[Significance of primary radical surgery for survival following treatment of breast cancer. Problems of interpretation of treatment results of prospective, nonrandomized studies exemplified by the Southwest Germany Breast Cancer Study].

In a prospective non-randomised trial 1642 patients were included. After a mean time of 10 years a follow-up analysis was performed. The importance of the radicality of the primary operative therapy of breast cancer should be evaluated. The mean 5 years survival rate was 67.3%, the 10 year survival rate 51.6%. Tumor stage and age had a highly significant influence on the survival rate. Patients with simple mastectomy had a lower survival rate as patients with radical mastectomy. This apparent difference was caused by a different age distribution in the therapy groups. Accordingly the mean life deficiency was the lowest in the simple mastectomy group. In a multiple hazard statistical analysis tumor stage and age showed a highly significant influence on the survival rate. The intraoperative radicality classification showed a significant correlation to the prognoses of the patients. The operative therapy itself had no significant influence on the survival rate of the patients.

Genetic studies at the receptor level: investigations in human twins and experimental animals.

In receptors, as in enzymes, quantitative as well as qualitative genetic variation may exist. Studies in inbred strains of mice have shown for various receptors that the receptor density as determined by Bmax values is under genetic control. In healthy adult twins we have shown that the density of alpha-adrenoceptors on platelets is also influenced by genetic factors, since monozygotic twins were much more similar to one another than dizygotic twins. However, Bmax values are up-regulated and down-regulated by endogenous neurotransmitters and pharmacologically active agents. Thus, receptor densities are under considerable regulatory influences. Bmax values therefore reflect regulatory mechanisms rather than innate characteristics of the receptor protein. In another twin study we failed to find evidence for a genetic influence on the density of imipramine-binding sites on platelets. Since qualitative variation (polymorphism) is well known in enzymes, it may also apply to receptors. Qualitative differences in the receptor protein within one species would be of particular interest because of possible functional implications. As a first approach we examined central benzodiazepine receptors by photoaffinity labelling and sodium dodecyl sulphate-polyacrylamide gel electrophoresis. A comparison of fish, frog, chicken, mouse, rat and calf led to the detection of variation between species. Investigations in five inbred mouse and rat strains have not so far revealed genetic variation in benzodiazepine receptors. Nevertheless variation may be detectable by more sensitive methods such as peptide mapping after limited proteolysis or two-dimensional electrophoresis.

The influence of low arylsulfatase A activity on neuropsychiatric morbidity: a large-scale screening in patients.

A total of 1728 patients consecutively admitted to a neuropsychiatric hospital and 379 chronically ill inpatients were examined for activity of arylsulphatase A (ASA) in leucocytes. A further 519 healthy individuals served as controls. We did not find evidence for the involvement of low ASA activity in chronic patients. The consecutive admissions showed a slight preponderance in the lower ASA activity classes. This activity range covers persons heterozygous for ASA deficiency alleles. The data are compatible with the hypothesis that carriers of low ASA activity alleles are at a slightly higher risk for neuropsychiatric disorders.

[Functional results following conservative and surgical therapy of pathologic fractures in malignant diseases].

From 1972-1982 149 pathological fractures in 91 patients were treated. 134 of these were caused by malignant disease. With 78% metastases were the most common cause for fractures. 59% of the patients were women. The most frequent fracture localisation was the femur followed by ribs and humerus. 62% of the fractures but 100% of the leg fractures were treated operatively. The best functional results were achieved after compound osteo-synthesis. The mean survival time of the patients was 10.7 months. The one year survival rate was 24.6% and the two year survival rate was 9.2%. The best survival rates were observed in women, patients with breast cancer metastases, femur shaft- or subtrochanteric fractures. The lowest survival rate was found in male patients, patients with unknown primary tumour and femur neck fractures.

[Clinical and mammographic late results following Strömbeck breast reduction-plasties].

Report about 197 breast reductions on 100 patients which were operated by the method of Strömbeck. Indication was mostly macromasty and excessive ptosis. In the mean 740 g were removed on each side, in 18.2% there was removed more than 1000 g on one side. The more tissue was removed, the higher were local complications. The cosmetic result was good to excellent in 80%. The behaviour to the husband was in most cases not changed after the operation. Mammographic studies showed a good interpretation in 80%. After the operation all patients lost their preoperative complaints.

Benzodiazepine receptor subunits in avian brain.

Using sodium dodecyl sulfate-polyacrylamide gel electrophoresis and fluorography of the photoaffinity-labeled benzodiazepine receptor in duck brain, two equally intensive bands of relative molecular masses of 53,000 and 54,000 were observed, thus being identical to the subunit pattern previously described for chicken brain. An attempt was made to characterize further the avian subunits. Comparison of the fluorographic subunit pattern in cerebellum and telencephalon revealed a pronounced quantitative heterogeneity. Addition of ethyl beta-carboline-3-carboxylate resulted in the selective inhibition of the 53,000 subunit. These results are compared with the mammalian subunit pattern in the corresponding brain regions. The findings are discussed in view of the apparent contradiction between the existence of regional heterogeneity when photoaffinity labeling is applied and its lack when monoclonal antibodies are used.

[Compound double plate osteosynthesis in subtrochanteric pathologic fractures. A clinical and experimental study].

A subtrochanteric resection was performed in 12 human corpse femora. In an experimental study stabilisation was achieved with a compound double plate osteosynthesis (condylar and medial intramedullar plate). 18 femora served as control. The loading capacity after double plate compound osteosynthesis is similar to that of normal femora. Due to the relatively rigid osteosynthesis device the plastic deformation was reduced significantly after compound osteosynthesis. The clinical evaluation comprises 26 fractures of the subtrochanteric region (out of a total of 208 pathological fractures). In 5 cases we performed a double plate compound osteosynthesis. In all patients a good or very good functional result was observed. Ten patients were treated by simple compound osteosynthesis.

[I.v. famotidine versus i.v. ranitidine: intragastric pH behavior in surgical intensive care patients].

20 patients of a surgical intensive care unit were treated in a randomised double-blind fashion with i.v. famotidine 10 mg and i.v. ranitidine every 6-12 hours for maximally seven days. Both groups were comparable with respect to the grade of risk of stress-induced bleeding. With both H2-blockers (ranitidine and famotidine) the intragastric pH could be increased to 5.0 and more and kept constant at this level over the whole test period. The mean daily doses required were 22.3 mg for famotidine and 154.3 mg for ranitidine. Stress-induced bleeding as well as adverse reactions could not be observed in both treatment groups.

[Ranitidine and cimetidine in the prevention of stress: ulcer hemorrhage a prospective comparative multicenter study].

In a multicentre single-blind study, ranitidine was compared to cimetidine as prophylactic treatment against stress-induced upper gastrointestinal bleeding in seriously ill patients in the intensive care unit (ICU). 380 patients entered the study. 192 patients were treated with ranitidine 50 mg q.i.d. as i.v. bolus followed by 150 mg orally twice daily. 188 patients received cimetidine 400 mg q.i.d. intravenously and 1,000 mg daily orally in divided doses. Five patients in the ranitidine group (2.6%) and 12 in the cimetidine group (6.4%) developed gastrointestinal bleeding definitely or possibly due to stress lesions. This difference was not significant. The incidence of stress erosions or ulcerations developing during the study was 11.8% for the ranitidine group and 18.3% for the cimetidine group (non-significant difference). Adverse events in the ranitidine group were nausea, tachycardia or vomiting in 4 patients. 5 cimetidine-treated patients developed cholestasis, and 5 additional central nervous system problems. The high degree of efficacy of both drugs compared very favourably with the high incidence of stress ulceration and hemorrhage in similar untreated populations.

[Experimental studies of the loading capacity and deformation of pertrochanter osteotomies in extra and intramedullary osteosynthesis methods].

52 human femora were examined for stabilization behaviour of pertrochanteric osteotomies as well as the significance of the osteotomy angle by intramedullary osteosynthesis with Ender nailing and by extramedullary osteosynthesis using a 145 degrees angular plate. The maximum stress tolerance of both osteosynthesis procedures in femora with flat angle osteotomies is only marginally different, but the stress tolerance of Ender nailing is much less in the steep osteotomies. With steep osteotomy angle the plastic as well as the entire distorsion of the osteosynthesis increased considerably. The distorsion following Ender nailing compared to the 145 degrees angular plating is approximately the same. With increased osteoporosis a decrease of stability and an increase of distorsion is found. This correlation is even more pronounced by steep angular osteotomy. In some cases the exact opposite can be found, so that the maximum stress can not be concluded from the osteoporosis degree found on an x-ray.

3H-imipramine binding in human platelets: a study in normal twins.

3H-imipramine binding was determined in freshly prepared intact platelets from 17 monozygotic (MZ) and 15 dizygotic (DZ) twin pairs, all of them male, adult, drug-free, and healthy volunteers. Sixteen males served as controls for determination of intraindividual variation of binding parameters. Both MZ and DZ twin pairs exhibited high intraclass correlations of Bmax values, but DZ twins were nearly as similar as MZ pairs. Interindividual variation of binding parameters is not large enough to reveal a significant genetic control.

Epinephrine-induced platelet aggregation. A twin study.

A study in healthy adult male twin pairs (17 MZ, 15 DZ) was devised to examine the genetic influence on epinephrine-induced platelet aggregation. Intraclass correlations of for MZ, 0.43 for DZ, and 0.61 for repetitive experiments in the same subjects point to the influence of genetic factors. The antagonizing effect of phentolamine on platelet aggregation did not prove to be under genetic control. There was no significant correlation between epinephrine concentrations which induce half-maximal aggregation and number of alpha-adrenergic receptors as measured by 3H-yohimbine binding. In one MZ twin pair concordantly, and also in two unrelated persons, epinephrine was unable to induce complete platelet aggregation. This phenomenon is not related to the number of alpha-adrenergic binding sites on platelets.

Intralipid causing adult respiratory distress syndrome.

Adult respiratory distress syndrome developed in a patient after he was administered intralipid. Intralipid was implicated as the causal agent after careful assessment of the complete clinical picture. To the authors' knowledge this is the first documented case of intralipid-induced adult respiratory distress syndrome. Clinicians should be aware of the possibility of its occurrence.
