Environmental measurements were made in three poultry confinement buildings in order to characterize gas and particulate contaminants. Levels of total and respirable dust averaged 4.4 and 0.24 mg/m3, respectively. Particle size distribution as measured by cascade impactors was similar in the three confinement houses with a mass median aerodynamic diameter of about 15 microns and a geometric standard deviation of about 2.2. Ammonia levels measured in the active areas of the buildings averaged about 25 ppm. Ammonia concentration was quite high, however, in an unused and unventilated portion of one of the buildings (mean = 170 ppm). CO2 levels ranged from 0.05-0.1%. Levels of CO, H2S, NO2, NOx, CH4, mercaptan, formaldehyde, and hydrocarbons were all below the limit of detection for indicator tubes. Concentrations of airborne bacteria and fungi were on average about 1.5 X 10(5) and 1.0 X 10(4) colony-forming units/m3, respectively. Endotoxin analysis was also performed on the total and respirable dust samples. Endotoxin levels (expressed in air concentration) ranged from 0.77 to 61 ng/m3 for total dust and from 0.71 to 15 ng/m3 for respirable dust. Endotoxin was also measured on the collection media from the individual impactor stages. Endotoxin was detected in all size ranges with the highest concentration of endotoxin per unit of dust found in the smallest (less than approximately 3.5 microns) size fraction. The endotoxin levels tend to be lower than those previously reported in poultry operations.

A cross-sectional study of 5 NaCl mines and 259 miners addressed the following questions: 1) Is there an association of increased respiratory symptoms, radiographic findings, and reduced pulmonary function with exposure to nitrogen dioxide (NO2) and/or respirable particulate (RP) among these miners? 2) Is there increased morbidity of these miners compared to other working populations? Personal samples of NO2 and respirable particulate for jobs in each mine were used to estimate cumulative exposure. NO2 is used as a surrogate measure of diesel exposure. Cough was associated with age and smoking, dyspnea with age; neither symptom was associated with exposure (years worked, estimated cumulative NO2 or RP exposure). Phlegm was associated with age, smoking, and exposure. Reduced pulmonary function (FVC, FEV1, peak, flow, FEF50, FEF75) showed no association with exposure. There was one case of small rounded and one case of small irregular opacities; pneumoconiosis was not analyzed further. Compared to underground coal miners, above ground coal miners, potash miners, and nonmining workers, the study population after adjustment for age and smoking generally showed no increased prevalence of cough, phlegm, dyspnea, or obstruction (FEV1/FVC less than 0.7). Obstruction in younger salt miners and phlegm in older salt miners was elevated compared to nonmining workers. Mean predicted pulmonary function was reduced 2-4% for FEV1 and FVC, 7-13% for FEF50, and 18-22% for FEF75 below all comparison populations.

In this article the utilization of medical services by Indochinese refugees is examined. A representative sample of the refugee population in one resettlement site provides basic data on health service utilization behaviors. The analysis focuses on the situational and attitudinal determinants of hospital contact, physician contact, and the volume of utilization of both services. The findings indicate that length of residence and ethnicity are important predictors of utilization behaviors. The most recently arrived refugees and the Hmong are low utilizers of physician and hospital services. A regression analysis of situational and attitudinal variables shows that effects of length of residence and ethnicity do not diminish when the effects of the other variables are considered. Although the results are similar to previous utilization studies, they do suggest that Indochinese refugees require more information about United States health services to utilize them properly.

To describe structures and biological functions of targets for antibody-mediated immune responses to Epstein-Barr virus (EBV)-infected lymphoid cells, we have characterized a membrane-associated protein of 105,000 daltons, p105, which was prominently recognized in immunoprecipitates with some EBV antigen-reactive patients' sera. A rabbit antiserum to immunopurified p105 was developed. [35S]methionine-labeled p105 was specific to EBV-superinfected Raji cells, and its synthesis was not blocked with phosphonoacetic acid, indicating that it is an "early" viral antigen. Phosphonoacetic acid treatment of EBV-superinfected Raji cells was associated with the accumulation, mainly in the cytosol, of an 88,000-dalton protein, p88, which was also recognized with anti-p105 serum, but was not detected in superinfected cells which had not been treated with phosphonoacetic acid. Although anti-p105 serum immunoprecipitated a membrane fraction protein, it did not neutralize P3HR-1 virus and was not considered to be an exposed virion component. We conclude that p105 is an early, EBV-induced, membrane fraction antigen to which EBV-infected patients generate a substantial antibody response.
The prevalence of cholesterol gall stones in young women has increased since the introduction of oral contraceptives. The synthetic female sex hormones used in these preparations, increase the degree of cholesterol saturation in bile. To determine whether oestrogens, progestagens, or both, are responsible for the change in biliary cholesterol saturation index, a prospective randomised, controlled study was performed. A significant increase in the cholesterol saturation index of bile was observed when either 30 micrograms ethinyloestradiol plus 150 micrograms norgestrel (p = 0.01) or 50 micrograms ethinyloestradiol plus 250 micrograms norgestrel (p less than 0.01) were ingested daily for two months. No change in the cholesterol saturation index was observed when 30 micrograms ethinyloestradiol alone, or 30 micrograms ethinyloestradiol plus 2.5 mg norethisterone were used. The mechanism for the increase in cholesterol saturation index did not appear to involve bile acid metabolism. These results indicate that the progestagen, norgestrel, and not as previously thought the oestrogen, ethinyloestradiol, is responsible for the increase in cholesterol saturation of bile which accompanies the use of oral contraceptives.

This paper examines the health care status of blacks in the American health care system and points out that blacks are burdened by a number of health inequities when compared to their white counterparts. The paper's central theme is that the degree of governmental commitment in a liberal, pluralistic society is at the foundation of inadequate health care for black Americans. Blacks lack input in the health care politics and decision/policy-making processes. This lack of input has resulted in a health care system that appears to be unresponsive to the health care needs of black Americans. This is most acute in the areas of health manpower planning and health planning. The conclusion suggests that an increase in the number of blacks in the health professions along with more black participation in health decision/policy-making could lead to a substantial improvement in the overall health care of blacks.

Selected cervical and thoracic intervertebral discs were removed from twelve human adults at autopsy. The discs were prepared by the paraffin method, stained with orcein or by Verhoff's technique and examined with the light microscope for the presence of elastic fibres. Elastic fibres were located only in the lamellae of the anulus fibrosus and the superficial zones of the nucleus pulposus from regions of intervertebral disc that connected or interfaced with osseous vertebrae. Elastic fibres were longitudinally, circularly and obliquely arranged in lamellae of the anulus fibrosus and penetrated bony vertebrae as Sharpey's fibres. Elastic fibres assumed a three dimensional lattice work in the superficial zone of the nucleus pulposus and connected it to the hyaline cartilage end plate as Sharpey's fibres. Irregularly arranged elastic fibres interconnected the superficial zone of the nucleus pulposus and lamellae of the anulus fibrosus. Elastic fibres were a major histological component of the ligamenta flava and anterior longitudinal ligaments.

Myocardial injury was studied in 104 patients undergoing coronary artery grafting without cold chemical cardioplegia using the quantity of the isoenzyme MB of the creatine kinase liberated as an indicator. This method of evaluation, which is said to permit comparison of different techniques of myocardial protection, allowed us to consider the relative importance of several factors believed to have an influence on intraoperative myocardial injury. Indices of significance were duration of symptoms before operation, presence of chronic arterial hypertension, and the type of antiangina treatment employed. Other operative factors included severity of the arterial lesions, number of anastomoses performed, and duration of extracorporeal circulation and of aortic cross-clamping.
