Lymphocytotoxins in leprosy.

Lymphocytotoxic antibodies (LCAs) were assayed in serum samples from 60 patients of leprosy spectrum before starting multi-drug therapy (MDT). Seventeen healthy volunteers without any history of viral infection provided control samples. Post treatment follow up samples were also included in the study. In all pretreatment sera of LL, BL and BT/TT patients the levels of LCAs were elevated, the figures were 39.05 +/- 23.87, 44.89 +/- 20.74 and 34.16 +/- 17.50 respectively. With treatment a significant fall in LcAs production was observed in all types of leprosy patients.

Studies on some iridium(III) complexes with Schiff bases derived from amino carboxylic acids.

The reactions of iridium(III) chloride with different Schiff bases gave complexes of types [Ir(SB)3], [Ir(SB')Cl(H2O)2], [Ir(SB'')Cl2]n, [Ir(SB'' ')Cl(H2O)]n (SBH = Schiff bases derived from anthranilic acid and benzaldehyde, acetophenone, vanillin, cinnamaldehyde or m-hydroxyacetophenone; SB'H2 = Schiff bases derived from anthranilic acid and salicylaldehyde or o-hydroxyacetophenone; SB''H = Schiff bases derived from p-aminobenzoic acid and benzaldehyde, acetophenone, vanillin, cinnamaldehyde, or m-hydroxyacetophenone; SB'' 'H2 = Schiff bases derived from p-aminobenzoic acid and salicylaldehyde or o-hydroxyacetophenone). These complexes have been characterized on the basis of elemental analyses, conductance, magnetic moment, and spectral (electronic, i.r., and 1H n.m.r.) data. The electronic spectra reveals octahedral geometry for these complexes except for [Ir(SB'')Cl2]n, which is trigonal bipyramidal. The thermal behavior of these complexes has also been studied by TG, DTG, and DSC techniques. The different kinetic parameters, viz., order of reaction, activation of energy, and heat of reaction were calculated. The antifungal and antiviral activities of the complexes with Schiff bases derived from anthranilic acid have also been investigated.

Immunogenic and antigenic characteristics of Salmonella heat-labile enterotoxin.

The intramuscular immunization of rabbits with enterotoxin of S. weltevreden failed to provide protection against challenges with homologous Salmonella enterotoxin as well as heterologous enterotoxins (cholera toxin or E. coli LT). Similar results were obtained in rabbits immunized with cholera toxin, choleragenoid and E. coli LT. However, Salmonella antitoxin contained neutralizing antibodies against Salmonella enterotoxin (but not against cholera toxin) and thus was capable of neutralizing Salmonella enterotoxin when tested for skin permeability reaction. Immunodiffusion experiments showed that antitoxin prepared against the enterotoxin of one of the strains of S. weltevreden formed precipitin bands with enterotoxin preparations of 5 strains of S. weltevreden and 2 strains of S. anatum. However, Salmonella antitoxin failed to form precipitin bands with enterotoxins of other heterologous Salmonella species (S. dublin, S. enteritidis, S. hindmarsh and S. newport), cholera toxin and E. coli LT. The immunoelectrophoretic studies corroborated the results obtained by double immunodiffusion experiments. However, both Salmonella and cholera toxins migrated electrophoretically toward the cathode and resembled globulin in this respect. Salmonella enterotoxin, though immunogenic, yet proved unprotective through the parenteral route and appears to be antigenically distinct from cholera and E. coli enterotoxins.

The effect of anti-reactional drugs on complement components in the type II, erythema nodosum leprosum, reaction.

Seventeen patients with the type II (erythema nodosum leprosum) (ENL) reaction were studied. They received multidrug therapy and also the anti-reactional drugs prednisolone clofazimine or chloroquin, and we measured serum levels of complement components before treatment and after the reaction had subsided. Factor B was significantly elevated after treatment with each of the three drugs. C3 levels were significantly increased after treatment, the largest change being in patients treated with clofazimine. In these patients there was also a concomitant decrease in C3d levels. This suggests that clofazimine has complement modulating activity and we would recommend it as the drug of choice in treatment of the ENL reaction.

Stimulation of muscarinic receptors raises free intracellular Ca2+ concentration in rat ventricular myocytes.

The effect of carbachol on free intracellular calcium concentration, ([Ca2+]i) and on intracellular hydrogen concentration (pHi) was determined from fluorescence signals obtained from rat ventricular myocytes. Application of carbachol (300 mumol/l) to quin2-loaded myocytes bathed in 2 mmol/l Ca2+-containing solution caused [Ca2+]i to increase within 7-10 minutes from 182 +/- 9 to 212 +/- 11 nmol/l (n = 4). Carbachol acted via stimulation of muscarinic receptors because atropine (1 mumol/l) either prevented or abolished the increase in [Ca2+]i. Carbachol also produced a positive inotropic effect in rat papillary muscles contracting isometrically at a frequency of 0.5 Hz and enhanced contracture in resting preparations in the presence of high extracellular Ca2+ concentration ([Ca2+]o) (20 mmol/l). The effect of carbachol on [Ca2+]i was dependent on [Ca2+]o. In the presence of 10 mmol/l [Ca2+]o, the increase in [Ca2+]i was about two times that elicited by carbachol when bath [Ca2+]o was 2 mmol/l. Reduction of [Ca2+]o to 50 mumol/l abolished the carbachol effect but did not prevent caffeine-induced Ca2+ release. The carbachol-induced rise in [Ca2+]i remained unchanged in the presence of either 10 mmol/l caffeine or 1 mumol/l ryanodine. In the absence of extracellular Na+ concentration [( Na+]o), carbachol no longer produced an increase in [Ca2+]i of cardiomyocytes and failed to enhance Na+-withdrawal contracture of the rat papillary muscle. In contrast to the effect on [Ca2+]i, carbachol did not produce any change in pHi as determined from fluorescence signals obtained from rat ventricular myocytes loaded with 2',7'-bis(carboxyethyl)-5,6-carboxyfluorescein.(ABSTRACT TRUNCATED AT 250 WORDS)

Bacillaemia in leprosy: correlation with slit-skin and nasal smears.

Twenty five multibacillary patients (BL/LL) were studied for bacillaemia. Majority (76%) showed acid fast bacilli in peripheral blood. There was good correlation between bacillary load in peripheral blood and bacteriological index (BI) but poor correlation with morphological index (MI) of skin slit smear and BI/MI of nasal smear.

Effect of multidrug therapy on the levels of antibodies to Mycobacterium leprae glycolipid-1 in the leprosy spectrum.

Sera from one hundred and forty five untreated leprosy patients, ten proven cases of active pulmonary tuberculosis and twenty five healthy volunteers not exposed to M. leprae infection were assayed for PGL-1 antibodies. All available follow up samples after multidrug therapy were also assayed. A decline in the level of PGL-1 antibodies were seen in many of the post-treatment samples, giving an indirect assessment of the bacterial load and the prognosis of the disease.

Isolation, purification and regeneration of protoplasts from Sporotrichum thermophile conidiospores.

Protoplasts of uniform size were prepared from mononucleated conidiospores of Sporotrichum thermophile. Conidia were preincubated in glucose yeast extract medium at 45 C for 4 h. The conidia were collected resuspended in buffer containing 0.6 M KCl (as stabilizer), and incubated with Novozyme SP249 and Cellulase CP at 37 C for 6 h. The protoplasts were separated from cell wall fragments and intact conidia by centrifugation over 50% sucrose. The purified protoplasts were regenerated in glucose yeast extract broth after 7 h of incubation at 45 C.

An appraisal of epidemiologic, clinical, bacteriologic, histopathologic, and immunologic parameters in cutaneous tuberculosis.

Cutaneous tuberculosis incidence was recorded as 0.15%. Of the 42 patients, 23 had scrofuloderma, 17 lupus vulgaris, and 2 tuberculosis verrucosa cutis. Both men and women were affected by the disease in the second and third decades. Its duration was variable. An affirmative family history was elicited in five scrofuloderma patients. The clinical expression largely conformed to the ritual text. Variation in Mantoux test positivity was unremarkable. The disparity in the demonstration of bacilli in the smear and tissue sections was, however, quite apparent in scrofuloderma. The correlation of different parameters indicates a continuous spectrum, formed at one end by lupus vulgaris, and at another by scrofuloderma. A moderate to strongly positive Mantoux text, enormous lymphocytes in the granuloma, absence of tubercle bacilli, negative culture, and an apparently normal immunoprofile were features of lupus vulgaris; whereas scrofuloderma had a moderately positive Mantoux test, lesser number of lymphocytes in the granuloma, large number of bacilli in tissue smear and/or tissue section, raised levels of immunoglobulins, and a grossly lowered C3 levels. The other variants probably occupy a position in between.

Psychological sequelae of kidney donation. A 5-10 year follow up study.

Twenty three out of 30 potential kidney donors were followed up after a period of 5-10 years. Fourteen were donors, and the other nine subjects who were refused from kidney donation on medical grounds constituted the control group (non-donors). Psychiatric morbidity was assessed by using the Present State Examination (PSE) Interview Schedule. CATEGO-ID Program identified one case in the donor group and one case in the non-donor group. The psychiatric problems of both the cases were unlikely to be related to the kidney transplantation. The majority of the donors, irrespective of the outcome of the transplant procedure, expressed positive feelings towards kidney donation. This study suggests that the kidney donation does not cause long term adverse psychological sequlae.

Pseudoelephantiasis induced by donovanosis.

In a recent survey the incidence of pseudoelephantiasis induced by donovanosis was found to be 5%. The ratio of women to men was 3:1. The incubation period was two to 20 days, and the duration was two to 24 months. The disease was contracted through premarital or extramarital sexual intercourse. The morphological features were characterised in women by globular pedunculated verrucous surfaced swellings affecting the labia majora and the clitoris and accompanying ulceration or its aftermath (depigmentation), or both. Constitutional symptoms were absent. Intracytoplasmic Donovan bodies in the tissue or a smear from the ulcers were diagnostic. Immune markers, such as total T lymphocytes and their subsets (T4 (helper/inducer) and T8 (suppressor/cytotoxic), B lymphocytes, immunoglobulins G, A, and M, and the pivotal complement component C3 were largely inconsequential.

Lymphocyte subpopulations in donovanosis assessed by monoclonal antibodies and immunoglobulins.

The haematological profile and peripheral blood lymphocyte subpopulations of 36 patients with donovanosis were estimated by an indirect immunofluorescence technique using monoclonal antibodies. Concentrations of serum immunoglobulins (IgM, IgG, and IgA) were also estimated. The total leucocyte and lymphocyte counts were generally raised, as were total T lymphocyte, T4, and T8 counts. Patients with the hypertrophic variant, however, had decreased numbers of T lymphocytes and T8 cells and increased B cells and IgG. B lymphocyte counts and IgM and IgG concentrations were raised generally.

Voltage-dependent effects of isoproterenol on cytosolic Ca concentration in rat heart.

The effect of the beta-adrenoceptor agonist, isoproterenol, on cytosolic calcium concentration ([Ca2+]i) was studied with the Ca2+-sensitive fluorescent indicator quin 2 in enzymatically dissociated rat ventricular myocytes. Under conditions in which cells have normal polarized resting membrane potential, isoproterenol (1 microM) produced a decrease in [Ca2+]i. In contrast, in the depolarized cells (by raising extracellular K+ concentration to 50 mM), isoproterenol (1 microM) caused an increase in [Ca2+]i. This isoproterenol-induced increase in [Ca2+]i in depolarized cells could be reversed by prior exposure of the cells to the Ca2+ channel blocker, verapamil (5 microM). The results indicate that isoproterenol can either decrease or increase [Ca2+]i depending on membrane potential. The actual effect of isoproterenol on [Ca2+]i at any given membrane potential probably reflects the relative contributions of isoproterenol-induced stimulation of Ca2+ buffering or effluxing activities (which favor a decrease in [Ca2+]i) and enhancement of Ca2+ influx through voltage-sensitive Ca2+ channels (which favors an increase in [Ca2+]i).
