[Co-renitek treatment of patients with moderate and severe forms of hypertensive disease].

AIM: To evaluate efficacy and tolerance of a compound drug co-renitec combining an ACE inhibitor enalapril maleate and diuretic hydrochlorothiazide co-renitec taken for 16 weeks in essential hypertension (EH). MATERIAL AND METHODS: 28 patients with EH (16 males and 12 females aged 47-74 years) of mean duration 13.1 +/- 1.6 years. Blood pressure (BP) was monitored for 24 hours with the device SL 90207 (SpaceLabs Medical, USA). Microalbuminuria (MAU) was estimated with the use of immunoturbodimetric test. RESULTS: By 24-hour monitoring, co-renitec reduced day BP by 14.9/8.9 +/- 3/2 mm Hg, nocturnal BP lowered by 18.6/11.4 +/- 3/2 mmHg, pulse pressure also fell. Coefficient T/P was 53.5% for systolic BP (SBP) and 59.6% for diastolic BP (DBP). The target SBP was reached in 77% patients, target DBP--in 69%. Co-renitec significantly decreased MAU, albumines excretion normalized in 46% patients. CONCLUSION: Co-renitec lowers both day and nocturnal blood pressure, improves 24-h rhythm of BP, has a positive effect on the kidneys. This allows its recommendation as a first-line drug in patients with moderate and severe EH.

[Therapy of patients with arterial hypertension with fixed dose combination of losartan and hydrochlorothiazide. Effect on 24 hour blood pressure and left-ventricular hypertrophy].

Twenty four hour blood pressure (BP) monitoring was carried out and structural state of left ventricular myocardium assessed in 20 patients with mild and moderate hypertension before and after 24 weeks of therapy with Hyzaar - fixed dose combination of losartan (50 mg) and hydrochlorothiazide (12.5 mg). According to data of 24-hour BP monitoring the use of Hyzaar was associated with lowering of diurnal (by 26.9/17.2+/-3/2 mm Hg, p<0.001), nocturnal (by 32.6/18.9+/-3/2 mm Hg, p<0.001), pulse (p<0.001) BP, and rate of morning systolic BP rise (p<0.05), decrease of nocturnal systolic and diastolic BP variability, and improvement of 24-hour BP rhythm. Trough/peak coefficients for systolic and diastolic BP (70 and 76%, respectively) reflected sufficient and steady hypotensive effect throughout 24 hours after single dose of Hyzaar. Target BP level was achieved in 70% of patients. At the background of 24-week treatment with Hyzaar left ventricular myocardial mass index significantly decreased (from 120.1+/-3.5 to 108.6+/-3.1 g/m2, D=11.5+/-1.0, p<0.001) and became normal in 60% of patients. Correlation analysis revealed independence of cardioprotective action of therapy from its hypotensive activity. Thus therapy with Hyzaar produced hypotensive and cardioprotective effects which were independent from each other.

[Results of international study of quality of life of patients with stable angina treated with nitrates (IQOLAN)].

Effect of transition from multiple administration of short acting nitrates to once daily use of isosorbide-5-mononitrate on angina class and quality of life was studied in 280 patients with stable angina pectoris. Transition to isosorbide-5-mononitrate was associated with increases of (in units) exercise tolerance (Delta=-6.6+/-0.35, p<0.001), satisfaction from treatment (Delta=-2.1+/-0.12, p<0.001), degree of psychological discomfort (Delta=-2.8+/-0.21, p<0.001), decreases of numbers of attacks of angina (Delta=-4.0+/-0.22, p<0.001) and side effects (Delta=-4.1+/-0.29, p<0.001), increase of average distance of walking without chest pain or dyspnea (from 372.1+/-415.1 to 586+/-663.5 m), shortening of duration of episodes of angina (from 4.7+/-4.0 to 4.1+/-7.2 min), significant lowering of angina class (p<0.0001). Thus in patients with ischemic heart disease long acting formulation of isosorbide-5-mononitrate provided rapid onset and stability of therapeutic action what eventually resulted in effective prevention of attacks of angina and improvement of quality of life.

[Efficacy of 12-week monotherapy with arifon-retard of patients with isolated systolic hypertension].

AIM: To evaluate the effect of arifon-retard (Servie, France) in patients with isolated systolic arterial hypertension (ISAH). MATERIAL AND METHODS: The efficiency of 12-week monotherapy with indapamid (arifon-retard) in a daily dose of 1.5 mg on systolic, diastolic, and pulse arterial pressure (SAP, DAP, and PAP, respectively) was studied in 20 patients with ISAH. RESULTS: SAP decreased by 25.1 +/- 1.8, (p < 0.00009), DAP by 4.2 +/- 0.9 (p < 0.0008), and PAP by 20.9 +/- 2.1 mm Hg (p < 0.00009). Antihypertensive effect was observed in all patients, AP normalized in 30% after 4 weeks of therapy and in 70% after 12 weeks, DAP not decreasing below 70 mm Hg in any of the patients. Antihypertensive effect of indapamid on SAP and PAP additionally increased from week 4 to week 12. With sufficient antihypertensive effect on SAP, the drug did not induce an excessive decrease of DAP and did not affect heart rate and blood biochemistry. The drug was well tolerated, no side effects were observed.

Formation of flat lamellar intramembrane lipid structures in microorganisms.

Analysis of freeze-fracture replicas and thin sections of cells of the bacteria Sulfobacillus thermosulfidooxidans and Anaerobacter polyendosporus showed that their cytoplasmic membranes contain some regions in the form of flat lamellar inverted lipid membranes a few tenths of nanometers to a few microns in size. The specific features of these membrane structures are as follows: (i) they contain no familiar intramembrane particles commonly present on freeze-fracture replicas; (ii) in cross thin sections, intramembrane structures are bi-furcate on the periphery and look like thylakoids; and (iii) the leaflets of intramembrane structures in S. thermosulfidooxidans cells are corrugated. These structures were revealed in bacterial cells cultivated under normal growth conditions. The data obtained suggest the occurrence of a complex type of compartmentalization in biological membranes.

Ultrasound fibrin clot destruction in vitro in the presence of fibrinolytic agent.

Ultrasound fibrin clot destruction was investigated in vitro using electron microscopy and by monitoring the changes in the light transmission of clot debris suspension. It has been established that in the course of a combined action of ultrasound and fibrinolytic agent at high ultrasound intensities and short sonification periods, fibrin clot is disrupted mainly due to sonomechanical treatment, while fermentative lysis takes place in parallel and at a significantly lower rate. However, the streptokinase action prevails after ultrasound switching off and results in the prevention of clot debris conglomeration.

Fine structure of fossilized bacteria in Volyn kerite.

Ultrathin sectioning and cryofracture of fibrous kerite, sampled from 1.8-1.75 billion year old Volyn sediments (Ukraine), revealed in bacteria-like bodies the presence of structures similar to sheath, cell wall, periplasm, cytoplasm, septum, membranes, intramembrane particles, poly-beta-hydroxybutyrate inclusions. On the strength of these data and also the fatty acid profiles of these microfossils, we concluded that fibrous kerites are biogenic formations, namely fossilized bacterial mats.

[Role of hypothalamo-hypophyseal-adrenal axis in the pathogenesis of arterial hypertension in patients with prolactinoma of the anterior lobe of the hypophysis].

AIM: To elucidate mechanisms underlying arterial hypertension (AH) in patients with hyperprolactinemia, prolactin-secreting adenoma of the anterior lobe of the hypothesis, in particular. MATERIAL AND METHODS: 9 females with microprolactin-secreting and 4 females with macroprolactin-secreting adenoma of the anterior lobe of the hypophysis (mean age 33.8 +/- 3.7 years) were examined using radioimmunoassay of the hormones, computed tomography, MR-tomography of the adrenals and brain. RESULTS: Humoral regulation of the hypothalamic-hypophyseal-adrenal axis was found different in patients with microadenoma due to dopaminergic insufficiency of the hypothalamus and in patients with macroadenoma--tumor of the hypophyseal genesis. Patients with microadenoma were diagnosed to have low-renin diastolic arterial hypertension with clinical symptoms of hyperaldosteronism and high levels of plasma aldosterone. Patients with macroadenoma had normal arterial pressure, aldosterone levels and plasma renin activity. Treatment with parlodel, agonist of dopaminergic receptors, reduced arterial pressure, prolactin level, plasma aldosterone and raised plasma renin activity in patients with microadenoma. Such changes were not observed in patients with macroadenoma. CONCLUSION: It is suggested that one of the causes of AH in patients with microprolactin-secreting hypophyseal adenoma lies in hyperaldosteronemia which develops as a result of dopaminergic insufficiency of the hypothalamus and hyperprolactinemia.

Phylogenetic status of Anaerobacter polyendosporus, an anaerobic, polysporogenic bacterium.

The almost complete sequence of the 16S rRNA gene of the Gram-positive polysporogenic bacterium Anaerobacter polyendosporus was determined. This allowed phylogenetic analysis of A. polyendosporus by comparing sequences of the 16S rRNA gene of this bacterium to similar genes of other Gram-positive bacteria. It was shown that this polysporogenic bacterium belongs to the Clostridium cluster I, subcluster A. Phylogenetically, A. polyendosporus is distantly related to another polysporogenic, but non-cultivatable, bacterium, 'Metabacterium polyspora' and can be satisfactorily clustered within the saccharolytic clostridia with a low DNA G+C content grouped in subcluster A. A. polyendosporus was most closely related to Clostridium intestinale (94.8% identity of 16S rRNA genes) and Clostridium fallax (93.1%). Like other members of the Clostridium cluster I, subcluster A, A. polyendosporus possesses such common phenotypic features as a Gram-positive cell wall structure, anaerobiosis, derivation of energy from carbohydrate fermentation yielding butyric acid among other organic acids and the capacity for endogenous spore-formation. However, the scale of evolutionary change in the 16S rRNA gene between A. polyendosporus and phylogenetically related Clostridium species does not correspond to the profound changes in the phenotype of A. polyendosporus. Distinctive phenotypic features of the latter are large cell size, polysporogenesis (up to seven spores per cell), alternative modes of development and an unusual membrane ultrastructure.

[Levels of biologically active substances in the blood of patients resistant and nonresistant to antihypertensive therapy].

AIM: To clarify the role of differences in the levels of some biologically active substances (BAS) in plasma of patients with resistance to therapy or without it measured before treatment, during treatment and application of plasmapheresis. MATERIALS AND METHODS: Resistance to therapy was assessed clinically and according to 24-h monitoring in 35 patients aged 35-56 years with essential hypertension (n = 19), related to chronic pyelonephritis (n = 13) and glomerulonephritis (n = 3). Plasma BAS were studied by radio- and enzyme immunoassays. RESULTS: Blood pressure monitoring provided more precise determination of the treatment responders than clinical tests. Initially, responders to therapy had higher level of endothelin-1 and low of hydrocortisone. CONCLUSION: Responders to antihypertensive therapy were distinguished by a significant decrease of plasma aldosterone within a year vs aldosterone levels before the treatment. Nonresponders had no significant decrease of plasma aldosterone. No other marked differences in plasma BAS between the responders and nonresponders to antihypertensive treatment were found.

[The prolonged-action form of Lomir (isradipine) in the treatment of mild and moderate essential hypertension].

24-hour monitoring of arterial pressure has been performed outpatiently in 20 subjects with mild or moderate essential hypertension using portable system Space Labs 90207. The design of the study implied 2 weeks of observation without any treatment followed by a 4-week course of lomir SRO-5 plus a 2-week course of lomir administration in a double dose. Control examination was performed at the end of each period. It was established that a 4-week lomir course results in a lowering of systolic and diastolic pressure by 8.4 + -2.1 and 6.9 + -1.3 mm Hg, respectively. Double-dose lomir was also effective: a fall in both systolic and diastolic pressure by 15.6 + -3.2 and 8.1 + -1.8 mm Hg, respectively, was achieved.

Cytological aspects of resistance to potassium tellurite conferred on Pseudomonas cells by plasmids.

The ultrastructure of strains Pseudomonas putida BS228 and Pseudomonas aeruginosa ML4262 harboring plasmids pBS10, pBS31, and pBS221, which determine resistance to potassium tellurite, was studied. Bacteria were grown in media containing increasing concentrations of potassium tellurite. Crystalline structures containing tellurium appeared in their periplasmic space. The dynamics of crystal growth was studied. Crystals were released into the medium by pinching off of the outer membrane vesicles containing growing crystals. A possible mechanism of this process was described; cytobiochemical peculiarities were discussed.

[Changes in biogenesis and secretion of periplasmic alkaline phosphatase, coded by the phoA gene included in the Escherichia coli DH1 plasmid].

Augmented synthesis of periplasmic alkaline phosphatase from E. coli DH1 coded by the phoA gene within the composition of plasmid pHI-7 results in alterations in the enzyme biogenesis, such as accumulation of intermediate enzyme forms corresponding to various stages of its posttranslational modification, and alternative localization. The cell cytoplasm was found to contain a large proportion of alkaline phosphatase precursors in the form of insoluble aggregates. The mature enzyme was detected in both the periplasm and the cultural fluid in the form three methazymes with an increased (as compared with the original strain) content of methazymes I and II. The cell envelope of E. coli DHI transformed by plasmid pHI-7 with the phoA gene differed from that of the original strain by an increased acid phospholipid (cardiolipin/phosphatidylglycerol) ratio as well as by increased distribution density of intramembrane particles on the surface of the cytoplasmic membrane facing the cytoplasm. Changes in the biogenesis of the enzyme during its augmented synthesis are due to disturbances in the equilibrium between the synthesis of polypeptide chains, on the one hand, and their translocation and processing, on the other, apparently as a result of restricted secretion sites and lack of inhibition of translation of secreted proteins in bacteria.

[Detection of polyphosphatase activity in isolated Saccharomyces cerevisiae nuclei].

Intact nuclei have been isolated from cells of a diploid strain of Saccharomyces cerevisiae. The isolated nuclei were free from cytoplasmic, mitochondrial and vacuolar marker enzymes. The protein to DNA ratio (w/w) was 11. Pyrophosphatase, tripolyphosphatase and exopolyphosphatase activities have been found in S. cerevisiae nuclei for the first time and were equal to 400, 130 and 55 mU/mg of protein, respectively.
