[Clinical investigations of the steroid anaesthetic agent CT-1341 (Althesin) (Cardiovascular and respiratory effects)].

The new steroid anaesthetic CT-1341 (Althesin) has been used as an induction agent in dose of 0.07 ml/kg body weight. Following injection, stage 1 anaesthesia is reached in an average of 30 sec with a clinical duration of between 12 and 20 min with total recovery, supported by a normal pre-anaesthetic EEG tracing, in 40 to 50 min. The respiratory depression following injection in characterized by a short period of apnea followed by an increased respiratory rate and a diminished tidal volume; there is a decrease of the pO2 of about 15%, but the pCO2 and pH values do not show significant variations. Concerning its action on the cardiovascular system, there was found an increase of heart rate (+14%), a decrease of both systolic (-19%) and diastolic pressures (minus decrease of both systolic (-19%) and diastolic pressure (-13.5%), central venous pressure and the peripheral vascular resistance (-14%) with hardly any change in the pulmonary artery pressure.

Methyoxuflurane and serum fluoride concentrations during anaesthesia and postoperative renal function.

In twenty gynaecological patients scheduled for hysterectomy, arterial blood concentrations of methoxyflurane and fluoride ions were measured during operation. Renal function was assessed by pre and postoperative measurements of BUN, creatinine, osmolality and serum electrolytes. Fluid balances were calculated by the daily intake and output values. During the first five postoperative days, from the 12 hour urine specimens the fluoride concentrations were measured, and from the 24 hour specimens urine electrolytes and creatinine levels were calculated. No clinical or laboratory evidence of renal dysfunction was observed. Fluoride levels were lower than those previously associated with renal toxicity. Methoxyflurane concentrations in arterial blood showed concentrations lower as previously reported.

Gas chromatographic determination of methoxyflurane in maternal and foetal blood during anaesthesia for Caesarean sections.

In 50 cases of Caesarean section for various indication, methoxyflurane was administered to two groups of patients in two different dosages by a Pentec-vaporizer. Blood was sampled simultaneously in the radial artery of the mother and in the umbilical vein. The methoxyflurane concentrations of both samples of blood were measured by gas chromatography. With an inspiratory concentration of 0.2 vol.-% methoxyflurane, the mean concentration was 166 mumol/1(2.75 mg/100 ml) in the maternal blood and 69 mumol/1 (1.14 mg/100 ml) in the umbilical vein. With 0.5 vol.-%, the corresponding values were 345 mumol/1 (5.72 mg/100 ml) and 137 mumol/1 (2.25 mg/100 ml) respectively. The condition of the new-born did not appear to be affected by the given doses of methoxyflurane.

Ependymal cyst of the thoracic spinal cord.

A unique case of an ependymal cyst on the anterior aspect of the thoracic spinal cord in a woman aged 68 years is described. Clinical signs were precipitated by trauma. Recovery of function, while incomplete, was remarkably good after extirpation of the cyst.

Influence of benzyl benzoate as oil core on the physicochemical properties of spray-dried powders from polymeric nanocapsules containing indomethacin.

To prepare spray-dried powders of poly(D,L-lactic acid) (PLA) or poly-epsilon-caprolactone (P epsilonC) from colloidal suspensions containing indomethacin (IND) using benzyl benzoate (BnB), nanocapsules (NC) were prepared by nanoprecipitation. To select the best NC formulations, increasing drug concentrations were tested (1.0, 1.5, or 2.0 mg/mL). The particle size was measured by Nanosizer. Spray-dried powders (SDP) were prepared by addition of Aerosil 200 into suspensions of NC. IND was assayed by HPLC. Free IND was determined using an Ultrafree. NC-SPD were examined under SEM. The particle sizes of all formulations are in the sub-300 nm range and are IND-associated, with drug recovery close to 100%. After 1 month, the formulations with highest drug content (2.0 mg/mL) showed a decline of total quantity of IND. After spray-drying, IND recovery for SDP presented values above 100%, indicating that the drug was concentrated from loss of mass during the process. To verify the relationship of oil phase with this loss of mass, similar NC (IND 1.5 mg/mL) prepared with Miglyol 810 (MI) were spray-dried, and SEM analysis showed nanostructures adsorbed onto SiO2. Similar nano-structures were not visualized for NC samples prepared with BnB. A swelling experiment showed the complete dissolution of both polymer by the BnB, whereas for MI the polymer masses remained unchanged. In conclusion, BnB is a solvent for PLA and P epsilonC and this ester is entrained during spray-drying. Despite the use of BnB in formulations of NC, PLA, or P epsilonC, colloidal suspensions prepared with BnB could be micelles instead of nanocapsules.
