[New antihypertensive agent of the imidazoline series: 5-fluoro-methyl-imidazolidinylidene-benzamine-monohydrochloride (flutonidin)].

A New Antihypertensive of the Imidazoline Series: 5-Fluoro-2-methyl-imidazolidinylidene-benzamine-monohydrochloride. 5-Fluoro-2-methyl-imidazolidinylidene-benzamine-hydrochloride (flutonidin, ST 600), a new antihypertensive imidazoline compound, was tested over 3 weeks in 20 patients with moderate to severe hypertension. Blood pressure was significantly lowered in both supine and upright position. In nearly 2/3 of the patients treated normalization of blood pressure was achieved. Side effects like sedation, dry mouth or orthostatic complaints were present in 30%. Toxic effects were not registered. It is recommended to reduce the dosage slowly especially in the aged to prevent, in part severe findings after abrupt withdrawal.

[The differential diagnosis of chronic bronchitis and emphysema from mutually supplemental functional and radiographic criteria].

In a retrospective study in 91 patients an attempt was made to differentiate the primary and/or predominating disorder "emphysema" or/and "chronic bronchitis" by independent evaluation of a number of X-ray signs and lung function criteria in routine diagnosis. Good congruency of the diagnosis from these two different aspects, could be established. Patients with 3 or more X-ray signs of emphysema were found to have larger lung volume and smaller diffusing capacities than those without. Vice versa, patients with functional diagnosis of predominant emphysema more often showed the full X-ray pattern of emphysema, than those with predominant "chronic bronchitis". In either method of diagnostic procedure, the occurence of the "small heart of emphysema" was obviously much more frequent in patients believed to have predominant or primary emphysema. Assessment of X-ray signs of changes of the lesser circulation can be obtained in a stage of the disease, when structural changes have become irreversible. Considering the different pathophysiologic pattern of predominant panacinar emphysema, the destruction of alveolar walls and, consequently of the capillary bed, is more likely to reduce DCO as well as cardiac output before pulmonary hypertension can develop. The X-ray signs resulting from this effect on the lesser circulation, exist in widened right hilar branch and abrupt narrowing of peripheral vessels, with a small heart and lack of prominence of the pulmonary trunk. In chronic bronchitis, however, signs of pulmonary hypertension are expected to occur early in the disease. The relevance of the X-ray changes of the heart size and the pulmonary circulation in both types of lung disease, could be confirmed by lung function data.

[Pitfalls of statistics as shown in a study on dysplastic hips (author's transl)].

In 97 cases of randomly selected children of an age from 5 to 16 weeks the angles of Hilgenreiner are determined. In relation to these findings also the influence of various factors such as hereditary anamnesis and untimely birth are observed. The so found data are compared with those published from Tönnis and Brunken. It seems that their data are higher than those in an average population. The necessity of an exact random test is pointed out.

A retrieval system for the selection and statistical analysis of clinical data.

Within the last years computer-aided clinical documentation has provided researchers and administrators with very large volumes of data for research. User-friendly retrieval tools are needed when processing these clinical databases. Clinical researchers require applications by means of which steps in selection and analysis can be performed in an iterative process. During the deduction of statistical parameters from routinely collected data a number of problems occur that do not appear in the analysis of data gathered within clinical studies. Unlike clinical studies, routine data have complex structures and must first be formatted and above all temporally synchronized. In this paper we will describe the medical retrieval system ArchiMed developed at the Vienna General Hospital. A main objective in the design of this system was to support a joint evaluation of data from clinical studies and routinely collected data. The retrieval system comprises the main functions: Selection of Patients; Selection and Joining of Variables; and Statistical Analysis.
