[Prognosis of filial generation from mothers with systemic lupus erythematosus].

Seventy three pregnancies in 44 women with systemic lupus erythematosus in our hospital from June 1970 to June 1993 were studied. It included 15 spontaneous abortions (20.55%), 8 induced abortions (10.96%), 9 perinatal fetal deaths (12.33%) and 41 live births (56.16%). Therapy with steroids during pregnancy is harmless and effective. It may reduce pregnant loss rate. Of 41 live births, 35 children were followed up (85.36%). Their physical growth, routine blood test and urinalysis were normal. There were no definite evidences of mental developmental abnormality in children. Two girls had been diagnosed of systemic lupus erythematosus at about nine years old. It is possible that systemic lupus erythematosus may be hereditable to female filial generation. So we suggest that the following-up on children born to mothers with systemic lupus erythematosus have to be done till puberty.

[Acute effects of platelet activating factor on the pulmonary circulation in rats].

To investigate the acute effects of platelet activating factor (PAF) on pulmonary circulation and cardiovascular function in rats, twenty anesthetized Wistar rats were given 100-800 ng/kg PAF by bolus intrapulmonary artery infusion. Mean pulmonary artery pressure (mPAP), mean systemic artery pressure (mSAP) and cardiac output (CO) were measured before and after administration. The results showed that PAF infusion caused: a rise in mPAP from 2.11 +/- 0.20 kPa to 2.65 +/- 0.11 kPa (100 ng/kg, P > 0.05), from 2.32 +/- 0.25 kPa to 2.85 +/- 0.24 kPa (200 ng/kg, P < 0.05) and from 2.11 +/- 0.33 kPa to 3.01 +/- 0.45 kPa (400 ng/kg, P < 0.05)p; a down in mSAP from 16.0 +/- 2.11 kPa to 9.1 +/- 2.39 kPa (100 ng/kg, P < 0.05); from 16.3 +/- 1.38 kPa to 7.0 +/- 0.57 kPa (200 ng/kg, P < 0.05) and from 16.4 +/- 2.56 kPa to 6.3 +/- 0.89 kPa (400 ng/kg, P < 0.05); and a down in CO from 48.1 +/- 2.16 ml/min to 30.8 +/- 3.27 ml/min (100 ng/kg, P < 0.05), from 47.8 +/- 2.12 ml/min to 25.6 +/- 2.86 ml/min (200 ng/kg, P < 0.05) and from 48.3 +/- 2.24 ml/min to 20.0 +/- 2.39 ml/min (400 ng/kg, P < 0.05). These findings suggested that PAF play an important role in the pathophysiologic course of pulmonary hypertension.

[The prospective research of risk factors of primary liver cancer in Nanhui County, Shanghai].

Based on a prospective study including a population size of eighty thousand in Nanhui County, the risk factors of primary liver cancer in Nanhui County were found as follows: history of hepatitis (OR 1.03, 95%CL 1.02-1.04), the length of drinking pond & ditch water (OR 1.16, 95% CL 1.02-1.32) and the length of drinking river water (OR 1.25, 95% CL 1.09-1.43). The length of drinking shallow well water (OR 0.65, 95% CL 0.59-0.73) and deep well water (including tap water, OR 0.20, 95% CL 0.16-0.25) seemed to be the protective factors of PLC. These data are useful for PLC prevention and to identify the high-risk individuals in Nanhui County.

[Observation on antibody levels of rabbits infected with single sex cercariae of Schistosoma japonicum].

Nine rabbits infected with male S. japonicum cercariae (worm recovery 15-133, mean 53.1) and 9 rabbits infected with female cercariae (worm recovery 1-102, mean 36.1) were bled periodically until 1 year post-infection. The sera were tested by ELISA, LA and CHR for antibody detection. Results showed that the positive rates were 88.9%, 55.6% and 83.3%, respectively. The duration of positive reaction was 3-51 wk post-infection for ELISA, 4-17 wk for LA and 2-35 wk for CHR and the fluctuated results were obtained in ELISA while positive CHR and LA results usually remained stable. The above-mentioned results suggest that antibody detection methods should not be neglected due to its sensitivity although antigen detection methods are considered more favorable. For the surveillance of schistosomiasis in endemic areas where schistosomiasis is under control, antibody detection methods are still worthy of recommending for case finding of new infections.

Factors related to long-term effects of thymectomy on myasthenia gravis.

The authors report the results of long-term follow-up after thymectomy on 92 cases with myasthenia gravis (MG), including 22 cases of thymomatous MG. These patients were followed up for 6-73 months with an average of 23 months. The surgically effective rate was 88.0% and the late mortality, 6.5%. The sex of the patients and the durations of disease were not obviously related to the prognosis, however the effect of thymectomy was much better in patients aged 10-39 years than in other age groups. The modified Osserman's clinical classification, acetylcholine receptor (AchR) antibodies and citric acid extract (CAE) antibodies titers in plasma and pathological patterns of the surgical specimens of thymus including immunohistochemical observation play an important role among various factors related to the long-term effects.

Radiologic manifestations of osteopetrosis.

The clinical radiologic manifestations of 8 patients with osteopetrosis (4 males and 4 females) are analyzed. Six of the cases are benign. The pathologic basis for the radiologic manifestations is discussed, and a classification is suggested. "Broom-like" metaphyses, metaphyses containing three or four dense layers of bone, and ossification of soft tissues around joints were found and differential diagnosis was made.

[Distribution of platelet activating factor in the pulmonary tissues of normal rats].

Platelet activating factor (PAF) is a biologically active phospholipid with a broad range of cellular targets and respective responses. Because lung homogenates obtained from normal rats contain measurable levels of PAF, it may be involved in both pathological and physiological processes of lung. In this study, we have searched for the distribution of PAF in the rat lungs by immunohistochemistry. We demonstrated that PAF immunoreactivity is present on pulmonary arterial and alveolar walls. The results showed that PAF are located in pulmonary arterial and alveolar wall under normal condition, and that PAF may play a role in maintaining pulmonary physiological process.

[G gamma/A gamma, A gamma I/A gamma T ratios of fetal hemoglobin of Han, Hui, Uygur and Kazak newborns in Xinjiang and gene mapping analysis of two cases with abnormal ratio].

The G gamma/A gamma, A gamma I/A gamma T ratios of fetal hemoglobin of 372 cord blood samples of newborns (Han 102, Hui 102, Uygur 99, and Kazak 69) in Urumchi were determined by HPLC. The means of G gamma ratios of 4 ethnic groups (Han, Hui, Uygur, and Kazak) were 66.83%, 68.33%, 70.44% and 69.70%, respectively. 4 cases (Hui 3 and Kazak 1) with low G gamma (< 50%), and 25 cases (Han 5, Hui 4, Uygur 11 and Kazak 5) with high G gamma (> 80%) were found. The occurrences of low G gamma in Hui and Kazak were 2.94% and 1.45% while in Han and Uygur no single case with low G gamma was found. Those of high G gamma in 4 ethnic groups were 4.9% (Han), 3.9% (Hui), 11.1% (Uygur) and 7.2% (Kazak), respectively. A gamma T heterozygotes in Han 21, Hui 23, Uygur 26 and Kazak 29 cases, and one A gamma T homozygote in each ethnic group were found. The average values of A gamma I were 56.83%, 55.58%, 50.94% and 54.68% respectively. The frequencies of A gamma T gene were 0.113, 0.123, 0.141 and 0.225, respectively. The gene arrangement of a low G gamma case was identified as-GA gamma I-/-G gamma-A gamma I-, and that of a high G gamma case as -G gamma-AG gamma-A gamma I(A gamma T)-/-G gamma-A gamma T(A gamma I)-.

Induction of osteogenesis by bovine platelet transforming growth factor-beta (TGF-beta) in adult mouse femur.

TGF-beta has been shown to be one of the most important regulators in chondrogenesis and osteogenesis. Daily subperiosteal injection of TGF-beta, which was extracted and purified from bovine platelets, into adult mouse femur resulted in proliferation and differentiation of the mesenchymal cells, chondrogenesis, and endochondral osteogenesis. After cessation of the injection, the endochondral ossification occurred widely, resulting in replacement of the cartilage with new bone. These phenomena demonstrate that exogenous TGF-beta stimulates mesenchymal proliferation and differentiation that occur in early fracture healing and suggest the possibility of clinical application of TGF-beta for fracture repair and bone transplantation.

[The characteristics of inhalation injury and pulmonary infection in burn patients and the influence on their mortality].

940 patients with thermal injury during the past 14 years were reviewed. Of them, inhalation injury was diagnosed in 75 patients, including 15 mild injury who all survived, 25 moderate injury, of whom 13 died with a mortality rate of 52.0% and 35 severe injury, of whom 31 died with a mortality rate of 88.6%. The data showed that 70% of burn that caused inhalation injury occurred in a closed space and 96% of which also caused facial injury. With the burn extent increasing, the incidence of inhalation injury and pulmonary infection rise correspondingly. The time and incidence of pulmonary infection in the patients with inhalation injury was earlier and higher than that in those without inhalation injury (P < 0.01). With similar burn area and age, the risk of death in the patients with inhalation injury was 17.2 times of the patients without inhalation injury (P < 0.001). Pulmonary infection further increased mortality of the patients (P < 0.001).

[Effects of ginkgolide B on isobaric hypoxic pulmonary hypertension in rats].

We examined the effects of ginkgolide B (BN52021), a specific platelet activating factor (PAF) antagonist, on hypoxia-induced pulmonary hypertension and pulmonary vascular remodeling. Chronic hypoxia can cause pulmonary hypertension and pulmonary vascular remodeling in rats. Treated with BN52021, the rat's pulmonary hypertension, pulmonary vascular remodeling and right ventricular hypertrophy at 3 weeks of hypoxic exposure (FiO2 = 0,10) could be reduced. The results suggest that PAF contributes to the development of chronic hypoxic pulmonary hypertension and pulmonary vascular remodeling, and PAF antagonist may be an useful agent for preventing hypoxic pulmonary hypertension.

[A study on behavioural characteristics of staying on the mountain and its relationship with malaria infection in Li and Miao minorities in Hainan Province].

A study of sociology combined with epidemiology was conducted in Li Minority's two villages and Miao Minority's two villages nearby the foot of mountain in a historically malaria hyperendemic area, Nanqiao Township of Wanning County in August, 1993. The results showed that malaria infection was closely correlated to the behaviour of staying overnight on the mountain of Li and Miao Minorities during farming seasons. Most of Li people stayed overnight on the mountain for planting areca were adults, accounting for 21.6% of the whole population, the duration of stay was about half a year; Miao people stayed overnight on the mountain planting and harvesting upland rice, always with their family member, so the percentage of people who had stayed on the mountain was as high as 82.1%, the duration of stay was about one and half a month. Blood smear examination showed that the malaria parasite rates in both Li and Miao villages were 11.1% and 24.1%, respectively, the positive rate of P. falciparum was 1.0% and 9.1%, respectively, the positive rate of IFA (titer >or= 1 : 40) was 31.2% and 46.5%, respectively. It is indicated that both the rate of parasitaemia and the P. falciparum infection were much higher in Miao Minority than those in Li Minority (P < 0.001). A cohort analysis showed that the malaria parasite rate of population who having not stayed, having ever stayed and being stayed on the mountain was 6.5%, 27.4% and 42.1%, respectively, of them the rate of P. falciparum infection was 0.5%, 8.8% and 18.4%, respectively. The results suggest that malaria acquired from mountainous forest referring to "up-mountain infection" is a major source of malaria infection and a significant risk factor in determining the prevalence of malaria (especially falciparum malaria) in Hainan Province. This behavioural risk factor has become the main obstacle for malaria control in Hainan Province. Therefore, controlling "up-mountain infection" and adopting appropriate anti-malaria measures to protect the risk population who have to work and stay on the mountain should be strengthened in the malaria control program.

[Cell culture and freeze-preservation of the human laryngeal epithelial cells].

The laryngeal epithelial cells for culture came from healthy fetuses of induced labor of middle and late period. Cells were cultured under 37 degrees C, 5% carbon dioxide and saturated humidity. The results showed that in the primary generation was cultured for 7 days, the aggregens could be seen by the naked eye. In 10-14 days, the cell layers formed and gradually covered the whole bottom of the flasks. The second generation grew faster than the primary one. Twenty flasks of the cultured cells were freeze-preserved for 14 to 28 days, of which 5 flasks (25%) were resuscitated successfully. By means of the cell culture, some characteristics of the laryngeal epithelial cells can be studied.

[Trabeculectomy combined with extracapsular cataract extraction and posterior chamber intraocular lens implantation].

PURPOSE: To evaluate the effect of trabeculectomy combined with extacapsular cataract extraction and posterior chamber intraocular lens implantation (triple surgeries) on the patients of coexisting cataract and glaucoma. METHODS: 18 cases (19 eyes) of coexisting cataract and glaucoma treated by the triple surgeries were reviewed. Mean follow-up was 14.7 months (rangs from 2 to 20 months). RESULTS: Intraocular pressure was controlled satisfyingly in all cases except one case required additional treatment of the anti-glaucoma drug. 78.9% of patients achieved 0.3 or better visual acuity. The operation complications consist of anterior uveitis and hyphaema. However, all of them were controlled within 14 days. CONCLUSION: The triple surgeries are safe and have definite effect on controlling the IOP and improving visual acuity.

Identification and synthesis of altered peptides modulating T cell recognition of a synthetic peptide antigen.

In studies of T cell responses to synthetic peptides we have observed agonist and antagonist activities associated with contaminants identified within the parent synthesis. The synthesis of two candidate analogues implied by a peptide contaminant formed during the synthesis of La 51-58 (IMIKFNRL) has been carried out. The peptide contaminant was 17-18 Da smaller than the parent peptide consistent with a modified asparagine residue at position 6 and so we synthesised both an aspartimide and a nitrile analogue, representing cyclisation or dehydration of the asparagine residue. The candidate aspartimide and nitrile analogues both bound empty MHC class I molecules to form allo determinants recognised by monoclonal antibodies. These results demonstrate that altered synthetic peptides can bind class I MHC molecules and prompt caution in the use of synthetic peptides as a source of immunising antigen.
