[Concept and first experiences of an oncologic therapy of head and neck carcinoma (author's transl)].

From the principle of an adjuvant chemotherapy evolved a concept of an oncologic therapy of head and neck carcinomas. Even according to the latest international publications there are no such studies until now (Clarysse, Kenis, and Mathé; Recent Results in Cancer Research, Vol. 53, p. 400; Springer, Berlin 1976). Without reducing or shifting the proved indications of curative operation or radiation we used in our clinic in the last years as an adjuvant different cytostatics applicating intra-arterial perfusions or synchronized radio-chemotherapy or polychemotherapy. Among the 200 patients treated hitherto the patients with oropharynx carcinomas conform at present the biggest collective group (intraarterial methotrexat perfusion). Now for the first time after more than two years of observation of 36 patients the efficacy of this treatment can be inferred from the survival rate respectively estimated for the period ahead. Comparing to other surveys it can be confirmed at present that for the total of all tumor stages the 2-year-survival-rate of our patients is 72% against 47% before whereas the survival-rate within the 5-year period can be estimated at 55% to 30%. On the other hand it turned out that 50% of the patients on radio-surgical therapy died after 1.7 years. After the application of adjuvant chemotherapy including operation and radio-therapy the death rate probably will be reached after 6-7 years.

Intraosseous ganglion (author's transl).

The intraosseous ganglion must be always included in the differential diagnosis of cystic epiphysial bone lesions, which are localised in the vicinity of a joint, especially since this bone disease can be succesfully operated on. The preoperative diagnosis can be established with the help of conventional and tomographic x-rays examination. A communicating chanel, which is recognizable in tomographic cuts as a lucent line between the ganglion and a joint, is a pathognomic finding. The interpretation of the radiological symptoms can be difficult in those cases in which the lesions are not yet well developed or in which concomitant degenerative joint changes are present.

[Muco-serous otitis media- a frequently missed disease of the middle ear (author's transl)].

Muco-serous otitis media (muco-serotympanon) is one of the most frequent otological diseases not only among children but increasingly also in the adult age group. In addition to microscopic otoscopy tympanometry is most relevant for diagnosis. Tympanometry is based on measurement of acoustic resistance at the level of the tympanic membrane. In cases of muco-serous otitis very characteristic types of tympanometric curves are obtained. Paracenlastic tube through the tympanic membrane are recommended for therapy.

[The clinical picture, motor activity, electro-encephalographic, neurophysiologic and linguistic findings in a case of left-sided hemispherectomy (author's transl)].

In a patient aged 35 the left cerebral hemisphere was surgically removed for a cerebral angioma. Cerebral function has been examined clinically and from neurophysiologic, neuropsychologic and linguistic aspects. History, pre-and postoperative clinical findings are described. Post-operative motor-activity was registered continuously with 6 electromyograms. It is described, compared with a case of right sided hemispherectomy and discussed. On the basis of a survey of the literature the cerebral activity before and after operation, examined with quantitative EEG analysis, is described and discussed. After a general exposition of the linguistic ability of the right non-dominant hemisphere it is shown that this must have been a case of left dominance. On this assumption the formal-quantitative linguistic findings after left-sided hemispherectomy are described. The neuro-psychologic tests, pre- and post-operatively carried out with several techniques (LPS, HAWIE, FLT, Token-Test), are compared with each other and the results of further tests (a short form of the "Minnesota-test and our own collection of aphasia tests) after hemispherectomy added. After a survey of relevant descriptions in the literature the postoperative linguistic findings (lexic and morphology, syntax, relationship between idea and expressive realization) are described and compared with similar findings in the literature.

[Details of a denounced physician (E.N.T.) suspected of bodily injury, and the following expert judgement (author's transl)].

An otologist was denounced and the patient's case history was confiscated because after tonsillectomy an absecess and a thrombosis of the vena jugularis interna had developed. The procedure and the medical aspect how at first being judged by the lawyer and the public prosecutor are described. On the base of the given medical opinion the court quashed proceedings. Because of the growing number of denouncements this casuistic paper ends with four guiding principles to protect against unjustified medicolegal blame.
