[EEG pattern during treatment of parkinsonism with L-dopa].

The authors studied the dynamics of EEG changes in 27 patients treated with L-dopa. The records obtained before treatment were compared with those made during treatment and with the clinical manifestations. In 8 cases the EEG pattern was normal before treatment and 6 of them improved (three-fourths of this subgroup). In 19 cases the EEG pattern was abnormal and 10 of them improved (about 50%). Some intensification of temporal episodic complexes in EEG records was observed during treatment without concomitant changes of clinical manifestations. In 2 cases with slowed down background activity in initial records psychic disturbances of hallucinatory obnubilation type developed requiring withdrawal of treatment. This psychotic reaction was characterized in EEG with increased intensity of generalized slowing down of EEG activity.

[Use of 24-hour casette monitoring of the EEG recording in the differential diagnosis of attacks of unconsciousness in adults].

24-hour cassette recording of EEG using the Medilog 9000 system was done in 36 patients referred for observations because of diagnostic difficulties in cases of refractors unconsciousness attacks. The method made possible isolation of 3 groups: 1. 6 patients with attacks not suggestive of epilepsy 2. 12 patients with false unconsciousness attacks which were in fact short partial complex seizures (temporal lobe absence) 3. 17 patients with actual unconsciousness attacks. The study demonstrated a high usefulness of 24-hour EEG monitoring in the differential diagnosis of unconsciousness attacks increasing the likelihood of attack recording and making possible their precise clinical and EEG characteristics which is often impossible with routine EEG and history taking.

[Diagnostic value of 24-hour simultaneous EEG and ECG monitoring of patients with heart diseases and atypical consciousness disorders].

In 24 patients with diagnostically not clear, short, recurrent episodes of consciousness disturbances and heart diseases and/or a history of arrhythmia simultaneous 24-hour recording was done of eeg and ecg. In the differential diagnosis epilepsy was considered, especially since in most cases routine eeg records demonstrated slight episodic changes. During 24-hour recording in 8 cases typical episodes of consciousness disturbances developed but in none of them these episodes were associated with arrhythmia which ruled out their cardiogenic origin. In 2 cases EEG recording served for establishing the diagnosis of partial complex seizures, 2 patients had hyperventilation syncope, one had TIA, in the remaining 3 cases absence of eeg and ecg changes during these episodes and coexistence of anxiety neurosis suggested functional origin. So the combined 24-hour eeg+ecg recording made possible establishing of diagnosis in 1/3 of these patients, enabling adequate treatment to be instituted.

[History of discoveries and advances in the studies of parkinsonism].

A review is presented of the more important discoveries and advances in the 175-year history of the studies and therapeutic trials on parkinsonism. The personality and achievements of James Parkinson are described, with a brief discussion on his most important paper "An Essay on Shaking Palsy". The important works of clinicians, neuropathologists and pharmacologists in the 19th and 20th centuries, especially in the "era of L-DOPA", are reviewed. The development of neurosurgical methods of parkinsonism treatment, including intracerebral grafting of dopamine-producing cells, is outlined.

[Epilepsy and other convulsive states in long-term EEG monitoring].

The authors report the results of EEG monitoring lasting many hours with cassette recording and with interpretation after reproduction on monitor screen. The Medilog 9000 System Oxford was used for that purpose. The group comprised 32 verified cases of epilepsy and 19 cases of seizures with unestablished aetiology. In epileptics the method made possible determination of the actual frequency of seizures and their clinico electroencephalographic qualification. In the remaining group of 18 patients the epileptic character of their seizures was demonstrated in 3 cases, in 9 cases the mechanism of these seizures varied, sometimes they were neurotic vasovagal. In 6 cases the monitoring failed to provide sufficient data. These results demonstrate a highly usefulness of cassette EEG monitoring in neurological practice, especially in case of seizures. A valuable supplementation of these investigations are records monitored by television, which make possible establishing of a more close correlation between clinical symptoms and EEG findings.

[Evaluation of the biological availability of 2 carbamazepine preparations].

The biological availability was determined of two carbamazepine preparations: Amizepin POLFA and Tegretol GEIGY in tablets. The study was carried out in 10 patients giving them the drug in doses of 400 mg by the crossover method at an interval of 3 weeks. The statistical analysis of the mean maximal values, mean times of reaching of maximal concentrations, and mean areas under the curve of serum carbamazepine concentration changes for both forms of the drug showed a comparable biological availability of Amizepin and Tegretol. The relative degree of bioavailability of Amizepin amounted to 98% of that of Tegretol. The values of pharmacokinetic parameters of both drugs were compared. The value of distribution volume was greater in the case of Tegretol, and the difference was statistically significant. No significant differences were found between the remaining calculated parameters.

[Role of the parathyroid glands in the pathogenesis of intracerebral calcifications].

Investigations of calcium-phosphate metabolism were carried out in a group of 11 children and adults with intracerebral calcifications. It was possible to isolate three different pathogenetic types of calcifications in the striatum and dentate nuclei in the cerebellum. The authors suggest restriction of the term "Fahr's syndrome" to cases without true of false hypoparathyroidism. The assessment of the calcium-phosphate metabolism, and particularly, the test with parathyroid hormone, seem to be an indispensable element in the differential diagnosis of this type of intracerebral calcifications.

[Characteristics of the clinical features and pharmacological treatment of epileptics with frequent seizures].

The analysis of clinical features of groups of patients with frequent and rare epileptic seizures showed that the mean age of the first seizure, the mean age at the time of examination, and the mean duration of the disease were not significantly different in both studied groups. In the group with frequent seizures a significantly more frequent presence of features indicating serious organic brain damage was found (more frequent occurrence of known aetiological factors, more cases of posttraumatic epilepsy, psychic and neurological abnormalities). In group C changes in the background activity in EEG were also more frequent. The analysis of pharmacological treatment showed a more frequent application of polytherapy in group C. On the other hand, the mean levels of antiepileptic drugs in the serum and the mean doses of these drugs for kg of body mass were not significantly different in both groups. In about 40% of patients in both groups the prescribed drugs failed to reach the levels generally regarded as therapeutic (slightly more frequently in the group with rare seizures). This was true mainly of PHT since about 81% of patients in both groups failed to reach the therapeutic level of the drug (10 ug/ml). The likely causes of this fact were: too low doses prescribed (55% of the patients were given doses below the generally recommended), lower bioavailability of PHT (27% of patients had no therapeutic level of the drug in the serum, despite doses exceeding 5 mg/kg); or drug interaction. This requires elucidation in further studies.

[Effect of the method of treatment on mean survival time of patients with glial brain tumors].

In the years 1976-1981 54 patients were treated for malignant glial tumours confirmed by histological examination. The purpose of the reported analysis was a comparison of the mean survival time and the mean time of survival without evidence of recurrence in 3 groups of patients. In group I of 13 patients not treated surgically but receiving only conservative management the mean survival time was 7.4 weeks. In group II treated surgically but without other therapy this time was 29.2 weeks. In group III in which the patients were operated on and treated with chemotherapy and radiotherapy this time was 58.9 weeks. The differences between the groups were statistically significant. Similarly, the mean survival time without recurrence was significantly longer in the group receiving comprehensive treatment than in the group treated only surgically (46.5 vs 18.0 weeks).

[1st hospital admissions and deaths from cerebrovascular diseases in the Warsaw population].

On the basis of an analysis of documentation from hospital the annual rate of first admissions for cerebrovascular disturbances was assessed in the population of the city district of Warsaw. The total rate was 123/100 000, including 22 for subarachnoid and cerebral haemorrhage. The rate was highest in the age group over 70 years (1162/100 000). The annual average death rate caused by these disease was 94/100 000. In the first month after admission 41% of patients died (including 77% with the diagnosis of cerebral haemorrhage) and during 6 months 52% died. The diagnosis of cerebrovascular disturbances was mentioned in only about 75% of death certificates of patients dying within one month from admission. In the Wrasaw population the annual rate of first admission and average annual death rate were on the average 1.5 times higher than the mean values for the whole country. Difficulties with the use of medical documentation for epidemiological studies are discussed.
