Epidemiological features of Adamantiades-Behçet's disease in Germany and in Europe.

The German Registry of Adamantiades-Behçet's disease was founded in 1990 in Berlin and it provides current data on the epidemiology, the clinical manifestations and the course of the disease in Germany on a continuous basis. A total of 218 patients, including 89 German and 100 Turkish patients, had been reported to the German Registry until October 1997. One hundred and ninety-six patients fulfilled the criteria of the Behçet's disease classification tree. The prevalence of the disease evaluated in Berlin-West was 1.68/100,000 in 1989 and had risen to 2.26/100,000 by 1994. The median age of onset was 25 years (range 5 to 66 years; German-Turks, ns). Juvenile disease was recorded in 6.9% of patients. The complete clinical picture according to the criteria of the International Study Group of Behçet's Disease developed in 15.5 months. The interval between onset of the disease and diagnosis was 35 months, which was significantly longer than the duration of the development of the complete clinical picture (p < 0.0001). The disease was diagnosed later in German (48.5 months) than in Turkish patients (25.5 months, p = 0.003). While German patients presented an equal male-to-female ratio, a male predominance was shown in Turkish patients (M:F 2.1:1, p = 0.022). Familial occurrence was detected in 2.0% of German and 15.9% of Turkish patients (p = 0.013). The frequencies of major clinical manifestations were: oral ulcers 99%, skin lesions 76%, genital ulcers 75%, ocular manifestations 59%, arthritis 59%, and positive pathergy test 52%. Clinical differences between German and Turkish patients were only found in the frequency of ocular lesions (48% vs. 66%, p = 0.025). Oral ulcers were with 72% the most common onset symptom of the disease followed by erythema nodosum (9%), uveitis (7%), arthritis (7%), genital ulcers (3%), superficial thrombophlebitis (2%) and papules/sterile pustules (2%). Uveitis and erythema nodosum as onset symptoms shortened the median interval to diagnosis to 1.5 and 15 months, respectively, while arthritis delayed diagnosis (43.5 months; p = 0.029). A severe course developed in 25% of the patients; irreversible retinal vasculitis to blindness in 15%, sterile meningoencephalitis in 8%, severe arthritis in 5%, hemoptysis in 2%, lethal outcome in 2% and bowel perforation in 1%. The relative risk of HLA-B5 positive German natives developing the disease. HLA-B5 was confirmed as a marker of severe prognosis. Cardiolipin autoantibodies were associated with cutaneous vasculitis and superficial thrombophlebitis was correlated with systemic vessel involvement.

[Backache from the internal medicine-rheumatologic viewpoint].

In only 30% of back pain patients an underlying pathology can be found. Rheumatologic causes in a narrow sense are fibromyalgia, osteoporosis and the group of spondylathropathies and reactive arthritis. Infectious disorders of the spine are emergency cases and need immediate and interdisciplinary action. Careful evaluation of signs and symptoms indicate the suspected origin of pain and lead to the use of more specialized diagnostic means. Therapy of specific back pain should be appropriate to the clinical disorders. In acute, nonspecific back pain, the aim is to prevent a chronification of disease by instruction and education of the patient and an early start of physical therapy. The rehabilitation process in chronic cases in complex and may need psychobehavioral methods for pain control. Pharmacologic modalities of treatment-simple analgesics, nonsteroidal antirheumatic drugs, muscle relaxants and antidepressants-should only be used for a limited period and monitored constantly.

[Routine hip ultrasonography in inflammatory rheumatoid diseases. Results in 119 consecutive patients of a rheumatism clinic].

METHOD: In 119 consecutive inpatients with rheumatic diseases--mainly rheumatoid arthritis, ankylosing spondylitis and psoriatic arthritis--both hips were routinely investigated by sonography. RESULTS: In 52 of the 119 patients definite or possible pathological findings (in 18 cases bilateral) were observed. The relative frequency of sonographic findings in 223 evaluable hips was 33% and was somewhat higher in patients with rheumatoid arthritis than in those with other diagnoses. Changes were found significantly more often when the patient had joint complaints, limitation of movement, pathological X-ray changes or serological signs of inflammation (elevated ESR, positive C-reactive protein). The relative frequencies were higher than were expected on the basis of data in the literature. Of the pathological sonographic findings, 22% occurred in clinically and radiologically asymptomatic patients. CONCLUSIONS: In view of the therapeutic consequences and the importance of early treatment, routine sonographic examination of the hip joint is recommended.

Comparison of US inactivated split-virus and Russian live attenuated, cold-adapted trivalent influenza vaccines in Russian schoolchildren.

In a blinded, placebo-controlled study, the reactogenicity, immunogenicity, and clinical efficacy of single doses of US inactivated split-virus and Russian live attenuated, cold-adapted influenza vaccines were compared in 555 schoolchildren in Vologda, Russia. Serial serum samples were collected and school absenteeism was assessed. Systemic reactions were rare, but local reactions (primarily erythema at the injection site) were observed in 27% of the inactivated vaccine group, and coryza (12%) and sore throat (8%) were observed in the attenuated vaccine group. At 4 weeks after vaccination a > or = 4-fold rise in titer of hemagglutination inhibition antibody to A (H1N1), A (H3N2), and B was noted, respectively, among 78%, 88%, and 53% of children who received inactivated vaccine and among 55%, 79%, and 30% of children who received attenuated vaccine. The vaccine efficacy for preventing school absenteeism due to respiratory illness during the period of peak influenza activity was 56% for inactivated vaccine and 47% for attenuated vaccine.

[Current status of basic antirheumatic therapy].

Basic treatment of inflammatory rheumatic diseases is effective, but not necessarily remission-inducing. Its mechanisms of action are only partly understood, and the reasons for the variations in response rate of between 40 and 80% are also unknown. The original concept of starting with the least toxic of these substances, and then only after the failure of treatment with non-steroidal anti-inflammatory drugs, has now been abandoned. Taking compliance as a measure of therapeutic value, among the most important of the substances employed, methotrexate is the most, oral gold the least, favorable, while azathioprine, sulfasalazine, antimalarial agents and D-penicillamine do not differ significantly from another.

[Clinical relevance of the antiphospholipid syndrome. Antiphospholipid antibodies in a mixed rheumatologic patient sample].

The antiphospholipid syndrome may occur either in association with systemic lupus erythematosus (SLE) or as a primary condition. Diagnostic criteria include the detection of antiphospholipid antibodies using cardiolipin antigen (anticardiolipin antibodies, ACA) or lupus anticoagulant, and at least two clinical manifestations, primarily venous or arterial thrombosis, thrombocytopenia or repeated miscarriage. Among 200 patients with various rheumatic diseases, a pathological ACA test in excess of 44 U/ml was detected with ELISA in 81 cases (41%), and of these 30% had values exceeding 100 U/ml. Both overall positive reactions and the particularly high titers were clearly more common among patients with rheumatoid arthritis (RA) and collagen disease, namely 51% and 33%, respectively, than in patients with sero-negative spondylarthritis and non-inflammatory diseases of the locomotory system (22% pathological, 13% highly pathological, titers. Correlations were also found with miscarriage in women, positive rheumatoid and antinuclear factors, and the number of diagnostic criteria of RA or SLE presenting. A questionnaire survey - evaluating 84 subjects, half with positive and half with negative, ACA reactions - confirmed the correlation between miscarriage and the detection of ACA.

Pilot evaluation of influenza virus vaccine (IVV) combined with adjuvant.

The safety of licensed influenza virus vaccine (IVV) combined with a novel adjuvant containing muramyl tripeptide (MTP) conjugated to phosphatidylethanolamine (PE) was evaluated in a randomized pilot study. Ten healthy 23-30-year-old men were given a single intramuscular dose of IVV combined with saline (n = 5) or with 100 micrograms of MTP-PE in the MF59 adjuvant emulsion (MF59-100) (n = 5). Evaluations were performed on days 0, 1, 2, 4, 7 and 28 after inoculation. IVV alone was well tolerated. All volunteers immunized with IVV/MF59-100 experienced moderate to severe local and systemic reactions which interfered with usual activities. Discomfort at the injection site was first noted at 2-6 h; induration (5/5), erythema (3/5), and regional adenopathy (3/5) persisted for up to 4 days. Systemic symptoms including chills (5/5), fever (3/5), nausea (3/5) and/or dizziness (2/5) developed within 12 h of inoculation and resolved by 48 h. Elevated white blood cell count (days 1 and 2), erythrocyte sedimentation rate and serum fibrinogen were transiently observed. Although peak serum neutralizing antibody titres versus influenza A/H3N2 and influenza B antigens were higher in the group given IVV with MF59-100, these unexpected reactions indicate that this dose of adjuvant is unsuitable for use in combination with this IVV.

[Basic therapy of rheumatoid arthritis. Comparison of methotrexate and sulfasalazine].

In a retrospective analysis a total of 167 patients with rheumatoid arthritis (RA) who had been treated with the basic therapy formulas of methotrexate (MTX) or sulfasalazine (SUL) were evaluated with respect to therapeutical result and side effects after a period of 12 months. There was no randomization as to either MTX (n = 87) or SUL therapy (n = 80), but deliberate use of therapy according to prior treatment and activity of the illness. Apart from a significantly higher number of inflammatory joint conditions in the MTX group there was no difference in the two patient groups at the beginning of the study. MTX treatment led clearly to a more conspicuous activity decrease of the illness (ESR, joint index) and a more favourable effect on the locomotor function. Furthermore, the sphygmomanometer readings (for grip strength determination), the dose reduction of the concomitant prednisolone medication as well as the doctor's opinion were in favour of MTX. The portion of patients who had to discontinue the therapy because of side effects (= 11%) and lack of effect (= 9%) was exactly identical in both groups of patients. Both substances have shown to be effective basic therapy formulas for rheumatoid arthritis, while methotrexate has a few advantages.

[The HLA pattern in Adamantiades-Behçet's disease in Germany. Association of occurrence, clinical symptoms and follow-up in 39 patients].

The frequencies of HLA-class I antigens were studied in 39 patients with Adamantiades-Behçet's disease and were compared with those in 1415 [corrected] healthy controls; both patients and controls were of German origin. Moreover, the correlations of HLA antigens with onset, various clinical features and the course of the disease were investigated and the familial clustering of the disease was examined. We found an increased frequency of the HLA-B5 antigen in these series (P < 0.05, relative risk 2.6, confidence interval 1.2-5.5). The presence of HLA-B5 was more frequently detected in male (41%, relative risk 4.9, confidence interval 1.8-13.1) than in female patients (9%; P < 0.05). A significantly higher frequency of HLA-B5 was found in male patients with severe vascular involvement, including blindness and thrombosis, than in HLA-B5-positive male patients without these features. Incidences of genital ulcers, ocular involvement, cutaneous features, positive pathergy test and of secondary symptoms as well as the age of onset and the duration of development of the complete clinical picture were all found not to be significantly correlated with any of the HLA-A, -B, -C alleles. Familial clustering was also not found in German patients. Because of the low relative risk in the presence of HLA-B5 and the lacking familial clustering, it seems likely that there is a weak immunogenetically determined predisposition for Adamantiades-Behçet's disease in Germany. HLA-B5-positive male persons of German origin show a higher relative risk to develop or acquire Adamantiades-Behçet's disease than female ones.(ABSTRACT TRUNCATED AT 250 WORDS)

[Behçet's syndrome. Possible differential diagnosis in multi-system symptoms].

Behçet's syndrome was described as a triad with recurrent ulcers of the mouth, aphthae of the genitals as well as changes of the eyes. A fourth later added main criterion comprises multifarious dermatological symptoms. Facultative signs of a disease, which, however, occasionally may stand also in the centre of the clinical picture are vascular, gastrointestinal and central-nervous participations, arthritic pains and swellings as well as an epididymitis. On account of the infrequency of the disease outside the endemic regions and the development of the individual manifestations over larger periods it is often recognized very late. Out of the six casuistics described three of them fulfilled the criteria of a complete Behçet's syndrome with evidence of all four main criteria, in the remaining casuistics the incomplete clinical picture was present with three main and one to four side criteria. In all cases the course was relatively benign. Recently, streptococcal antigens were considered to be etiological factors; correlations with the HLA-system are ascertained for patients from endemic regions. In phases of clinical activity the combination of immunosuppressive agents with cortisonoids is the therapy of choice.

Effect of live attenuated, cold recombinant (CR) influenza virus vaccines on pulmonary function in healthy and asthmatic adults.

The effects of intranasal inoculation with live attenuated, CR influenza virus vaccines on pulmonary function in healthy and asthmatic adults were evaluated in placebo-controlled, double-blind studies. In 46 healthy adult volunteers, there were no statistically significant alterations in pulmonary function as measured by spirometry and histamine bronchoprovocation tests in the first week following monovalent CR influenza virus vaccine [type A (H3N2, H1N1) and type B]. Among healthy adults with pre-inoculation PC20s less than 10 mg ml-1, 8/12 were infected following vaccination but no significant alterations occurred in histamine bronchoprovocation. In 11 asthmatic adults, no statistically significant alterations in pulmonary function, as measured by spirometry, were noted during the first 7 days postinoculation with bivalent CR influenza virus vaccine type A (H3N2 and H1N1). Postinoculation respiratory illnesses were more common in CR influenza virus vaccine recipients than placebo recipients, but they were mild, consisting of afebrile pharyngitis and transient rhinorrhea. Attenuated CR influenza virus vaccines do not appear to impair pulmonary function during the first week following immunization of healthy and asthmatic adults.

[The Saxon test for objective assessment of xerostomia. A contribution to the diagnosis of Sjögren's syndrome].

In 100 patients with rheumatoid arthritis and in a control group of 100 test persons with healthy joints who were exactly equal concerning age and sex with the help of a simple method (back-weighing of a sponge after defined retention time in the oral cavity) the secretion of the saliva was measured. According to the test variant in 9-10 patients and 0-1 controls, respectively, pathologic values were found. In comparison to other symptoms which are characteristic for the presence of a Sjogren syndrome, an average sensitivity and high specifity could be ascribed to the test for the evidence of a functional disturbance of the salivary glands.

Immunoassay for serologic diagnosis of influenza type A using recombinant DNA produced nucleoprotein antigen and monoclonal antibody to human IgG.

Influenza type A nucleoprotein (NP) derived from the full length cloned gene expressed in E. coli was evaluated in a solid phase enzyme immunoassay (EIA) for detection of human antibody to influenza. Monoclonal antibody to human IgG was used for detection. Direct and indirect assays were developed and sera were tested in serial and single dilution formats. Preliminary results indicated that recombinant-and virion-derived NP antigens were comparable in binding ability to plastic and binding human antibody. Eighty-seven paired sera from influenza patients were tested. The most sensitive assay (indirect-serial dilution) detected 56 (64%) rises and the simplest assay (direct-single dilution) detected 43 (49%) rises, compared to 36 (41%) for complement fixation. Paired sera from 18 control patients showed no evidence of antibody rises by any of the assays. Forty-nine paired sera from influenza B infected patients were negative for antibody rises except for one borderline rise by the indirect-serial dilution assay. These results indicate that the use of recombinant DNA derived nucleoprotein for immunoassay is feasible. The sensitivity of immunoassays using NP adsorbed to the solid phase and monoclonal antibody specific for human IgG to detect bound antibody exceeded that of conventional complement fixation testing for establishing serologic evidence of influenza type A infection.
