A comparative study of the renin-like activity in the heart and vascular system under various experimental conditions.

Renin-like activity in the heart and aorta of rats being slightly modified by binephrectomy, its variations in DOCA hypertension and infarcted ventricular muscle were studied. The daily i.p. administration of DOCA 12 mg/kg body weight for 35 days in male adult rats resulted in a significant decrease of renin activity in plasma and tissues of the heart, aorta, hypothalamus and hypophysis. In contrast to renin-like activity, cathepsin D measured in the same animals increased in all organs, except for the plasma. Similar changes of renin-like activity were observed in salt-loaded animals with 1.7% sodium chloride solution ad libitum for 35 days. In the infarcted myocardial ventricular muscle of the rats and rabbits, the tissue isorenin showed a tendency to decrease, associated with a significant increase in cathepsin D activity. Like in aorta, isorenin seems to be a different enzymatic entity of cathepsin D in the myocardial tissue. The measurement of isorenin content of the vascular endothelium and cardiac muscle fibers seems to reveal much higher amounts in the coronary vascular endothelium than in the myocardial fibres. The activation of the enzymatic angiotensin forming mechanisms in the coronary vascular bed could be one of the risk factors in myocardial infarction.

[Posttraumatic optochiasmic arachnoiditis].

Optochiasmic arachnoiditis is a complex symptomatological ocular and neuroendocrine disease. We present a 32-years-old patient who was hospitalized in Ophthalmological Clinic from Iaşi because a slow visual acuity diminishing, visual field troubles and neurological symptoms which appeared after a cranial contusion. Ophthalmological and neurosurgical exams established "optochiasmic arachnoiditis" diagnosis, which was confirmed peroperator. The exceptional postoperator evolution was, after a while, eclipsed by final functional ocular result.

[Functional results in the surgery of congenital cataract].

During 1983-1993, 202 patients (a total of 230 eyes), 113 males and 117 females, were operated. From these, 47 selected cases could not be properly followed and the functional results were established only after repeated checking. In these cases, the surgical techniques consisted of 7 cryoextractions, 47 extracapsular extractions and 18 irrigations-aspirations in double stream. 47 lens implants have been introduced, 43 were Fedorov [correction of Fyodorov]-type with pupillary fixation and 4 were posterior chamber implants. The visual acuity was almost the same to both the patients who have benefited from implants and to those corrected with contact lens or glasses.

[The Wermer syndrome].

The difficult diagnostic and therapeutic management of patients having gastrinoma with multiple endocrine neoplasia type I (MEN I) has been discussed by reference to the literature and a personal experience of 2 patients with Wermer syndrome. The syndrome is often familial and might be inherited as an autosomal dominant trait with a high but variable degree of penetrance. Pancreatic islets, parathyroid glands and adenohypophyseal [correction of antehypophyseal] cells are the three localization main for endocrine involvement in MEN I syndrome.

[The value of scintigraphic imaging with 99mTc-MIBI for the diagnosis of gliomas].

Imagery is the only possibility of early glioma diagnosis. Both the gravity of the evolution and the bad prognostic, through all CNS tumors, justify the necessity to asses new techniques in order to improve glioma imagery. From this point of view, cerebral tomoscintigraphy has developed, its usefulness depending highly on the radiotracer tumoral specificity. One of these tracers, 99mTc MIBI, has been found to be uptake in glioma malignant cells, in relation with their viability and even malignity. In our study, -14 cerebral tomoscintigraphies have been realized on patients with glioma (initial or recurrent tumor), after 20 mCi 99mTc MIBI i.v. doses. The results are in correlation with the histologic glioma grade: high grade astrocytoma has positive images, but low grade astrocytoma doesn't. In conclusion 99mTc MIBI glioma tomoscintigraphies can be useful in their initial or recurrent diagnosis; the uptake tracer intensity being correlated with the histologic tumoral pattern, 99mTc MIBI SPECT could make possible a better localization of highest malignity tumoral points, in further biopsies.
