[Clinical and morphological analysis of adenoid vegetations in children].

The paper outlines the authors' clinical and morphological experience in clinically rehabilitating children with adenoid vegetations and sinusitis. Retrospective morphological and immunohistological studies have revealed that the structure of adenoids corresponds to the morphology of humoral and cellular immune responses. This enables adenoid vegetations to be regarded as an actively functioning secondary immunity organ that requires maximum sparing in young children.

[Clinical and morphological aspects of benign tumor-like growths in the vocal cords].

The structure of the nasal septum was studied on 34 samples obtained from dead bodies of children aged 3 to 14. The septums were removed from the skull cavity. The morphometric studies consisted of measurements of the anteroposerior, superior-inferior, septum and its area, as well as composite anatomical structures. It is shown that the size and area of the septum and its anatomical structures undergo age-specific changes which are not regular. The data should be accounted for in correction of nasal septum structural defects in children of different age.

[Histological evaluation of the condition of the trepanation cavity coating after radical surgery on the middle ear].

Tissue samples obtained intraoperatively from different parts of the middle ear in the course of the radical operation (35 cases) or reconstructive operations in patients with the history of surgical intervention on the ear (67 cases) were examined histologically. Fibrous-atrophic changes in the trepanation cavity coating correlated with the duration of the disease and the time since the radical operation. Pathomorphological and topographic profile of the changes in the operated ear coating copies relevant picture of otitis media purulenta chronica. The tympanomastoid cavity coating was characterized by increasing fibrous-atrophic changes reflecting regeneratory-plastic insufficiency of the epithelium and persistent tympanofibrosis in conditions of open trepanation cavity with disturbed anatomic relations in the middle ear.

[Morphofunctional organization of the palatine tonsils as a lymphoepithelial organ].

The paper considers compartment structure of the tonsils reflecting cooperative interactions of the cells in immune response. A special emphasis is laid on the lymphoepithelial relations at the level of criptal epithelium the area of which serves as an immunoregulatory compartment of the palatine tonsils. Similarity of the regions of lymphoepithelial symbiosis in the zone of the criptal epithelium and lymphoepithelial pattern in the thymus is discussed as well as the role of the palatine tonsils as a secondary organ of immunogenesis in the general hierarchy of the immune system.

[Choanal polyps: principles of current diagnosis and treatment].

Computed tomography, endoscopic investigation, surgical endoscopic revision of the paranasal sinuses performed in 20 patients with choanal polyps (CP) showed that according to their growth origin, CP may be antrochoanal, sphenochoanal and ethmochoanal (11, 3 and 6 patients, respectively). Thorough preoperative examination prevents opening of unaffected paranasal sinuses. However, when several sinuses are involved, site of CP fixation may be detected at surgery. Endoscopic techniques presented as a good alternative to routine polypotomy and Caldwell-Luc operation. In atypical CP location a differential diagnosis with inverted papilloma must be made.

[Morphologic features of chronic otitis media in children].

Surgical material from the middle ear of 80 children at the age of 5 months to 15 years who suffered from chronic otitis media as well as 20 temporal bones of foetuses (24-36 weeks) and stillborns without ear diseases are studied. It is established that the character of topography and inflammation in the ear is associated with persistence of myxoid tissue and a low degree of differentiation in the retrotympanic regions of the middle ear this excluding the possibility of its functioning as a local immune barrier. Myxoid tissue is the main reactive substrate of purulent otoantritis and otomastoiditis in children of early age and, as mesenchymal tissue with angioplastic potencies, may be regarded as a conductor of a secondary cholesteatoma of the attico-antral region.

[A case of secondary syphilis simulating plasmocytoma].

A clinically unclear case is described of stomatitis with a long duration in a male of 28. Exfoliations in the region of the mouth angle were observed resembling pemphigus vegetans. Biopsy examination revealed a thick plasma cell infiltration which was considered to be an extramedullary plasmacytoma. However, a polyclonal composition of plasmacytes was found immunomorphologically producing immunoglobulins A, M and G with predominant production of IgG. This allowed rejection of neoplastic nature of the infiltrate, confirming its inflammatory origin and specific character. Serological tests (Wassermann, immunofluorescence test, in vitro immobilization test) confirmed the diagnosis of secondary recurring syphilis. Differential diagnosis of plasma-cell syphilid of skin and mucosal extramedullary plasmacytoma is under discussion.

[Characteristics of immunity in children with lymphoid hyperplastic diathesis].

As many as 100 children aged 3 months to 4 years with lymphaticohypoplastic diathesis (LHD) were examined for clinico-immunologic++ interrelations. The children suffering from manifest LHD demonstrated the reduction of the amount of circulating T lymphocytes and IgA, IgM and IgG subpopulations. At the same time the changes in local immunity correlating with enlargement of the thymus and lymphoid formations in the nasopharynx were identified. The immunity unbalance in LHD was supported by morphologic and immunologic studies of the tissue of the removed hypertrophied tonsils. The specification of the deranged mechanisms of immunity in LHD requires early diagnosis of the illness, screening of the methods for immunocorrection and closer follow-up of the sick children.

[Results of implantation of teflon paste into the vocal cord according to the data of light and electron microscopy].

Light and electron microscopy was used to study 5 observations of the tissue response to the teflon paste implanted into the vocal cords to modulate the vocal function. The study showed that teflon was not an absolutely inert substance, it produced inflammation as giant cell granuloma of foreign bodies. Giant cells of foreign bodies (GCFB) enclosing and resorbing the particles of teflon were found to have all the ultrastructural signs of actively phagocytizing cells presenting with various differentiation. The ultrastructure of GCFB provides support for the fact that the cells making up polykaryons belong to the system of mononuclear phagocytes.

[Materials for the morphologic diagnosis of rhinosporidiosis].

A case of a nasal form of rhinosporidiosis is described in a 12-year-old girl. Its clinical diagnosis was made on the basis of histological examination of specimens taken during surgery. The inflammatory response to rhinosporidiosis was non-specific and followed by mucosal papillary hyperplasia. There were massive areas of fungal invasion and microorganisms at various stages of development, reflecting the ability of the parasite to autoinoculate and spread. The observation validates the well-established practice of surgical treatment of rhinosporidiosis that provides for coagulation or freezing of the area of the pathologic formation removed.

[Allergic inflammation].

Current ideas of allergic inflammation reflected in the concept of immune inflammation are described. This concept considers immune inflammation to be an immune response mediated by a certain immunity reaction which gives the inflammation a number of qualitative and quantitative differences. The nature of the differences allows allergic (immune) inflammation to be evaluated as a biologically expedient type of defence-adaptative reaction in which the inflammatory response is optimally expressed owing to the participation of immunological mechanisms.

[Morphohistochemical characteristics of tympanosclerosis and problems in its morphogenesis].

The results of morphological and histochemical examinations of tympanosclerosis performed on the operation materials from 64 patients suffering from conductive hypoacusis are presented. The structure of the tympanosclerotic plaque was shown to reflect the combination of degenerative and fibroplastic alterations of mucoperiosteum with the outcome into petrification and hyalinosis. The location, structure, and histochemical features of the tympanosclerotic plaque prevent considering it to be a derivative of the granulation tissue. At the same time there are reasons to assume the serous nature of the preceeding mucoperiosteum inflammation. The development of tympanosclerosis reflects the reactive features of mucoperiosteum as the bradytrophic tissue possessing osteogenic potentials.

[Mucormycosis of the maxillary sinus and the ethmoid labyrinth].

An observation of mucoromycotic sinusitis in a patient with diabetes mellitus is described. The disease was in the form of acute progressive suppurative-necrotic maxillary sinusitis-ethmoiditis complicating an abscess of the nasal septum. Mucoromycosis was diagnosed by histological examinations of tissues removed from the affected sinuses. Pathomorphology of this disease was characterized by deep invasion of the fungus into the tissues including blood vessels, the development of vascular disorders, extensive necroses, formation of nonspecific granulomas.
