[Potentialities of angiography in the diagnosis and treatment of angiosarcoma of the liver].

Clinicoroentgenological evidence is described for 3 patients with nonresectable hepatic angiosarcoma. Two of these underwent therapeutic embolization of hepatic artery. Differential diagnosis between angiosarcoma and other hepatic malignancies by angiographic images seems impossible. Hepatic artery embolization can be employed as palliative treatment of advanced angiosarcomas.

[Clinical aspects of diagnosis and treatment of aneurysms of the splenic artery].

Literature data and original clinical evidence are reviewed concerning diagnostic and treatment aspects of splenic artery aneurysms (SAA). Five cases of aneurysms of the splenic artery trunk in 4 females and 1 male aged 23-54 are described. There was a case of a false SAA due to previous abdominal trauma and consequent pancreatitis. In another case SAA combined with cavernous hepatic hemangioma. Portal hypertension resultant from hepatic cirrhosis was seen in 3 out of 5 SAA cases. Two patients underwent embolization of the artery trunk proximally from SAA, three patients--ligation of the splenic artery with removal of the SAA and the spleen. Current methods of investigation are believed capable of SAA diagnosis before fatal complications, with differential approach to SAA evaluation warranting definition of surgical treatment scope.

[Embolization of the hepatic artery in hemangiomas of the liver].

Embolization of the proximal part of the hepatic artery was carried out in three patients with hemangioma of the liver who did not meet the criteria for radical surgery. Metal spirals were used for this purpose. Response was observed in all the cases. Patients were followed for 9-36 months. The data obtained suggest that embolization of the hepatic artery is effective and may be used for the treatment of inoperable vascular tumors of the liver.

Preoperative portal vein embolization for liver malignancies.

BACKGROUND/AIMS: To study effectiveness of preoperative portal vein embolization before extensive hepatic resection in patients with primary or secondary liver malignancies. METHODOLOGY: Between December 1997 and May 2003, right portal vein embolization was performed in 24 patients. The indication to the procedure was a small amount (< 30%) of the future remnant liver. Ultrasound-guided percutaneous transhepatic puncture of the portal vein and embolization of its right lobar branch with Lipiodol, ethanol, gelatin sponge and/or steel coils were used. RESULTS: There were no complications of the procedure. In 19-56 (mean, 30) days, CT or MRI showed significant increase of the future remnant liver volume from 25% to 35% of the whole liver; the mean increase was 40%. Laparotomy was performed in 15 patients. Right, extended right hepatectomy, and repeated resection were performed in 7, 6, and 1 patient respectively, while one patient underwent only exploration because of tumor progression. Of 14 resected patients, 11 showed no postoperative liver failure. The latter was medically cured in two of three remaining patients, while one patient (future remnant liver < 30% even after the embolization) died from liver failure in 20 days after the surgery. Hepatic resection is planning in 4 of 9 non-operated patients. The 5 patients were not operated because of tumor progression. CONCLUSIONS: Right portal vein embolization is a safe and technically simple procedure that decreases the risk of postoperative hepatic failure after major liver resection.

Transcatheter treatment of splenic artery aneurysms (SAA). Report of two cases.

Splenic artery embolization with steel coils was performed in two patients who both had large splenic artery aneurysms and hepatic cirrhosis complicated by hypersplenism. A good clinical effect was noticed after the procedure. It was concluded that this treatment is safe and effective and decreases the risk of splenic artery rupture. It also corrects hypersplenism. Transcatheter embolization appears to be a preferable alternative to surgery in such cases.

[Arterial embolization in unresectable cavernous hemangiomas of the liver].

Seven transcatheter hepatic artery embolizations (HAE) were carried out in 4 females with unresectable symptomatic cavernous hemangiomas of the liver. An only complication occurred (acute cholecystitis), treated conservatively. Devascularization of hemangiomas resulted in decrease (3) or disappearance (1) of patients' complaints. Three tumors slightly decreased in size while the rest one remained unchanged. Partial fibrosis of the tumor was seen in two cases. All the patients are alive and well for 1 to 2.2 years after HAE. Such an operation may be useful for palliative treatment of unresectable cavernous hemangiomas of the liver.

[Premises for intravascular radiological interventions in hepatic hemangiomas].

In the paper anatomic and radiological preconditions are considered for intravascular treatment of hemangiomas of the liver based on the study blood vessels of corrosion preparations of 5 tumors, removed during a resection of a liver; results of 69 angiographies of the liver and 73 hepatic radiographies. The authors offer a 3-marks evaluation of the arterial blood flow in a total blood supply of the hemangiomas. Study of the versions of the arterial blood supply for hemangiomas enabled to validate the different approach in use of intravascular treatment (ligation or embolization of hepatic artery) in patients with hemangiomas of the liversince a surplus arterial flow in the tumors was found in 53.6% of observations.

[Endovascular surgery in the treatment of hepatic hemangiomas].

Results of endovascular interventions in 46 (45%) patients from 102 cases are analyzed who underwent surgical treatment with regard to hemangiomatosis of the liver. Embolization of hepatic artery (HA) and its branches was performed in 35 pts., ligation of HA-in 11. Endovascular intervention was performed once in 27 (77.2%) patients, twice--in 6 (17.1%), three times--in 2 (5,7%). Metal coils of Gianturco type (8), Spongostan (7), Ivalon (7), coil in a combination with Spongostan (20), coil and Ivalon (3) were used for embolization. Comparative evaluation of the results of the embolization. and ligation of HA is presented taking into account advantages and disadvantages of the considered treatment methods. Indications, techniques and complications endovascular treatment of hemangiomas are discussed in this paper considering a lesion spread in the liver, number and size tumor nodes, variants of their arterial blood supply.
