Comparison of clonidine and lithium in the treatment of mania.

The authors compared the antimanic effects of clonidine with lithium carbonate in a double-blind crossover design with 24 volunteers. Lithium was observed to be more effective than clonidine. Some patients reported experiencing hypotension (N=8) and depression (N=7) while taking clonidine.

Contemporary drugs of abuse.

The physician needs to know the signs, symptoms and recommended treatments of drug overdoses. Overdose of hallucinogens usually does not require drug therapy. Overdose of amphetamines ("uppers") may be complicated by the presence of PCP, a dissociative substance. It is important for the physician to be familiar with the street terminology for contemporary drugs of abuse and to be aware of how users obtain these drugs.

Premenstrual tension syndrome in rapid-cycling bipolar affective disorder.

Premenstrual tension syndrome (PMS) and rapid-cycling bipolar affective disorder have similarities of symptoms, cyclical mood swings, and putative neurotransmitter dysfunction. The possible relationship between these disorders was assessed by evaluating 25 patients with rapid-cycling disorders and 25 normal controls for PMS symptoms. Patients with rapid-cycling affective disorder had an increased tendency to have more severe forms of PMS. In addition, patients with rapid-cycling disorders and more severe forms of PMS tended to cycle more frequently. The significance of this finding and its clinical implications are discussed.

Anorexia nervosa in later life.

Anorexia nervosa is generally considered a disease of adolescence. However, recently there has been a growing number of case reports of anorexia nervosa occurring in later life. Physicians must be aware of the fact that anorexia nervosa can occur well into the fifth, sixth, and seventh decades of life. Three cases of probable anorexia nervosa in later life are presented. When confronted with cases of unexplained weight loss, physicians should include anorexia nervosa in their differential diagnosis and pursue the answers to questions that will help make the diagnosis.

Biopsychosocial approach to premenstrual syndrome.

Premenstrual syndrome is a constellation of behavioral, physical and psychologic symptoms that occur every 28 days in some women of childbearing age. A multidimensional, biopsychosocial approach, combining stress reduction, increased exercise and proper diet, has been found effective in treating this syndrome. Medications can be added to relieve symptoms. The cornerstone of this approach is empathic understanding.

Verapamil in the treatment of premenstrual syndrome: case report.

A woman with a long history of premenstrual tension syndrome (PMS) received verapamil for treatment of mitral valve prolapse. Associated with verapamil therapy was a decreased severity in many symptoms of PMS, including agitation, depression, emotional outbursts, and irritability. A possible mechanism is discussed.

Synthesis and pharmacology of the potent angiotensin-converting enzyme inhibitor N-[1(S)-(ethoxycarbonyl)-3-phenylpropyl]-(S)-alanyl-(S)-pyroglutamic acid.

Structure 3a, a potent angiotensin-converting enzyme inhibitor, was prepared in five steps from L-(+)-alpha-amino-4-phenylbutyric acid by construction of the activated side-chain ester 16, displacement with L-pyroglutamate ester anion, and deblocking. Diastereomer separation was accomplished by chromatography at the diester stage, 17. Pharmacological assays established that 3a parallels enalapril in its ability to inhibit converting enzyme and lower blood pressure.

Effect of L-tryptophan on electroconvulsive therapy seizure time.

L-Tryptophan, the amino acid precursor of serotonin and several other monoamines, has frequently been employed as an adjunct with tricyclic antidepressants and monoamine oxidase inhibitors to increase their effectiveness in treating affective disorders. Combined use of L-tryptophan with electroconvulsive therapy (ECT) has generally not been successful and, in fact, may actually have a deleterious effect upon treatment. A case of decreased seizure duration due to an L-tryptophan-ECT interaction is presented and its implications discussed.

Treatment of phenylcyclohexylpyrrolidine (PHP) psychosis with haloperidol.

Twenty white males who presented with psychosis were later found to have ingested PHP. Treatment with haloperidol 5 mg IM caused significant improvement while placebo treatment did not. Results of haloperidol treatment of PHP psychosis were similar to previously published reports with phencyclidine (PCP) psychosis.
