[Antibodies to D-penicillamine in patients with chronic polyarthritis--diagnostic aid in undesired and insufficient drug effects?].

Antibodies to D-penicillamine (DPA) were determined in 25 patients with classic rheumatoid arthritis being treated with DPA. Positive results were found before and during therapy with DPA. Antibodies were found in seven out of ten patients without the desired therapeutic effects; in five cases before starting the therapy. It is hypothesized that antibodies against drugs are heterogeneous and influence the effect of the drugs, e.g. inhibition of the desired effect and stimulation of an undesired (adverse) reaction.

Diclofenac and piroxicam in a double-blind trial results and methodological problems. Working Party of the Society for Rheumatology of the GDR for the Testing of Anti-rheumatic Drugs.

Diclofenac (150 mg/d) and piroxicam (20 mg/d) were compared in a cross-over trial with 12-day periods of double-blind treatment and 3-day wash-out periods. The following open phase of the trial lasted for two months and served to assess the tolerance. For this the dosage of diclofenac was reduced to 100 mg/d. 134 volunteers from 5 departments of rheumatology participated in the study. Statistical analysis showed that the two groups were of homogenous composition and that--with certain limitations--the necessary assumptions for the evaluation as a cross-over experiment were fulfilled. All tests used to determine the efficacy showed approximately parallel improvement with both formulations and in the wash-out phase this was followed by marked deterioration. The sum of the parameters for individual improvement showed a tendency in favour of piroxicam. However, clear superiority of one trial formulation over the other with respect to efficacy or tolerance could not be demonstrated by the statistical analysis.

[Pathogenesis of gouty arthritis].

The urate crystal stands in the centre of the pathogenesis of gouty arthritis. External influences, such as changes of temperature and mechanical loads as well as factors, which lead to a short term change of the level of uric acid in the serum, such as alcohol, meals rich in purine, uricosurics and saluretics are, indeed, able to induce an attack of gout, but they are no obligatory prerequisite. The granulocytes are to be regarded as the second important prerequisite of the crystal arthritis. They become effective either via the process of phagocytosis or their cell membrane produces prostaglandins, the formation of which is evoked by electrostatic forces of the crystals. Mediators, such as kinins, Hageman's factor and complement are mainly responsible for the initial pain reaction, vasodilation and leukotaxis, lysosomal enzymes evoke the secondary changes at the joint.

[Factors affecting the localization of joints in rheumatoid arthritis].

For testing a possible connection between the localization of a joint and anthropometric data in rheumatoid arthritis the numbers of a maximum and minimum affection in the region of the hand were at first determined. Former findings of a more frequent participation of the right hand could be confirmed. In the region of the distal skeleton of the hand the changes were to be found more radially, in the proximal parts more frequently ulnarly. Examinations concerning the influence of the breadth, length and angular measures of the hand showed above all connections of the affection of the joints with the breadth of the hand. An influence of measures and indices of the body on the frequency of the affection of peripheral joints could statistically ascertained only for few of the constellations examined. Future investigations shall take into consideration the regional vessel and nerve supply, shall issue from homogeneous groups of test persons and shall be performed with other methods of evaluation.
