Inattentive and noninattentive ADHD children: do they constitute a unitary group?

Teacher-rated ADHD and normal control children were administered a continuous performance test (CPT), and were then further subdivided based upon the presence or absence of objectively assessed attentional deficits. In addition, children were assessed using several measures of cognitive and behavioral functioning. Attentional deficits were significantly more prevalent among the ADHD group, but about half of the ADHD children showed no evidence of objectively assessed attentional dysfunction. Further group analyses indicated that ADHD children with objectively assessed attentional dysfunction appeared cognitively impaired, while ADHD children without objective evidence of attentional dysfunction had more conduct problems. CPT inattention was not related to the presence of cognitive impairments or conduct problems in the control group. These data must be considered preliminary because teacher ratings were the only source of diagnosis and a single measure of inattention was used. However, they suggest that two subtypes of ADHD children can be identified, one characterized by inattention and learning problems, and the other by conduct problems.

Influence of superovulation on endometrial and embryonic development.

The authors have studied the temporal relationship between follicular rupture and endometrial development in 13 women during a natural ovarian cycle (length 25 to 35 days), and subsequently after standard treatment with clomiphene citrate, human menopausal gonadotropin and human chorionic gonadotropin (hCG) to induce multiple folliculogenesis for oocyte recovery, in vitro fertilization, and embryo freezing (cycle length 23 to 27 days). An endometrial biopsy was taken during both cycles 1.5 to 2.0 days after the oocytes had been released or removed. The samples were examined by light and transmission electron microscopy. Samples of peripheral blood were taken at defined times for hormone analysis. After treatment 11 subjects (85%) had advanced morphological development of the endometrium (8 women by 3 to 4 days, 3 women by 1 to 2 days). The concentrations of plasma estradiol (E2) and progesterone (P) on the days of follicular rupture and endometrial biopsy were significantly raised in the treatment cycles. The concentration of total urinary estrogens on the day of hCG administration and the mean change in the concentration of plasma E2 (treatment/control) on the days of endometrial biopsy were positively correlated with the extent of endometrial advancement. In addition, the mean change in the concentration of plasma P (treatment/control) was markedly increased on the days of follicular rupture and endometrial biopsy in those subjects with an advanced endometrium. Embryonic development was not so obviously related to the extent of superovulation. Asynchronous endometrial and embryonic development may therefore contribute to the low pregnancy rate in these patients.

Cervical intramedullary neurofibroma.

Intramedullary spinal neurofibroma is extremely rare. A case of cervical intramedullary neurofibroma is presented in a 21-year-old man. Only 20 case have been previously reported in the literature. The site of origin is discussed with various hypotheses.

Pyogenic meningitis.

Fifty cases with pyogenic meningitis, aged 15 to 72 years, with a male:female ratio of 5:1, were studied. The majority of cases (70%) were in the 15 to 25 years age group. The organisms detected in the 29 culture-positive cases included S aureus (12), pneumococci (9) and meningococci (8). Thirteen patients died; the poor prognostic factors included old age, the presence of coma or shock, CSF sugar below 10 mg/dl or protein above 750 mg/dl and the presence of gram negative organisms in the CSF.

Radiation therapy in early glottic carcinoma: significance of prognostic factors and dose fractionation.

A retrospective analysis of 208 cases with early glottic cancers treated by radiation therapy is presented. Early T1 (156 cases) and T2 (52 cases) lesions accounted for only 43 per cent of all glottic tumours that presented at the Tata Memorial Hospital between 1975-80. A markedly predominant male to female ratio of 22:1 was noted with 66 percent presenting in the fifth and sixth decades of life. Radical Radiation therapy was delivered with two different regimens according to the then prevalent physician preference. Regimen 1 delivered 50 Gy/15 frs. in three weeks and regimen 2 with 60 Gy/24 frs./5 weeks. The actual total survival and disease free survival was 92 per cent and 85 percent for T1 lesions and 82 percent and 67 percent for T2 tumours respectively. Minimal tumour volume involving a unilateral mobile vocal cord had the best prognosis. With extension to more lengths of cord and/or involvement of the anterior commissure or with tethering or fixation of the cord the prognosis was adversely influenced. In T1 lesions, the shorter course with the higher fractionation dosage levels showed improved local controls. However, the results were not statistically significant. This was not so in the T2 lesions probably due to small number of cases. Treatment failure was recorded in 19 per cent cases of which 90 per cent was seen in the first two years after treatment. Persisting laryngeal oedema confirmed a high index of suspicion towards recurrent disease. A radiotherapeutic technique using proper patient positioning and accurate beam direction with total dosage level above 1850 rets and TDF between 101-106 values is the recommended optimal treatment. In conclusion, an understanding of the prognostic features and an appropriate dose fractionation schedule allows optimization to effectively control early glottic tumours and retain function in the majority of patients.

Incidence of australia antigen (HBs Ag) in Himachal Pradesh.

2405 high risk subjects (1193 patients attending STD clinics, 1012 blood donors and 200 hospital personnel) and 500 apparently healthy individuals representing all the twelve districts of the State of Himachal Pradesh were screened for HBs Ag employing reverse passive haemagglutination (RPHA) technique. HBs Ag positivity was found to be 6.77 per cent in test groups and 3.6 per cent in the control group. Maximum positivity was found in STD patients (9.55 per cent) followed by hospital personnel (8 per cent) and blood donors (3.26 per cent). The highest incidence was noticed in district Kullu and no positive case was found in Lahaul and Spiti district of Himachal Pradesh. Remedial measures for prevention of Hepatitis-B virus infection have been emphasized.

Phytobezoar extending from stomach to cecum.

We report a patient with a phytobezoar extending from the stomach through the small intestine to the cecum. The patient presented with acute intestinal obstruction and bezoar was suspected at exploration. The specimen showed two masses of cotton fibres connected with a long string of variable thickness.
