Pharmacologic management of eating disorders.

Treatment of eating disorders is difficult regardless of the methods employed. Pharmacologic management in anorexia nervosa and in bulimia nervosa is especially helpful when it is part of a multimodal treatment approach that includes individual, family and behavioral therapy. Care must be taken to guard against side effects, abuse and noncompliance in a group of patients that tends to be prone to all three.

Blunting of TSH response to TRH in chronic cocaine and phencyclidine abusers.

The TRH stimulation test was administered to 10 cocaine and 10 phencyclidine abusers as well as to 10 controls. No subjects had clinical evidence of depression. Significantly more blunting of the response of TSH to TRH was shown in cocaine and phencyclidine abusers compared with that seen in controls. No significant differences in blunting of response were seen between the cocaine and phencyclidine groups.

Antipsychotic effects of verapamil in schizophrenia.

The antipsychotic effects of verapamil were tested in a double-blind, placebo-controlled, randomized study in 18 patients who met Research Diagnostic Criteria for acute schizophrenia. The antipsychotic effect of verapamil as measured by the Bunney-Hamburg Global Rating Scale were equal to those of haloperidol. Both verapamil and haloperidol were superior to placebo in decreasing psychotic symptoms. The results indicate that verapamil may have clinical utility in the treatment of schizophrenia.

Treatment of depression in chronic cocaine and phencyclidine abuse with desipramine.

An open field trial was conducted comparing desipramine and an active placebo in separate populations of chronic cocaine and phencyclidine (PCP) abusers, who discontinued their abuse. Subjects who received desipramine showed a decrease in depressive symptoms after a 20-40 day period regardless of whether they abused PCP or cocaine.

Despite this fact, factitious hypoglycemic coma is rare. Because hypoglycemia is potentially fatal, with numerous sequelae, physicians need to be aware of its occurrence and method of detection. A case of chronic factitious disorder presenting as hypoglycemic coma is presented and its implications discussed.
