacements); crossclamping time: 71 min (SD 22); total time of ischemia: 57 min (SD 18; max 96, min. 27 min); reperfusion time restoring normal excitation-contraction of the heart: 3 min (SD 2); weaning off bypass: 23 min (SD 14). Hemodynamic 12 h postop.: SO2 venous 76% (SD 6). No sympathicomimetics were used. Only 30% of myocardial ATP is splitted after 120 min of cardiac arrest. Electron microscopic findings show only small, reversible alterations of fine structure.

[Peculiarities of myelin formation in the spinal roots of the rabbit].

The axon sheath formation in the ventral and dorsal spinal roots of newborn rabbits is discussed. The mesaxon grows at a greater rate than the outer plasmalemma of the Schwann cell, thereby giving rise to folds in the mesaxon. The compact myelin spiral is formed by the apposition of 2-3 lamellae.

[Postnatal development of the rat retina under DNS-inhibitor (chloroquinediphosphate) mediation (author's transl)].

The morphogenesis of retina has been studied in 1-15 day-old rats after a single injection of chloroquinediphosphate. Beside degeneration of neuroblasts, rate of development was markedly retarded as evidence by regression of the strata reticulares. Neuroblasts or receptor cells remained disorganized and were arranged in rosettes. In the inner layer containing the third neuron myelinated bodies were produced by the drug.

On some aspects of schwann-sheath development and the possible role of their desmosome-like structures.

An investigation was made of ventral and dorsal spinal roots (L7 and S1) in the postnatal life of the rabbit. The development of the Schwann sheath of the nerve fibers was studied and correlated with the distribution and morphological characteristics of the desmosome-like structures connected with them. The association of these structures with constituents of the endoplasmic granular and smooth-surfaced reticulum of the Schwann cell is pointed out. The possibility is discussed of a direct relationship between the inner mesaxons, as well as along the length of the peripheral myelin lamellae. In addition, it is suggested that in some cases already existing membrane formations of the adjacent cytoplasm become attached to the developing Schwann sheaths, which results in branching mesaxons or "poli-mesaxonal" nerve fibers.

[Metabolism and ultrastructure of magnesium aspartate-procaine arrested hearts of rabbit and man (author's transl)].

In normothermia, mild, and deep hypothermia the metabolism and the electron microscopic structure were investigated in human and rabbit heart muscle after magnesium aspartate-procaine cardioplegia. In comparison to plain ischaemic arrest splitting of adenine nucleotides and glycogen was significantly reduced in all experiments with the induced cardioplegic arrest. For 40 min at 32 degrees C almost no changes in ultrastructure were seen in heart muscle after induced arrest, while severe and/or irreversible damages were seen in the cell structure of the heart muscle due to plain ischaemic arrest.
