Isolation and identification of spirorenone metabolites from the monkey (Macaca fascicularis).

The plasma level of spirorenone was determined 3 h after 1, 8, 22 and 46 daily oral administrations of 20 mg/kg to two female and two male monkeys (Macaca fascicularis). A fifth animal, female, was treated with eight daily doses of tritium-labelled drug and was completely bled from the carotid vein 4 h after the last administration in order to isolate and identify plasma metabolites. After repeated daily doses of spirorenone the mean plasma level of unchanged drug was 711 +/- 213 ng/ml. In the plasma of the fifth animal four radioactively labelled compounds could be detected after extraction and subsequent HPLC separation. Mass spectrometric identification of three of the substances indicated 1,2-dihydrospirorenone, hydroxy-1,2-dihydrospirorenone and the unchanged drug itself.

Penile flow index and its role in classification of erectile dysfunction: a preliminary report.

A method is described to confirm vascular disorders in sexual impotence by a simple noninvasive screening technique. Doppler ultrasound signals are registered from superficial dorsal penile arteries and arteria radialis. The penile flow index (PFI) is determined by dividing the mean values of both measurements. An insufficient vascular support of the penis is assumed if the amplitude of the dorsal penile arteries is less than one third of that of the a. radialis. Identical Doppler signals are registered in superficial dorsal and deep cavernosal arteries.

[Endocrine findings during hormonal treatment of metastasizing prostatic carcinoma].

Endocrine findings of hypophyseal, testicular and adrenal function in the hormonal treatment of advanced prostatic carcinoma are described. Diethylstilbestrol (DES) reduces significantly the plasma levels of testosterone, follicle-stimulating hormone (FSH) and luteinizing hormone (LH). In contrast, those of prolactin, dehydoepiandrosterone (DHEA), and cortisol as well as sexual hormone binding globulin (SHBG) are elevated. The additional administration of bromocriptine decreases prolactin to normal levels and further decreases the testosterone levels. The adrenal steroids are influenced differently: DHEA decreases as compared to the DES-treated group, while the cortisol levels rise significantly. The addition of cyproterone acetate to the therapeutic regimen decreases the cortisol levels, and all other values, including SHBG, are not altered significantly compared to the group treated with DES plus bromocriptine. What is of interest is that the estradiol levels are not altered by any treatment schedule. In a group of patients, orchiectomized prior to our study, the endocrine values are compared to those mentioned above. Following orchiectomy FSH and LH levels rise significantly, but not prolactin and SHBG. The testosterone levels were as low as under treatment with DES. From our results it is concluded that the combination of DES plus bromocriptine plus cyproterone acetate best achieves the inhibition of androgenic influence onto the prostatic carcinoma.

[The "robotron NATALI" microcomputer-assisted birth monitoring system].

The microcomputer system "robotron-NATALI" from VEB Kombinat robotron Dresden, GDR, is applicable for bed side-monitoring and disposed about a new graphical representation of CTG's for fetal condition and uterine activity. A dialogue between physician or nurse and the computer is possible. After parturition all administered data are printed out in form of a partogram, inclusive the graphical representations during labor.

[Intrapartum tcpO2 measurement (author's transl)].

The value of intrapartum tcpO2 measurement is discussed, with reference being made to clinical examples. Also mentioned are certain setbacks of the method which, at present, still are limiting factors to general clinical intrapartum use.

[Ejaculation inadequacy in sexual disorders].

Sixteen men with missing ejaculation were observed as outpatients in our department within the past years. Among these, four patients with retrograde ejaculation are included. Possible reasons for the missing ejaculation are operations in the genital region or the pelvis, and spinal cord injuries. Endocrine disorders, diabetes mellitus, drug dependence, and psychogenic alterations have to be discussed as etiologic factors. Secondary lack of ejaculation, which is acquired in later years of life, seems to have a poorer prognosis than the primary disease, which begins with puberty. This group of patients is well responsive to psychotherapy. Other therapeutic approaches are hormonal substitution, care of drug dependence, and treatment with sympathicomimetica in some cases of retrograde ejaculation.

Determination of plasma and urine levels of a new anti-inflammatory agent, 4,5-bis-(4-methoxyphenyl)-2-(2-hydroxymethylsulfinyl)-imidazole, by high-performance liquid chromatography and electrochemical or ultraviolet detection.

The determination of a new anti-inflammatory substance, 4,5-bis-(4-methoxyphenyl)-2-(2-hydroxyethylsulfinyl)-imidazole, in plasma and urine by high-performance liquid chromatography is described. Ultraviolet and electrochemical detection modes are compared and special consideration is given to the mechanism of the electrochemical reaction. The site of oxidation in the molecule seems to be an aliphatic hydroxyl group yielding a carboxyl function in a four-electron transfer reaction. Plasma levels and urinary excretion after an oral dose of 250 mg to two male volunteers have been measured.

Influence of LRH on gonadotrophin and testosterone levels in the foetal male rat.

Gonadotrophin and testosterone levels were measured in male foetal rats at the 21st day of gestation 30 min following administration of LRH. The releasing hormone was administered by 3 different routes: subcutaneously as a single injection to the pregnant rat; subcutaneously as 12 times repeated injection to the pregnant rat from 16th-21st day of gestation; direct injection into the amniotic cavity. Three different doses of 1, 5, and 10 microgram/kg were used. The single administration to the dam was without effect. Repeated doses resulted in a significant dose-dependent elevation of mean foetal LH: 163 ng/ml in the control animals and 133,256, and 363 ng/ml in the 1, 5, and 10 microgram/kg LRH group resp. Intra animal injections significantly increased FSH and LH levels, but only those of FSH were clearly dose-dependent. A mean of 390 ng/ml in the control group was observed, with 1 microgram/kg LRH FSH was elevated to 723 ng/ml, at 5 microgram/kg to 928 and at 10 microgram/kg to 1017 ng/ml. Testosterone levels were not significantly altered. Our results demonstrate that the pituitary of foetal rats is able to respond to LRH in the same manner as adult animals.

[Histological, electron microscopical and X-ray microanalytical findings in malakoplakia of the kidney (author's transl)].

Malakoplakia which was found in the kidney if a man aged 55 years is described. This rare granulomatous lesion was extirpated together with a renal cyst. So-called Hansemann-cells with inclusion bodies and ring structures were found by light and electronmicroscopic examinations. These concentrically arranged ring like structures were not typical Michaelis-Gutmann-bodies, and they consisted of microfibrils and small granula. Using X-ray microprobe analysis could be detected calcium. These findings were considered to be an early stage of malakoplakia.

Effect of glucagon on human gastric mucosa: histochemical studies.

We studied the effects of glucagon (2 mg intravenously) on the histochemical localization and staining intensity of 10 enzymes in human gastric mucosa. Glucagon caused a significant increase in the histochemical activity of glucose-6-phosphate dehydrogenase in the undifferentiated neck cells and mucous cells of the foveolae and of ATPase activity in and around the mucosal capillary walls. Glucagon also stimulated mucus secretion from the surface epithelial cells. These changes were observed 15 and 30 min after glucagon in the oxyntic, but no pyloric mucosa. they indicate that glucagon, in addition to its effect on parietal cells, also affects other structures in human gastric mucosa.

[Determination of carcinoembryonal antigens in the blood of bullous and genetically determined skin diseases].

It was reported that increased levels of carcinoembryonic antigen (CEA) in blood were observed in patients with epidermolysis bullosa. We tested a group of total 82 patients with bullous dermatoses (bullous pemphigoid, pemphigus, epidermolysis bullosa, dermatitis herpetiformis), vesicular dermatoses (herpes simplex und zoster) as well as hereditary dermatoses (psoriasis, atopic dermatitis, dyskeratosis follicularis). The CEA levels were all within the range of healthy control persons (10.3-42.1 ng/ml). Our patients do not support the suggestion that certain dermatoses cause an increased production of CEA-like substances.

[A new form of representation of foetal cardiotocograms (author's transl)].

Based on a computerized analysis of CTG's we have developed a new graphical representation of fetal CTG's. The reproduction results from displays over 13 hours labor-progressing. A mathematical model is used. We have applied a discrimination function (DF) which is composed by 10 quantitative electronic parameters. Two fixed periods are compared. For mathematical and statistical evaluation the multivariate analysis of variance in connection with methods of dimension-reduction and discrimination was applied. A nonelementary discrimination function was taken into account. The value of the discrimination function was checked by means of postnatal data (pHNA and APGAR-score 5 minutes postnatal). The predictability "normal" pHNA - related to the computerized CTG- analysis - was exact in 80% (pHNA greater than or equal to 7,20). In 20% we found prepathological pHNA-values (pHNA:7,19-7,11). The predictability "prepathological" pHNA - related to the computerized CTG- analysis - was in 3,5% correlated with pathological pHNA-values (pHNA less than or equal to 7,10). Pathological pHNA - values may be almost ruled out by computerized CTG - analysis. Computerized "normal" CTG's were in 97% correlated with APGAR-scored values (5 minutes postnatal) > 7. Computerized "prepathological" or "pathological" CTG's were in 26% correlated with APGAR-score values (5 minutes postnatal) less than or equal to 7. The proposed computer-program is already used in this form in clinical practice.

[Behavior of phosphatases and carcino-embryonal antigens in serum following specific immunotherapy of metastasizing prostatic cancers].

The efficacy of specific immunotherapy was examined in patients suffering from metastatic prostatic cancer. For this purpose determination of prostatic acid phosphatase and carcino-embryonic antigen in serum was performed using conventional enzymatic and radioimmunological techniques before and after therapy. The results were as following: 1. A significant decrease of the above mentioned parameters was demonstrable after therapy. 2. The determination of prostatic acid phosphatase was helpful for immediate control of therapy, the determination of carcino-embryonic antigen was more helpful in follow-up studies. 3. Contrary to disappointing relevance of radioimmunological determined prostatic phosphatase in early prostatic cancer this parameter seemed to be more suitable in follow-up studies than conventional determined phosphatase.
