Midazolam sedation for outpatient fibreoptic endoscopy: evaluation of alfentanil supplementation.

Alfentanil, a short-acting opioid, was used as an adjuvant to midazolam for sedation of 30 outpatients undergoing upper gastrointestinal endoscopy. The operating conditions and recovery times were compared with those of a similar group of 30 patients sedated with midazolam only. The use of alfentanil resulted in improved operating conditions and a more rapid recovery. Patient acceptance was high.

Cyclosporin alters the induction of allospecific tolerance in vivo.

CBA mice were made hyporesponsive to A/J alloantigens by either neonatal inoculation of (CBA X A)F1 hybrid lymphoid cells or by intravenous injection of adult mice with A/J bone marrow cells. Specific alloreactivity was assessed in vitro by induction of anti-A/J cytotoxic T lymphocytes (CTL) or in vivo by skin graft rejection. Cyclosporin A given at the same time as the tolerance-inducing regimen of F1 (or parental) lymphoid cells abolished the hyporesponsiveness normally induced by these injections.

Coronary thrombolysis by intravenous infusion of recombinant single chain urokinase-type plasminogen activator or recombinant urokinase in baboons: effect on regional blood flow, infarct size and hemostasis.

An occlusive thrombus was produced by thrombin-induced coagulation in the left anterior descending coronary artery of 18 open chest baboons. In six control animals, occlusive thrombosis persisting for 4 hours resulted in a large transmural infarct (66 +/- 4% of the perfusion area, mean +/- SEM). In six animals, single chain urokinase-type plasminogen activator, obtained by recombinant deoxyribonucleic acid (DNA) technology, was infused intravenously at a rate of 20 micrograms/kg per min for 60 minutes after approximately 45 minutes of coronary thrombosis. Persistent reperfusion occurred within 21 +/- 4 minutes (mean +/- SD). The mean duration of occlusion before reperfusion was 72 +/- 6 minutes. Recanalization resulted in a reduction of infarct size (42 +/- 4%, p less than 0.01 versus control animals). Myocardial blood flow in the perfusion area of the left anterior descending coronary artery was 107% of normal 2.5 hours after recanalization. The infusion of recombinant single chain urokinase-type plasminogen activator was not associated with systemic activation of the fibrinolytic system, fibrinogen breakdown or evident bleeding. In six baboons recombinant low molecular weight urokinase (molecular weight 33,000) was infused intravenously at a rate of 20 micrograms/kg per min for 60 minutes after approximately 45 minutes of coronary thrombosis. Persistent reperfusion occurred within 14 +/- 5 minutes (p less than 0.05 versus recombinant single chain urokinase-type plasminogen activator). The mean duration of occlusion was 69 +/- 14 minutes.(ABSTRACT TRUNCATED AT 250 WORDS)

Clonal generation of multipotent and unipotent hemopoietic blast cell colonies in vitro.

Colonies with a unique compact morphology and displaying high replating potential develop in methycellulose cultures when bone marrow cells from hydroxyurea- or 5-fluorouracil-treated mice are cultured in the presence of syngeneic thymocytes. These compact colonies are clonal, originating from a single unit, the compact-colony-forming unit (CFU). Based on replating analysis, at least three groups of compact colonies exist: (1) those that do not replate, (2) those that replate and give only GM colonies, and (3) those that give rise to granulocyte-macrophage (GM), erythroid (E), and mixed-erythroid (MIX/E) colonies. Colonies in the last group have a mean size of 2,060 cells and generate an average of 244 colonies in secondary cultures. Velocity sedimentation studies showed the compact-CFU to be similar in size to the MIX/E-CFU, sedimenting at rates between 3.5 and 7 mm/hour. In addition to giving rise to large numbers of in vitro CFU, compact-CFU are also able to generate day 8 spleen colony-forming units (CFU-S).

Reproducible growth in tissue culture of retinoblastoma tumor specimens.

Retinoblastoma is a unique embryonic tumor which frequently arises because of an autosomal dominantly inherited mutation. Study of the genetic changes associated with retinoblastomas requires techniques that allow proliferation of fresh tumor specimens in tissue culture. However, until the present study, there were no reported methods for routinely obtaining in vitro growth of fresh retinoblastoma tumor cells. When placed on selected fibroblast feeder layers, 65% of fresh retinoblastoma specimens grew; but in the absence of fibroblasts, the same specimens died. Individual fibroblast strains from normal donors, retinoblastoma patients, and mice were effective feeder layers. A fibroblast strain that provided good feeder function for one tumor generally supported growth of other tumors also. Direct contact was required between the tumor cells and the fibroblast; conditioned media alone did not support tumor growth and immobilization of tumor cells in semisolid media above fibroblasts failed to give colony growth. Cultured tumor cells injected into the eyes of athymic nude mice formed tumors histologically characteristic of retinoblastomas.

Organizational and client influences on psychiatric admissions.

This paper analyzes the decisions which take place at the entrance gate of the mental hospital-the organizational and client factors which influence the decision to admit. More specifically, it explores variations in the determinants between first admissions and readmissions. In the process, a theoretical model will be developed, based on a study of admissions to state hospitals, which may help explain several of the underlying and continuing obstacles confronting efforts to reduce readmissions.

Human leukocyte interferon produced by E. coli is biologically active.

A human leukocyte interferon cDNA was enzymatically synthesized, inserted into the vector pBR322, and cloned in Escherichia coli. The DNA sequence codes for a 23-amino acid signal peptide followed by an interferon polypeptide of 165 amino acids. An expression plasmid was constructed which permits the synthesis in E. coli of 2.5 x 10(8) units of interferon per litre of culture. This LeIF protected squirrel monkeys from lethal encephalomyocarditis virus infection.

Stimulation of human NK-cell activity by cultured cells. I. Response of normals.

Natural cell-mediated cytotoxicity, observed on co-cultivation of peripheral blood lymphocytes (PBL) with malignant melanoma tumor cells (M4), is described. The activity was inhibited when PBL were cultured in autologous serum. Studies using competitive inhibition with cold targets and stimulation of PBL with other tumor cell lines failed to detect specificity.
