[Intussusception in the newborn (author's transl)].

A five day old male neonate had bloody stools and decreasing appetite. He was subjected to laparotomy after x-ray investigations had shown an intussusception. This was due to a cystic duplication of the ileum just before the ileocecal valve. After an ileocecal resection the postoperative course was uneventful. Special problems of the intussusception in the newborn are discussed.

[Ultrastructural changes of the lungs in experimental burn shock].

The alteration of the ultrastructure of lungs was investigated in the course of the early shock period in the experimental burn disease. The histologic investigations of the lungs were carried out with the Philips 300 electron microscope. A stronger scaling of the surfactant of alveoli was observed in the first days after the burn. Number and size of the myelin structures in the pneumocytes typ II increased simultaneously. The walls of the capillaries were not injured. An edema of th capillary endothelium was observed frequently. The most extensive alterations of ultra structure of lungs appeared in the course of the burn disease on the first or the second day after the thermic injure.

[Tuberculous stenosis of the transverse colon].

A 14 year old Turkish girl presented with unspecific abdominal symptoms and barium enema showed an almost complete obstruction in the region of the splenic flexure. The picture was that of a neoplasm and a left hemicolectomy was performed. Histological examination revealed ulcerating hypertrophic tuberculosis of the colon. The incidence, pathology and symptomatology of colonic tuberculosis are discussed.

[Small bowel stenoses as a consequence of intrauterine transfusion (author's transl)].

An infant is reported who, because of Rhesus incompatibility, required 4 antenatal blood transfusions. At the last of these transfusions performed during the 30th week of pregnancy, the catheter could not be removed and was left in the foetus's abdomen. Two weeks later, spontaneous delivery of an 1820 gm. male infant resulted. He had to be operated on on the 2nd day of life because of small bowel atresia, which was caused by two severe intestinal strictures which had been produced by the catheter. Following removal of the catheter and resection of the strictures, the post-operative course was essentially uneventful.

[Treatment of meconium ileus equivalent].

A 12-year old boy with mucoviscidosis developed a mechanical obstruction of the small intestine. The diagnosis of a meconium ileus equivalent was made and a conservative treatment was started. After this treatment failed, a laparotomy was performed which verified the diagnosis of a meconium ileus equivalent. It was possible to move the inspissated feces inside the terminal ileum into the ascending colon. The postoperative course was uneventful. The etiology, differential diagnosis, and both conservative and operative treatment of this complication of the mucoviscidosis are discussed.

[Lower limb varicose veins in textile workers].

441 women working in various textile industry plants were examined to detect possible varicose veins, ulceration of shins, post thrombotic syndrome and active thrombotically inflammable changes. Frequency of the above mentioned changes was evaluated withthe following criteria: age, duration of employment in textile industry and work-post. Additionally, sickness absenteeism analysis was done in patients treated preventively and by operation. Investigations carried out indicate that the frequency of varicose veins in lower limbs, of trophic changes, shin ulceration, thrombotically inflamed processes increases with age and duration of employment in textile industry, resulting in considerable sickness absenteeism. The surgeon working in a given plant should detect varicose veins in lower limbs as early as possible and send his patients for an operation. Operation prevents many complications and shortens duration of disablement for work.

[Polyorchidism].

Polyorchidism is a rare anomaly of the testicles. Only about 60 cases have been described so far. A four-year-old boy who was admitted because of a maldescent of the left testicle was found at operation to have a third, atrophic testicle, which was removed. The various forms of polyorchidism, their embryological origins, other malformations and therapeutic measures are discussed.

Fetal and adult liver stem cells for liver regeneration and tissue engineering.

For the development of innovative cell-based liver directed therapies, e.g. liver tissue engineering, the use of stem cells might be very attractive to overcome the limitation of donor liver tissue. Liver specific differentiation of embryonic, fetal or adult stem cells is currently under investigation. Different types of fetal liver (stem) cells during development were identified, and their advantageous growth potential and bipotential differentiation capacity were shown. However, ethical and legal issues have to be addressed before using fetal cells. Use of adult stem cells is clinically established, e.g. transplantation of hematopoietic stem cells. Other bone marrow derived liver stem cells might be mesenchymal stem cells (MSC). However, the transdifferentiation potential is still in question due to the observation of cellular fusion in several in vivo experiments. In vitro experiments revealed a crucial role of the environment (e.g. growth factors and extracellular matrix) for specific differentiation of stem cells. Co-cultured liver cells also seemed to be important for hepatic gene expression of MSC. For successful liver cell transplantation, a novel approach of tissue engineering by orthotopic transplantation of gel-immobilized cells could be promising, providing optimal environment for the injected cells. Moreover, an orthotopic tissue engineering approach using bipotential stem cells could lead to a repopulation of the recipients liver with healthy liver and biliary cells, thus providing both hepatic functions and biliary excretion. Future studies have to investigate, which stem cell and environmental conditions would be most suitable for the use of stem cells for liver regeneration or tissue engineering approaches.

Heterotopic pancreas--clinical presentation and pathology with review of the literature.

BACKGROUND/AIMS: Heterotopic pancreas is usually a silent gastrointestinal malformation, but it may become clinically evident when complicated by chronic inflammation or by growth. METHODOLOGY: We report on eleven cases of heterotopic pancreatic tissue. The cases were selected from the records of our Surgical Department and Institute of Pathology. The literature about heterotopic pancreas is reviewed. RESULTS: Nausea and vomiting (27%), epigastric pain (27%), ulceration (27%) and weight loss (18%) were the three most frequent symptoms and signs. The lesions were diagnosed as gastrointestinal tumor or ulcer by gastroduodenoscopy (36%). The other patients were diagnosed during surgery (64%). Definitive diagnosis was only achievable by pathology. Heterotopic pancreas was the reason for surgery in 36% of the cases, in another 45% diagnosis was incidental during surgery and in 18% the diagnosis was established endoscopically and surgery was not necessary. CONCLUSIONS: The diagnosis of heterotopic pancreas is rarely established, most cases remain clinically silent. In symptomatic patients diagnosis should to be secured histologically to exclude malignant disease.

[Perioperative blood transfusion in patients with malignant neoplasms].

Within 1986-1988, 3,358 patients were treated at the Department of Surgery, Military Teaching Hospital in Bydgoszcz. Hundred twenty six out of these patients were operated for the malignant tumors. Indications to blood transfusion in case of patients with cancer were analysed retrospectively. In our patients about 50% whom blood was given were justified cases. It was found that the analysis of indications based on laboratory findings may decrease the amount of blood given patients who underwent surgery for cancer.
