[Analysis of residues of the fungicide cymoxanil in grapes using multicolumn HPLC].

Residues of the contact fungicide Cymoxanil in grapes were determined by a multidimensional multicolumn high pressure liquid chromatography technique (MC-HPLC). The sample pretreatment is a simple water extraction. Further clean-up and trace enrichment of an aliquot of the aqueous acidic sample solution is performed on-line via an automatized microprocessor controlled MC-HPLC system. In the range of 0.05-2.0 mg/kg grapes good linearity is achieved. Recoveries of Cymoxanil added to untreated grapes range between 70% and 80% at 0.05 and 2.0 mg/kg values, respectively, with a reproductibility of s +/- 3% at 0.2 mg/kg (n = 5). The total time for analysis is approximately 30 min.

[Focal epithelial hyperplasia in lepromatous leprosy].

Focal epithelial hyperplasia Heck (FEH) is most likely caused by human papilloma virus. It mainly occurs in children and young people showing no associated diseases. For the first time, we describe a case of FEH in a patient with lepromatous leprosy who due to persistent erythema nodosum leprosum has been treated with a lang-term glucocorticoid therapy. The question of the competence of lepromatous patients in resisting certain viral infections arises.

Histiocytosis-X in gynecology.

(1) Histiocytosis-X can manifest itself in virtually every organ, but in gynecology it is an absolute curiosity. (2) Differential diagnosis must exclude specific and nonspecific ulcerations and granulations such as syphilis, tuberculosis, Boeck's disease, and also neoplastic processes like lymphomas, sarcomas, carcinomas, and malignant diseases of the hemopoietic system. (3) The diagnosis by light microscopy alone, as in our case, may be insufficient; therefore, electron microscopy should be used. As soon as the diagnosis is confirmed histologically, an extensive examination of all organs is necessary in order to establish an exact prognosis and an optimal plan of therapy. (4) Because of the unknown etiology of histiocytosis-X, a causal treatment is not yet possible. In spite of this, with a symptomatic, individualized therapy by means of excision, low-dose irradiation and cytotoxic agents a 5-year survival of 90% was obtained for the patients. (5) Because of its rarity and multidisciplinary character, histiocytosis-X is a challenge to interdisciplinary and interregional cooperation. Though not being a malignoma in the strict sense, diagnosis, therapy, and in part prognosis are not essentially different from a malignant disease.

[Congenital arteriovenous fistula with an angioma. Therapeutic possibilities].

Arteriovenous fistulas associated with an angioma are characterized by a relatively fast growth. Therapy consists in operative removal of the entire angioma. This often means partial amputation of extremities. An embolization of the angioma, as in an observed patient can prevent further progression. A disfiguring operation can be delayed or possibly even avoided.

[Post-thrombotic axillary vein syndrome (Paget-von Schroetter syndrome)].

In acute thrombosis of the subclavian and axillar vein--known in the literature as Paget-von-Schroetter syndrome--skin can also be involved if no treatment is provided. The skin lesions resemble stasis dermatitis seen in chronic venous insufficiency of the lower extremity. The specific role of intima lesions of the subclavian vein caused by microtraumas is discussed because the Paget-von-Schroetter-syndrome is mainly seen in physically hard working men.

[Excessive allergy due to benzoic acid followed by anaphylactic shock. (author's transl)].

The case of a trained chemical worker highly sensitive to benzoic acid is described. He suffered from allergic reactions of increasing intensity while being constantly exposed to benzoic acid during work. After oral exposure to benzoic acid he suffered a severe anaphylactic shock and later on showed similar but milder reactions when eating food containing benzoic acid. No allergic reactions reoccurred avoiding every contact with benzoic acid especially observing a strict benzoic acid-free diet.

[Myoglobin increase during tocolysis (author's transl)].

The serum myoglobin level was performed before as well as two and four hours following the beginning of intravenous tocolysis in 22 pregnant patients who were treated with the betasympathicomimetic agent, hexoprenalin because of premature labour. In six of the patients a rise of 26--140% in myoglobin was observed in comparison with the basal level. One of the patients even produced a twenty-fold rise. The value of serum myoglobin determination in the diagnosis of myocardial necroses is discussed with respect to the literature and the presented material.

[Cutaneous histiocytosis X].

Histiocytosis X comprises three clinical entities whose common substrate is a localized or systemic proliferation of atypical histiocytes. On the basis of the age of manifestation, acuity of the clinical course and organ involvement Abt-Letterer-Siwe's disease, Hand-Schüller-Christian's disease and eosinophilic granuloma can be differentiated from each other, although transitional varieties of these syndromes are possible. Not infrequently oligosymptomatic forms are misinterpreted, especially when the skin is the only involved organ. In the following case report cutaneous histiocytosis X will be discussed in terms of its clinical expression. Electron-microscopy has proved to be the best methods to make the diagnosis of such atypical cases.

[Does myocardial damage arise as a result of administration of tocolytic drugs? (author's transl)].

Fifty pregnant women admitted consecutively on account of threatened abortion or premature labour and treated with Ritodrine and Verapamil intravenously in the usual dosage were investigated with respect to creatine kinase activity and, in the event of a raised level, its isoenzyme creatine kinase--MB. Determinations were carried out before treatment and, in accordance with expected peak values in case of myocardial necrosis, after 6 and 12 hours. Three pathological values were recorded and these are discussed individually. Furthermore, statistical evaluation of the recorded values showed no significant differences.

[Determination of the heart minute volume by impedance cardiography: a way to prophylaxis of premature birth? (author's transl)].

30 gravid women between the ages of 18 and 38 years who had gone into premature labour of unknown etiology were examined with respect to cardiac output and various other cardiovascular parameters in the standing, supine and left lateral positions. Measurements were carried out using an impedance-cardiography. A group of 30 women of similar age and duration of pregnancy but not in premature labour was also tested as controls. In the standing position, the test group showed a cardiac output which was 15% less than that of the controls. Statistically this was highly significant (p less than 0.005). No difference was found in the supine and left lateral positions. These results are in agreement with the literature, but with the method used in this study, an invasive, or X-Ray procedure is avoided. The physiology of the cardiovascular system in pregnancy and the means by which premature labour may be prevented, are discussed.
