[Moulages from the Würzburger University Dermatologic Clinic as an example of a German collection--historical development and didactic value].

Dermatological wax moulages were often used as clinical teaching models well into the 1930s. Their didactic possibilities and their historical development in the German-speaking area are described. The extensive collection of such moulages in Würzburg is currently being reviewed and is presented here as an example of a moulage collection typical for those formerly used in this country. Several moulages by wax modellers whose work is represented in the collection are illustrated in detail.

[Intrapartum fetal death].

Among the 18,394 children, who have been born at the Obstetric Department of the University of Innsbruck, Austria, from 1976 to 1984, 16 intrapartum deaths have been observed. Eight of these who have been vaginally delivered showed anomalies of presentation, three were anencephali, the others were singularities. There was an impressive improvement in the course of the years, but also in comparison with international results. In bur eyes, the consequence of this study is the avoiding of preterm births, an increased use of the caesarean in preterm infants with anomalies of presentation and the intensive observation intrapartum combined with a human background.

[Obstetric aspects of radiation exposure following the Chernobyl accident].

Radioactivity was determined in the milk from 39 patients and in amniotic fluid, urine and placentas from 17 patients shortly after the Chernobyl disaster by means of a beta and gamma-scintillation counter. Elevated radioactivity (7.6 nCi/l beta activity and 7.4 nCi/l gamma activity) was found only in one sample of breast milk.

[Adenoid cystic sweat gland cancer].

Two primary adenoid cystic carcinomas of the eccrine sweat glands of the skin are presented. This tumor, first described in recent years, represents the rarest form of carcinoma of the eccrine sweat gland. The tumor demonstrates characteristic histological features and clinically often exhibits slow progression and absence of lymph-node metastases. The most important differential diagnosis to be excluded is metastasis of an adenocarcinoma of an internal organ.

Sweet's syndrome associated with Yersinia enterocolitica infection.

Acute febrile neutrophilic dermatosis (Sweet's syndrome) was diagnosed on the basis of typical clinical and histological features in a 45-year-old woman. Gastrointestinal symptoms had preceded the dermatosis, and an intestinal infection with Yersinia enterocolitica could be proven by culture and serology. This first report on an association between Yersinia infection and Sweet's syndrome widens the spectrum of both infectious diseases possibly inducing acute febrile neutrophilic dermatosis and dermatological manifestations of yersiniosis.

[Metastasizing soft tissue tumor of light damaged skin: atypical fibroxanthoma or malignant fibrous histiocytoma?].

A 86-year-old female patient presented with a ca. walnut-sized, centrally ulcerated preauricular tumor on the right side as well as a firm submandibular lymph node. Histology disclosed a soft-tissue tumor breaking into the lymph node consisting partly of fibroblast-like and partly of histiocyte-like cells. Clinical and histological criteria speak for both an atypical fibroxanthoma (AFX) as well as a malignant fibrous histiocytoma (MFH). A differentiation into two entities does not appear appropriate in our opinion. As the term "AFX" is classified as a benign tumor, "metastasizing AFX" (mAFX) should be considered a special form of MFH or be diagnosed as such, respectively.

Correlation of oestriol levels in saliva, plasma and urine of pregnant women.

Unconjugated and total oestriol (E3) have been determined in saliva (S) and plasma (P) by radioimmunoassay using a highly specific antiserum against E3. Total oestrogens (E = E1 + E2 + E3) in urine were analysed by the Kober-Ittrich method. No correlation (P greater than 0.05) was found between unconjugated E3 (S) and total E (U) as well as total E3 (S) and total E (U). Total E3 (S) and unconjugated E3 (P), total E3 (S) and total E3 (P) correlated weakly (P less than 0.05 and greater than 0.01). The six remaining correlations were highly significant (P less than 0.001 or less than 0.0001).

[Hand-foot-mouth disease].

A case of hand-foot-and-mouth disease is reported in a 20-year-old female patient. Infection by Coxsackie A 16-virus was diagnosed by serology.

[Eccrine spiradenoma].

Eccrine spiradenomas are rare and usually painful tumours, but most textbooks and reviews contain no photographs of typical cases. Therefore we report on this tumour and document our report with clinical photographs.

[Stimulation of ovulation by follicular puncture].

We report on a patient with long-standing sterility, who became pregnant only after laparoscopic puncture of the follicle in spite of all possible diagnostic and therapeutic procedures being applied. The methods so far used for stimulation of ovulation are discussed and the puncture of the ovary is proposed as new possibility in the treatment of sterility.

Epidermodysplasia verruciformis in lepromatous leprosy. Report of 2 cases.

2 African patients (sisters) suffering from lepromatous leprosy had epidermodysplasia verruciformis. Both patients had in addition recalcitrant superficial fungal infections and extensive relapsing scabies. The question is raised whether a generalized impairment of cell-mediated immunity, found in lepromatous leprosy, predisposes towards other particularly viral infections.

[Analysis of residues of the fungicide cymoxanil in grapes using multicolumn HPLC].

Residues of the contact fungicide Cymoxanil in grapes were determined by a multidimensional multicolumn high pressure liquid chromatography technique (MC-HPLC). The sample pretreatment is a simple water extraction. Further clean-up and trace enrichment of an aliquot of the aqueous acidic sample solution is performed on-line via an automatized microprocessor controlled MC-HPLC system. In the range of 0.05-2.0 mg/kg grapes good linearity is achieved. Recoveries of Cymoxanil added to untreated grapes range between 70% and 80% at 0.05 and 2.0 mg/kg values, respectively, with a reproductibility of s +/- 3% at 0.2 mg/kg (n = 5). The total time for analysis is approximately 30 min.

[Focal epithelial hyperplasia in lepromatous leprosy].

Focal epithelial hyperplasia Heck (FEH) is most likely caused by human papilloma virus. It mainly occurs in children and young people showing no associated diseases. For the first time, we describe a case of FEH in a patient with lepromatous leprosy who due to persistent erythema nodosum leprosum has been treated with a lang-term glucocorticoid therapy. The question of the competence of lepromatous patients in resisting certain viral infections arises.

Histiocytosis-X in gynecology.

(1) Histiocytosis-X can manifest itself in virtually every organ, but in gynecology it is an absolute curiosity. (2) Differential diagnosis must exclude specific and nonspecific ulcerations and granulations such as syphilis, tuberculosis, Boeck's disease, and also neoplastic processes like lymphomas, sarcomas, carcinomas, and malignant diseases of the hemopoietic system. (3) The diagnosis by light microscopy alone, as in our case, may be insufficient; therefore, electron microscopy should be used. As soon as the diagnosis is confirmed histologically, an extensive examination of all organs is necessary in order to establish an exact prognosis and an optimal plan of therapy. (4) Because of the unknown etiology of histiocytosis-X, a causal treatment is not yet possible. In spite of this, with a symptomatic, individualized therapy by means of excision, low-dose irradiation and cytotoxic agents a 5-year survival of 90% was obtained for the patients. (5) Because of its rarity and multidisciplinary character, histiocytosis-X is a challenge to interdisciplinary and interregional cooperation. Though not being a malignoma in the strict sense, diagnosis, therapy, and in part prognosis are not essentially different from a malignant disease.
