[The effect of bone-seeking metal salts on the biodistribution of tumor-seeking heavy metal complexes].

This work attempted to overcome the problem of unwanted bone radioactivity after injection of tumour-affine heavy-metal compounds (prototype 169Yb-citrate) by pre-application of stable yttrium- and calcium-compounds into tumour-bearing mice in doses of 1 mg metal/kg body weight. The pre-application of stable yttrium and calcium resulted in a smaller bone radioactivity. The most favourable results were achieved by injecting the metal salts simultaneously at or within 5 h before the 169Yb-citrate. On the other hand a strong radioactivity increase in the RES (liver and spleen) by a factor of 2 to 4 was observed after yttrium-preapplication.

[Long term biokinetics of 169Yb following injection as citrate, hydroxyethylethylenediaminetetraacetate and nitrilotriacetate].

169Yb complexes with known biokinetics in tumour-bearing mice up to 48 h p.i. were injected into healthy mice to study the radionuclide biodistribution in various organs and tissues for 672 h after injection, in order to obtain reliable biokinetic data in an animal model, not affected by tumour-growth, as a basis for the calculation of biological half-life and dose distribution. The results demonstrated the existence of at least two components with different biological half-lives in the organs and tissues investigated. The effective half-lives of these components decreased with increasing stability of the complexes administered. The effective half-life of the fast component was a few hours and that of the slow one between about 200 and 800 h.

[The effect of Ca-diethylenetriamine pentaacetate on the bio-behavior of tumor-affine metal complexes].

The complexon CaDTPA was injected into tumour-bearing mice in concentrations of 0.05, 0.1, 0.3, and 0.6 mole/l (pH:6) 30 min after the 169Yb-injection. 100 microliters of a 0.3 M CaDTPA solution were injected at different time points (simultaneously, 2, 5, 10, 20, 30, 40 and 50 min, 1, 1.25, 1.5, 2.5 and 10 h) after 169Yb-citrate injection. The animals were killed 24 h after radionuclide administration. A strong radioactivity decrease was observable 24 h p.i. not only in blood, liver, spleen, muscle and bone but also in the tumour if CaDTPA was administered within the first 2 h after ytterbium injection. Thereafter no change in radioactivity could be achieved by DTPA. A time phase in which the Yb could be eliminated from the tissues by means of DTPA (time intervals < 2 h) was distinguishable from a time phase in which it was not attainable for DTPA (time intervals > 5 h). This indicates that the incorporation of Yb into the cells is completed after 5 h and that the metals are intracellularly bound, probably to the lysosomes. Improvements of the scintigraphic tumour detection cannot be expected from the use of complexons.

[Thyroglobulin content of the blood in cases of hanging].

Thyroglobulin (Tg) was determined in serum of heart blood from the bodies of 24 people who had died by hanging. In contrast to cases of sudden death (Tg 22.7 +/- 13.5 ng/ml) the Tg values varied over a wide range (from 12 to more than 400 ng/ml). The highest concentrations were observed in cases of incomplete suspension, e.g. hanging in a sitting or semi-reclining posture. We conclude that the duration of the death agony has an influence on the Tg level in serum. Except of diseases of the thyroid gland (carcinoma, adenoma) a high level of Tg in serum may be a helpful adjunct in the diagnosis of mechanical forces to the neck region inflicted before death.

Parametric imaging in cerebral radionuclide angiography (RNA) by planar imaging improving presentation and objectivation of cerebral blood flow.

Methods of parametric imaging of radionuclide angiography using parameters like appearance time, peak time, transit time, height of peak, arterial slope and area of inflow were developed and evaluated regarding their diagnostic meaning in 111 patients suffering from TIA or PRIND and in 30 normal persons. The meaning of these single parameters could shown depended on the specificity of the diagnostic question. Local cerebral blood flow can be estimated most favourably by parametric images of area of inflow whereas transit time is most promising as a diagnostic tool for evaluation of total cerebral blood flow classified with reference to severity of the perfusion disturbance. Appearance time is suited very well to estimation of collateral perfusion. Blood flow in great cerebral arteries could be seen well by non parametric imaging of radioactivity inflow in the brain supplying arterial vessels in the cranial floor. Applying a combination of the parametric images, the sensitivity for detection of disturbances of cerebral blood flow amounts to 0.91. A specificity of 0.88 and accuracy of 0.90 were found. The described combination of evaluation of RNA using various parameters is considered a well suited method for detection of disturbances in local and total cerebral blood flow by means of planar imaging.

[Nuclear medicine functional diagnosis of the myocardium using (123) I-heptadecanoic acid].

On the basis of the results which we obtained having performed 25 examinations on 15 patients with angina pectoris syndrome, myocardial infarctions and condition after bypass operation the preparation 123I hepatadecanoic acid produced in the Central Institute for Nuclear Research Rossendorf is very well suited for the scintigraphic demonstration of the myocardium. Moreover it is possible to recognize selectively diagnostically interesting regions with different radioactivity intake, retention and elimination of the radiopharmacon pharmacon and with the help of quantitative parameters to characterize concerning the metabolic efficiency. We consider the 123I hepatadecanoic acid suitable for the clinical use in establishing the global and regional metabolic efficiency of the myocardium. It is very well to be used for the primary diagnostics and represents an evident diagnostic remedy for the assessment of the course of myocardial diseases and the success of therapy.

[Demonstration of dual liver systems after experimental auxiliary partial liver transplantation using the alternative of sequential hepatobiliary scintigraphy].

Hepatobiliary sequential scintigraphies by 99m Tc-ROTOP-EHIDA were carried out for judging the dual liver system after experimental auxiliary transplantation of a part of the liver. A functional advantage of the host liver in contrast to the transplant was found in all cases investigated. Ischemic transplant damages could be detected in all experiments and were scintigraphically recognised at an early stage. In addition to transplants with a functioning entire parenchyma, the investigations showed transplants with functionally active parenchyma zones and such ones without any functional evidence. A quantifiable determination of the global functional capacity of the hepatobiliary system of both livers was possible and also the evidence on residual functions. The different behaviour of the kinetics of the radio-pharmacon with normal function and in case of hepatic ischemia is discussed.

[Results of functional scintigraphic studies of the hepatobiliary system following reconstruction of bile duct injuries with ring drainage splinting].

Functional-scintigraphic examinations were conducted to establish liver functionality and kinetics of extrahepatic bile flow, using 99mTc-Rotop-Ehida, in 25 patients with bilio-biliary or bilio-digestive anastomosis calibrated by ring drainage. Checks were made with indwelling drainage in place or after its removal. Liver damage was detectable in 80 per cent of all probands. The method based on nuclear medicine has proved to be more suitable for quantitative functionality assessment than clinical examinations and enzyme activity tests. With ring drainage in place, it may provide useful information complementary to X-ray presentation of bile duct conditions. After removal of ring drainage, it should be preferred to invasive methods of examination.

[Myocardial scintigraphy using omega-I-123 heptadecanoic acid (Rossendorf) in patients with coronary disease].

Scintigraphy using labelled myocardial metabolism precursors makes possible a noninvasive qualitative--and increasingly also quantitative--evaluation of global and regional myocardium, both normally and insufficiently perfused, with normal and disturbed metabolism. Among the labelled precursors, free fatty acids appear as most important, especially the omega-123I-heptadecanoic acid. The experience with the use of this myocardial metabolism precursor in 23 examinations of 11 patients is described.
