Melanoma update. Second primary melanoma.

BACKGROUND: Second primary melanoma is not a rare phenomenon. It occurs in at least 3 to 6% of melanoma patients and up to a third of individuals from melanoma-prone families. OBJECTIVE: To review the clinical and histologic features of multiple primary melanoma. A schema for follow-up of these patients is presented. CONCLUSIONS: As the incidence of melanoma continues to rise, we will be faced with an ever increasing number of patients who have survived their tumor. These patients are at risk for subsequent melanomas. This article reviews the literature regarding multiple primary melanoma, and discusses patient management.

Clinical and radiologic features, lung function and therapeutic results in pulmonary histiocytosis X.

Clinical appearance, radiologic findings, lung function and results of corticosteroid therapy were analyzed in 42 adult cases of biopsy-proven pulmonary histiocytosis X. Symptoms were present in only 64%. Using the ILO classification 1980, the evaluation of chest radiographs revealed all categories of profusion, size and shape of nodules. The characteristic 'ring figures', i.e. thin-walled cysts, in the parenchyma were detected in 78% of conventional tomographies (n = 27) and in all patients examined by CT (n = 5). In patients with early disease, lung function tests (n = 26) including body plethysmography, pulmonary diffusing capacity and ergospirometry revealed that parameters of gas exchange are most sensitive (TL,CO 84%, KCO 72%). Bronchial reactivity to carbachol was significantly higher than in controls (n = 12). Lung perfusion scintigram showed an abnormal, but uncharacteristic pattern in 81% (n = 26). During corticosteroid therapy, no progression was observed (n = 36). 85% of patients with radiographic evidence of progressive disease improved after administration of corticosteroids (n = 14).

Cloning of the polyubiquitin cDNA from the marine sponge Geodia cydonium and its preferential expression during reaggregation of cells.

Ubiquitination of proteins is a critical step in the controlled degradation process of many polypeptides. Here we show that sponges, the simplest multicellular group of eukaryotic organisms, are also equipped with the ubiquitin pathway. The polyubiquitin cDNA was isolated and characterized from the marine sponge Geodia cydonium. The open reading frame contains six ubiquitin moieties, which are lined up head to tail without spacers. A comparison of the predicted amino acid sequence of the six sponge ubiquitin-coding units with those from other organisms revealed a high degree of homology (> 93%). The ubiquitin gene is expressed to almost the same extent in the two main compartments of the sponge, the cortex and the medulla. However, only in the cortex are detectable amounts of the ubiquitin protein synthesized. The ubiquitin protein isolated from the sponge organism was found to initiate protein degradation in the heterologous reticulocyte system in the same manner as bovine ubiquitin. In vitro studies with dissociated sponge cells revealed that the homologous aggregation factor causes (i) a strong increase in the steady-state level of mRNA coding for ubiquitin and (ii) a drastic increase in ubiquitin protein synthesis, while the homologous lectin failed to display that effect in isolated cells. These data suggest that ubiquitin may play a role in sponge morphogenesis.

[The effect of praziquantel on Trichobilharzia (Digenea, Schistosomatidae), a cause of swimmer's dermatitis in humans].

Cases of swimmer's itch caused by cercariae of the genus Trichobilharzia have spread during the last years in Southwestern Germany and increased in intensity. The only consequence was to forbid swimming because no effective strategies are known which would be ecologically or financially acceptable. In this examination we therefore tried, by means of medication of ducks, the possible final hosts, to kill the different stages of Trichobilharzia in patency and prepatency in order to interrupt the parasitic cycle. For this the effectiveness of praziquantel on the parasitic stages in the final host (preadults, adults, eggs or fully developed miracidia) was tested in experimentally infected dwarf mallards (Anas platyrhynchos, hemerot.) and mallards (Anas platyrhynchos platyrhynchos). During therapy of ducks with patent Trichobilharzia infection, onefold or twofold application of this drug (1 x 500 mg/duck, 1 x 300 mg/duck, 2 x 200 mg/duck) only prevented the releasing of eggs with hatchable miracidia for up to 24 h. Only a threefold application of 200 mg/duck in 24 h intervals led to a permanent reduction of detectable miracidia. Application of praziquantel in low doses (30 or 40 mg per duck and day) did not reduce the number of released miracidia. Medication with praziquantel led to a strong shift of adult worms located in the enteric veins of the ducks to the liver veins in a little as 3 h; examinations by scanning electron microscopy revealed changes in the worms' surface and damage of the tegument. During prepatency doses of 22.5 mg praziquantel per duck and day, given continuously for one week, were sufficient to completely stop the release of miracidia. In this low dosage, a special preparation of the bitter drug was voluntarily swallowed by the ducks, thus providing in principal a simple, cheap and ecologically sound method of controlling cercarial dermatitis.

[Pseudolesions in liver sonography].

Several common hepatic ultrasound pseudolesions are presented that may closely resemble pathological conditions. The majority of these pitfalls is caused by normal anatomic structures, anatomical variants of the liver or the perihepatic regions, and benign alterations of the echotexture of the liver. Exact knowledge and a precise scanning technique by an experienced sonographer are essential to avoid mistaking such findings for true liver disease. In some instances (especially when dealing with anatomical variants) computed tomography is also helpful.

[Possibilities of error in sonography of the pancreas and spleen].

Sonography plays an essential role in imaging suspected disease in the pancreas and spleen. A variety of different factors, such as artifacts, the close anatomical relationship of the pancreas to other intestinal organs, variant anatomy, and the well-known effect of the skill and experience of the sonographer may cause diagnostic difficulties. In this article, we discuss potential pitfalls and differential diagnoses in ultrasonographic imaging of the pancreas and spleen.

Microsurgical approach to the abdominal thoracic duct in the rat: considerations in the collection of lymphocytes.

In experiments involving the collection of thoracic duct lymphocytes the anatomy of the abdominal thoracic duct in the rat has been further defined. In general, the abdominal thoracic duct lies posterior and to the left of the aorta between the renal arteries and the diaphragm. There are variations in the microsurgical approach to the classically described location of this organ that should be noted by investigators attempting to identify and dissect this structure.

Anorectal melanoma. A case report and brief review of the literature.

Anorectal melanoma in an uncommon tumor that has a poor prognosis. We present a case of anorectal melanoma and review the literature pertaining to prognosis and treatment. Although no controlled trials of treatment of anorectal melanoma have been done, the evidence suggests that conservative excision confers comparable survival with less morbidity than radical surgery.

The effect of BCG on thoracic duct lymphocytes.

Adoptive immunotherapy is becoming increasingly more important in the management of advanced malignancies. This report describes the results of immunomodulation therapy with BCG in the Dunning tumor. In addition it describes new techniques in the harvesting of lymphocytes. Thoracic duct lymphocytes from 34 rats were evaluated for the effect of the presence of the Dunning R-3327 AT-3 tumor as well as for the response to bacillus Calmette Guerin (BCG). Both tumor and BCG resulted in significant changes in the helper/suppressor T cell ratios.

[Imaging of the liver segments by ultrasound. A practical approach].

Segmentation of the liver according to Couinaud is the anatomical basis for hepatic surgery. Based upon the dichotomic branching of the portal vein and its ramifications the liver is divided into lobes (right, left and caudate): 4 double segments (DS) (right lobe: anterior and posterior DS; left lobe: medial and lateral DS); and 8 segments (S): caudate lobe (S I); lateral DS (S II and S III); medial DS (S IV); anterior DS (S VIII and S V); posterior DS (S VII and S VI). This three-dimensional concept can also be used in US using hepatic veins, the portal vein and its branches, hepatic fissures and ligaments as landmarks. The localization and extent of a tumor and its relationship to the vasculature of the liver can thus be determined and the surgical management of such patients can be improved. The exact definition of each segment of the liver by US is shown and the preoperative localizations in several cases of surgically confirmed liver tumors are presented.

[Seroepidemiologic studies of human toxocariasis].

People of Southern Germany as well as European and non European asylum applicants were examined for Toxocara-infection by means of an ELISA with excretory/secretory antigen of Toxocara canis-larvae; in default of a reference method the cut off was defined statistically. In the Stuttgart area, the average seroprevalence of healthy blood donors was 4.8%; children aged 1-7 years showed a seroprevalence of 2.1%. However the low percentage in children sera is probably not the effect of a lower risk of infection, but may result from the fact that most of the tested children live in city areas. Non-Europeans and Eastern Europeans showed a significantly higher seroprevalence (13.7% or 17.7% respectively) in comparison to the blood donors from Southern Germany. This indicates a higher risk of infection in these countries. It was possible to determine the different risk factors for a Toxocara canis-infection with some of the test-participants in Southern Germany. Compared to the low-risk-group (seroprevalence 1.9%), the test groups with risk factors showed a significant increase in seroprevalence: dog-owners 5.6%, owners of domestic cattle 9.4%, cat owners 10.9%, farmers 22.6%. In the presence of several risk factors an additive effect on the seroprevalence could be found: In the test group with one risk factor the seroprevalence (9.2%) was lower than in the test groups with two (10.8%) or three risk factors (18.6%). Considering the high prevalences found in this study and the possible dangerous complications of Toxocara canis-infections, medical and hygienic measures seem to be advisable.

[Better evaluability of post-therapeutic mammograms using a double-exposure technic. A prospective perception study].

The authors have found the results of a dual-exposure technique for the breast which proved very useful for evaluations following surgery and irradiation. The technique consists of double exposure of the treated breast on two films, one with the exposure data of the normal breast and one with two additional mAs steps. The two films demonstrate anatomical details better than conventional techniques. The over-exposed film shows 18% more reticular patterns than the standard exposure. The authors believe that the dual-exposure technique is a valuable adjunct in the management of the postoperative breast-cancer patient.

Lymphoid follicular hyperplasia in excluded colonic segments: a radiologic sign of diversion colitis.

Double-contrast barium enema (DCBE) studies showed lymphoid follicular hyperplasia (LFH) in excluded colonic segments after colostomy in 12 of 40 patients. In most patients with LFH, more than 80% of the excluded colon was affected. In eight patients, regression of LFH was demonstrated with DCBE studies after reanastomosis. This work suggests that LFH is the most common DCBE study finding in diversion colitis.

[Laser and afterloading therapy of bronchial cancer].

Endobronchial afterloading irradiation by remote control was administered to 304 patients with lung cancer. The use of a highly active 192-Iridium source (740 GBq) is possible; irradiation time lasts only a few minutes. A combined treatment modality with laser was employed in 24%, with chemotherapy in 2%. The response rate of all patients amounted to 67%. There was an improvement in 81% of patients with retrostenotic complications.
